Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and maybe subject to a fee equal to one-half of the original permit fee.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE

Dear Mr. Senna:
In response to your request, Iris Technologies, LLC investigated the site associated with the
above-noted claim on October 20, 2003. The results of the investigation, which include site
observations and conclusions, are presented herein.
PROJECT BACKGROUND
The insured's garage is a detached two-story, double wythe brick masonry structure with a
conventionally framed gable roof. The structure was built in the late 1800s and was originally
constructed as a barn. For the purposes of this report, the front of the garage is considered to
face west.
The severe snowstorm of March 2003 reportedly deposited approximately 4 feet of wet, heavy
snow of the roof of the insured's garage. Several months after the snowstorm, a neighbor
informed the insured that the garage roof was sagging. The insured then noted that the ridge of
the garage roof was severely bowed and the faces of the roof structure exhibited significant
deflection. The insured then discovered several twisted and broken members in the enclosed
attic space above the garage, and gaps were noted between the edge of the garage roof and the
exterior walls at the front and back of the structure.
PERTINENT OBSERVATIONS AND PROJECT INFORMATION
The following issues are based on observations made by the investigator during the site survey
and information obtained through interviews with the insured:

1. The floor of the attic space over the garage is constructed with I-inch by 6-inch tongue and
groove planking laid over 2-inch by 10-inch dimensional wood floor joists. There are two
interior vertical posts that serve as supports for the ridge of the roof. One of these ridge
supports has broken through the attic floor. The plank that broke had no means of
structural support beneath the attic area.
1.1. The board dropped through the attic floor approximately 4 inches and now bears on a
large structural beam that spans the lower garage ceiling.
1.2. The ridge board remained connected to the vertical support, resulting in the
downward displacement of the ridge by approximately 4 inches at its connection
point.
2. The vertical I-inch by 4-inch roof rafter supports are bowed and cracked. Only one toe-
nailed fastener connects the ends of the inadequate supports to the roof rafters and attic
floor boards.
2.1. Most of these vertical supports have caused the supporting attic floorboards to deflect
downward. There is little additional support beneath the vertical roof supports,
resulting in a downward deflection of the attic floorboards by approximately 1 inch
without breaking.
3. Some of the ridge connections have separated from the ridge board.
3.1. Although the 2-inch by 4-inch rafters exhibit little deflection and no flexural cracks
were found, many of the support members have separated from the lx4 ridge board.
The nails have pulled away at the ridge connection causing the ends of some rafters to
split.
4. The top plates of the exterior walls have deflected outward.
4.1. The 2xl0 top plates along the exterior bearing walls have deflected outward by as
much as 5 inches. There is a noticeable gap between the top plate and the top of the
exterior brick walls.
4.2. The top plates are attached to the top of each brick walls by only two bolts. Further,
the top plates are spliced near the mid-span of the exterior walls, and the splices have
separated. One bolt has pulled loose from the brick material on each side of the
structure.
4.3. Structural calculations based on the design of the insured's garage indicated that once
the vertical ridge support failed, the roof rafter assemblies could have deflected 5
inches horizontally. Since some of the top plate bolt connections failed, and the top
plates separated at their splice connection, there was little horizontal restraint to hold

the top plate in place.
5. There are several cracks in the exterior brick walls that appear old and are thought to be
unrelated to the recent deflection of the roof structure.
5.1. There is a significant accumulation of dirt and debris within the cracks, indicating that
these fractures occurred prior to the March 2003 event. Also, the location and pattern
of the cracks are not consistent with lateral loads imposed by excessive roof loads.
5.2. The exterior walls of the garage are not bowing outward. There are, however, a few
areas, particularly along the top of the east wall, that are leaning inward. These
sections of the exterior brick walls appear to have been originally built with these
observed irregularities.
CONCLUSIONS AND RECOMMENDATIONS
Based on the site investigation, it is the opinion of Iris Technologies, LLC that the damages
observed to the garage roof occurred when the inadequately constructed roof system deflected
and failed under the weight of the March 2003 snow load. The excessive weight of the snow
caused the roof system's vertical support system to fail, which in turn overstressed and
damaged the exterior wall top plates. The exterior brick structure, which exhibits signs of
previous distress, were not damaged during the March 2003 event.
Due to the extent of the observed damages, it is believed that repair of the existing roof system
would be cost prohibitive. Hence, it is recommended that the roof system over the garage be
removed and replaced with engineered trusses. An engineered truss system would be more
cost effective, would relieve the horizontal pressures exerted on the walls by the existing roof
configuration, and is easy to install. Further, attic trusses can be designed to maximize the
usable space within the existing attic space. New exterior wall top plates will also have to be
constructed and new bolt connections drilled and grouted into the existing masonry walls.
The design and construction specifications for engineered trusses can be obtained through a
representative of a qualified truss manufacturer; Iris Technologies, LLC can assist in identifying
a qualified truss design professional and coordinating the required specification and installation
efforts upon request.
All comments made in this document are based on the conditions observed at the time of the
site investigation. Inasmuch as the investigations and/ or site assessments of an existing
structure/ facility requires that certain assumptions may not be verifiable without expending
additional sums of money or destroying otherwise adequate or serviceable portions of the
property, and that unknown or unknowable conditions may exist within the structure/facility,
the homeowner/agent agrees by acceptance of this report, except for negligence on the part of
