Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an. approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding A/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€ms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and

This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must
be made in writing and received prior to the date of expiration. An extension of no more than 18o days made be granted
at the discretion of the Chief Building Official and may be subject to a fee equal to one- half of the original permit fee.

Per your Request, we performed limited visual observations at the above referred residence on April
9,2008, The purpose of this visit was to observe and evaluate the foundation wall cut to enlarge the
window at the northwest corner of the basement of the above referenced home, For purposes of this
report, the home assumed to face east.
Site Observations:
During our visit we noted the following:
I, The contractor was in the process of enlarging the north widow in the west basement
foundation wall ofthe home, The width of the window appears to have remained unchanged,
but the window was being made taller by cutting the foundation wall beneath the existing
window opening,
2, It appears that the contractor was able to keep the overcuts at the bottom corners of the
window to a minimum,
3, It does not appear that any reinforcing steel was cut during the process of enlarging the
window opening,
4, The wall appears to retain approximately 5' -0" of soil.
5, We observed no significant foundation wall or brick distresses in the immediate vicinity of the
noted window,
6, The basement slab, at the northwest corner of the home, does not appear to have experienced
significant vertical movement.
Conclusions:
Upon completing a structural review ofthe above noted site observations we have concluded the
following:
I. It appears that the foundation wall cut being completed to enlarge the north window in the
west foundation wall is being completed in a standard workmanlike manner.
2, It does not appear that the enlargement of the noted window will adversely affect the load-
bearing capacity of the foundation system at the northwest corner of the home and the
modified potion of foundation should be capable of providing adequate support for the applied
loads,

original plans and specifications and any suspension or abandonment has not exceeded one (1) year. If changes are made or if suspension or abandonment
exceeds one (1) year, full fees shall be paid for a new permit

Contractor shall notify the Building Inspector twenty-four (24) hours in advance for all inspections and shall receive written approval on inspection card before
proceediing with successive phases of the job.
