inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced

be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€rms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 24-48 hours, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Existing permits that require amendments (for example, to add
redecking) must be completed in person at City Hall. All other non -roofing permits
must be completed in person at City Hall. The Building Division will be open from
7:30-10:30 a.m., Monday through Friday to process these types of requests.

i) Once signed by both parties, this agreement shall become a legally binding.
ii) Replacement of bad decking shall be an additional charge of no less than
$40 per sheet. Vents, flashing etc not required by code shall be additional.
iii) This contract constitutes entire agreement. No changes or verbal otherwise
shall be part of this contract unless agreed upon by both parties in writing.
iv) Homeowner has the right to cancel this contract in writing within 3 days of
signing with no penalty.
v) Warranties are as stated above and shall be limited to exterior repairs only.
Interior damage shall not be covered under said warranties.
vi) Homeowner is hereby notified to remove all pictures, mirrors, breakables from walls or
shelves. Homeowner also understands that there may be significant amounts of dust and
debris which will fall into attic and/or garage and should cover or remove items as necessary.
vii) Afinance charge of 1.5% per month shall be applied to outstanding
balances that are not paid within 10 days of invoice, unless waiting for
insurance proceeds. Owner hereby agrees that all insurance proceeds
shall be paid to Roofcorp for work conducted under this agreement.
In the event a claim is approved and owner does not proceed with
installation or materials are restocked, owner shall be responsible to
pay a 20% cancellation/restocking fee based on replacement cost
value. In addition, owner shall be responsible for all legal, attorney,
court and collection agency fees required to collect said debt.
Said fees shall be added to total amount owed.

Conditions:
Both front and back of permit need to be posted on job site. If one or the other
is not present, INSPECTION WILL NOT BE PERFORMED. 6 nails per shingle is
required. Ice dam membrane is required from eave edge to 2' inside exterior
walls. Board sheathing with any gap greater than 1/2" requires panel sheathing
overlay on entire roof. Sheathing inspection is required prior to covering.
Contractor shall provide ladder for inspections. Scheduled inspections that
cannot be performed due to inclement weather must be cancelled prior to 6:30
a.m. on the day of inspection or a re- inspection fee will be assessed.

State of Colorado and to the Zoning Regulations and Building Codes of wheat Ridge, Colorado or any other applicable

The proposal set forth herein is SUbject to the approval of a dully authorized officer oltha Company. The Company reserves the right to change specifications where conditions indicate that such change
would be coincidental with the best practice in the industry. Since this contract calls for made-ta-order goods, it is not subject to cancellation after the statutory period has expired on the notice of "Buyer's
Right To Cancel." Damages recoverable by DUN-RITE HOME IMPROVEMENTS, INC., for failure to the Buyer to accept delivery and installation shall be the lull purchase price.
In the event that the Buyer breaches this contract by failure to make payments as specified herein or in any other respect, then the Company shall be entitled to damages in the amount of the full purchase
price, plus any other damage caused 10 the Company by said breach, including a reasonable attorney's fee for collecting the same in the event that it is necessary to resort to legal action to enforce the
Company's rights, together with Court costs and interest upon all such delinquent payments. Failure of Company to enforce any breach of this Agreement shall not be construed to be a waiver of that
condition Of any other term orcondition of this agreement. The Company may accelerate the maturity of any part or aU 01 the amount owing under this contract in the event of the Buyer's default in the
per10rmance of any of the Buyer's obligations hereunder.
The Company assumes no responsibility for meeting any requirements or specifications except as set forth herein, and upon completion in accordance with these specifications, all payments provided
for herein shall be due and payable. No credit or allowance will be made by the Company for any alteration in this agreement, except such changes as shall have been consented to in writing and approved
officia\ly by the Company.
The Company shall not be liable for any delays caused by strikes, weather conditions, or delays in obtaining materials and has the right to substitute materials of equal Quality if necessary for reasons
beyond Company's control.
The Buyer acknowledges thaI: THE ABOVE CONTRACT CONSTITUTES THE ENTIRE AGREEMENT BETWEEN THE PARTIES AND THAT NO PROMISES OR REPRESENTATIONS HAVE BEEN MADE
WHICH DO NOT APPEAR IN SAID AGREEMENT. Further, that no rebate or discount and/or offer to pay value was given to the Buyer as an inducement forthe sale in consideration of the Buyer's offering
names of prospective purchasers forthe purpose of referral sales. Discounts or price reductions not agreed to and indicated herein shall not be considered in this agreement, and any such announcements
