Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Scope of work: Your insurance company has provided an estimate known asthe Sc\ope of Loss (SOL). This
SOL will serve as the interim scope of work and line items to be performed on the property, until such time as
amended and/or supplemented by us. Any line items we do not perform will be credited back to you at the
ACV value and not billed to the insurance company at completion. We are not obligated to perform any work
not approved by the carrier unless you authorize the work through a change order and pay for the upgrade or

Roofing quality control: We install total "Roof Systems" that your rep will discuss. Remove existing layers of
roofing to the decking and dispose, inspect for damage and repair any damages at time and material. Felt
paper will be applied to the decking as specified. Ice and water shield installed (if Applicable) as specified on
eaves, valleys and around penetrations. Flashings, drip edge and counter flashings installed as specified. Pipe
jack boots will be installed using galvanized no -caulk collars and IWS. Roof ventilation will coincide with our roof
system recommendations.

We then submit these changes to the carrier for payment so that your repairs are completed per code and at the highest level of quality
the carrier will allow. These "supplements" will add to the total contract and be due to us as your contractor.

1. If the property owner plans to use the proceeds of a property and casualty insurance policy to pay for
the roofing work, pursuant to §6-22-105, the roofing contractor cannot pay, waive, rebate, or promise to
pay, waive, or rebate all or part of any'insurance deductible applicable to the insurance claim for
payment for roofing work on the covered residential property. You may cancel the contract and obtain a
full refund of any deposit within seventy-two hours after entering the contract. In addition, you may
rescind a roofing contract pursuant to C.R.S. § 6-22-104.
2. If this contract is cancelled by the customer later than seventy-two hours from receipt of the written
notice identified in § 6-22-104 or if the customer cancels the contract for a reason other than the receipt
of written notice identified in § 6-22-104, the customer shall pay to S&S C.S. (20%) of the contract price
as liquidated damages, not as a penalty, and the S&S C.S. agrees to accept this amount as a reasonable
and just compensation for cancellation and the efforts that SRS C.S. expended in relation to the subject
of this contract.
3. If material has to be reordered or restocked because of a cancellation by the customer other than permitted
by §6-22-104, you will be responsible for any restocking fee charged to S&S C.S. All excess materials belong
to the Company.
4. S&S C.S.'s liability insurer is The Cincinnati Insurance Companies, 6200 S. Gilmore Road, Fairfield, OH
45014-5141 Mailing address: P.O. Box 145496, Cincinnati, OH 45250-5496.
5. Company shall not be liable for the failure of performance due to labor controversies, strikes, fire, weather,
inability to obtain material from usual sources, acts of God, or circumstances beyond the control of the
Company.

deteriorated decking, roof jacks, ventilators, flashing, or other materials and will be charged on a time and
material basis if needed.
8. Company's warranty does not cover damage to roofs caused by lighting, gale (50 MPH), hurricane, tornado,
hailstorm, foreign objects, or damage to roofs due to the settlement, distortion, failure or cracking of the roof
deck, walls, or the foundation of a building.
9. Company is not responsible for any damage below the roof due to leaks caused by excessive wind, ice, or
hail during the period of the warranty. The Company's warranty covers roof only. This contract and labor
warranty shall not be assigned and are nontransferable unless specified in writing.
10. Company willl not be held liable for any structural movement of the foundation or walls, settling, cracks in
dry -wall, cracking or breaking of the driveway during the roofing or loading processes, or minor scratching
or denting of rain gutters.
11. All pictures, wall hangings, lights, and collectables, both indoors and out MUST be secured by
homeowner. Any damage to personal items during installation will not be the responsibility of the
Company.
12. Customer will not receive a labor, material, or manufacturer warranty if the contract is not paid in full. Any
discounts, sign allowance, referral fees, or the like will be revoked if account is past due.
13. Customer has the sole responsibility to provide safe -keeping areas for all pets. Company shall not be
responsible for gates left open during or after Company's work
14. Homeowner is solely responsible for Homeowners Association (Covenant approval) of roofing products and
color.
15. Company is not responsible for damage to stucco, siding, or wall abutment due to re -flashing. Company is
not responsible for satellite dish realignment.

This agreement is the entire agreement by the parties. Any representation, statements, or other
communications not written in this contract are agreed to be immaterial, and not relied on by either party,

RCV Coverage: A replacement cost value policy applies the value to replace the property with other property of comparable
quality and used for the same purpose. In contrast, if you have an actual cost value policy or roof surface coverage that
deducts an amount from the replacement cost value based upon the age of the roof or property, you will be responsible for
the difference between the replacement cost value and the insurance company's deduction for actual cash value or the roof
service coverage. The final scope agreed between the insurance company and S&S G.S. shall become the final contract
price for the insurance approved scope. You will be responsible to pay for any upgrades or additional work authorized by
you that the insurance company determines is not within the scope of coverage provided by your policy. Supplemental
claims billed by S&S C.S. that are approved by the insurance company for additional work or increases in costs become a
part of this contract.
Warranty: S&S C.S., expressly warranties its installation hereunder to be free from defects in workmanship for a period of 5
(FIVE) years. Our obligation under this warranty shall be limited to any necessary labor if, within the warranty period, any
portion of our work shall be demonstrated to have been improperly completed. If Owner shall make or permit, without prior
written consent from S&S C.S., repairs, alterations or additions to the roof which affect the Work, this Warranty shall
become null and void and of no further effect effective as of the date of commencement of any such unauthorized repairs,
alterations or additions.
S&S Construction Services, LLC will respond promptly and reasonably to any written request for consent for alterations or
additions. This warranty specifically excludes damage caused by third parties, damage from ice damming, condensation, acts of
God, or damage caused by other trades during or after the completion of our work. Owner hereby acknowledges receipt of the ice
damming information package and is aware that this warranty shall be void in the event roof repair, maintenance or snow
removal is not performed by S&S C.S.
This warranty is in addition to the manufacturer's warranty for materials and is expressly in lieu of all other warranties
express or implied. Correction in the manner provided above shall constitute a fulfillment of all liabilities of S&S C.S.

EXCLUSIONS: Snow removal, interior flashing, interior downspouts or drains at flat roof and any decks, door and

representative should have protection of interior of residence from any fallen dust and/or debris caused by the roof
replacement. Mold and asbestos removal or mitigation, or patina. Saw cutting, mock ups, shop drawings ice damage or
liquid damages, bonds, permits and fees for such.
Unless specifically noted above, there are no additional terms presumed to be included in this proposal.
ALL CHECKS FOR PAYMENT MUST BE MADE PAYABLE TO S&S CONSTRUCTION SERVICES, LLC ("COMPANY").
HOMEOWNER WILL BE LIABLE TO S&S C.S. FOR PAYMENT OF THIS CONTRACT IF CHECK IS MADE OUT TO ANY OTHER
ENTITY OR PERSON. S&S CONSTRUCTION SERVICES, LLC WILL ADVISE YOU IN THE PROCESS OF SUBMITTING
INSURANCE CHECKS TO BANKS OR MORTGAGE COMPANIES WHEN REQUIRED. AT TIMES THESE ENTITIES WIL REQUIRE
DOCUMENTS FROM US AS YOUR CONTRACTOR IN ORDER TO RELEASE PAYMENT. S&S CONSTRUCTION SERVICES, LLC
SHALL HOLD IN TRUST ANY PAYMENT FROM THE PROPERTY OWNER UNTIL THE ROOFING MATERIALS HAVE BEEN
DELIVERED TO THE PROPERTY.

REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.
