Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.

subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

This Constru tion Contract is entered into this day of by and between AG Contractors. Inc. ("Contractor")
and ("Owner"), for the constructs of an addition to the property
located at

CONTRACT DOCUMENTS. This Construction Contract consists of the following documents (which
are collectively referred to as the "Contract"):
1. The Construction Contract.
2. All drawings, blue prints, plans, specifications (collectively "Specifications") which are now or by
later agreement of the parties attached to this Contract. The initial Proposal is attached as Exhibit "A.'
3. Any change orders in the form attached hereto later executed by both of the parties to this Contract in
the form attached as Exhibit "B."
4. Any supplemental Contract documents, modifications or Change Orders executed by both parties after
the date of the Contract.
The construction and services required by the Contract Documents shall be referred to as ""Work"," and
includes all labor and equipment provided or to be provided by the Contractor to fulfill the Contractor's
obligation. Contractor represents that the "Work" and Initial Specifications include all trench and utility
"Work" as may be required by the County and/or City regulations, and/or Energy Company regulations,
to relocate the electrical lines and/or any parts necessary to prove electrical service to the property,
including the addition and all existing parts. The "Work" may constitute the whole or part of the
"Project." The amounts agreed upon to perform the "Work" shall be referred to as the "Contract Price."
START DATE, COMPLETION DATE. The Contractor shall begin the "Work" upon execution of this
Contract and will be completed by . If the Contractor is delayed at any time in
the progress of the "Work", Change Orders, Owner's failure to make timely progress payments, fire,
adverse weather conditions not reasonably anticipated, unavoidable casualties, or any other causes beyond
the Contractor's control, then the Completion Date will be extended by a Change Order for a reasonable
period of time as the parties may determine by their written agreement.
CONTRACT PRICE. Owner agrees to pay the Contractor for the con truction of an addition as
specified by the Contract Documents the sum of $ 121 /17_.-Z/,. Z as set forth herein.
PAYMENTS. Thi Owner will make progress payments to the Contractor for "Work" as follows:

FINAL PAYMENT. Final payment for the unpaid balance of the Contract Price shall be paid by the
Owner no more than 5 business days after the Completion Date. Simultaneously with Final Payment, the

LATE PAYMENTS: Owner agrees that any payments not made in accordance with this schedule shall
be considered delinquent after ten days and agree to pay interest thereon of 11 '/2 percent per month (or
the highest amount allowed by state law, 24.9 percent annually) until paid.
CREDIT CARD PAYMENTS: Subject to the following additional charges: 2.44% of Total Price.
ALLOWANCES. The Contract Price includes Allowances as provided in the Contractor's Proposal, and
any supplemental modifications or change orders (the "Allowances"). Contractor shall inform Owner of
the balance remaining toward Allowances as change orders are processed. If Owner exceeds any of the
Allowances, Owner shall pay to Contractor at the time the Contractor purchases any item which has an
Allowance the amount by which the cost of such Allowance Item exceeds the applicable Allowance. If
the cost of any Allowance Item is less than the applicable Allowance, then the Contract Price shall be
reduced accordingly.
OWNER'S RESPONSIBILITIES. If agreed by the Owner and the Contractor, the Owner shall secure
and pay for all necessary approvals, easements, assessments and charges required for the construction, use
or occupancy of permanent structures or permanent changes to existing facilities.
PERMITS/APPROVALS. Builder shall obtain and pay for any and all permits or licenses required
under any federal, state, or local laws for performance of the "Work". Builder shall make all notifications
required under any federal, state, or local laws for performance of the "Work". Builder shall obtain all
required approvals from any federal, state, or local authority having jurisdiction over the Project, except
for the approval of any homeowners' association or similar organization or group. The Owner shall be
responsible to obtain the approval of any homeowners' association or similar organization or group.
SUBCONTRACORS. The Contractor will have sole discretion as to who he hires for subcontracted
"Work" (a subcontractor is defined as any person or entity who has a direct contact with an authority from
the Contractor to perform any portion of the "Work") and shall be solely responsible for the conduct and
performance of such subcontractors.
INTEGRATION CLAUSE AND CHANGE ORDERS. This Construction Contract shall constitute the
complete expression of the intentions of the parties with respect to the Project and may not
construe, modify, or alter except by a writing agreed upon by the parties. Subject to the
provisions herein, the "Work", Contract Price and Completion Date may be modified only by a properly
executed Change Order (a copy of which is attached hereto as Exhibit "B") All Change Orders must be
signed by the Owner and the Contractor to be binding upon the parties.

QUALITY OF "WORK". The Contractor will complete, the "Work" according to the Contract
Documents. Upon substantial completion of the "Work", all "Work" that in the Owner's reasonable
opinion is not yet complete or which falls to meet Contract requirements will be specified in a punch list
executed by the Owner and submitted to the Contractor no later than 30 days following notice of
substantial completion. The punch list items will be promptly corrected by the Contractor. If the
Contractor believes, in his reasonable opinion, that the "Work" meets Contract requirements, he will
inform the Owner within 30 days of his receipt of Owner's punch list items and the parties shall make
reasonable efforts to reach agreement as to the disputed punch list items.
The Contractor warrants that the "Work" will be free from defects in workmanship. Upon Owner's
request, the Contractor will take reasonable steps to obtain for the Owner's benefit, and assign to Owner if
necessary, all manufacturers warranties applicable to the materials or equipment installed by the
Contractor or any of its subcontractors, if such warranties exist.
CHANGES 1N THE "WORK". Owner may with the consent of Builder make changes in the "Work"
within the general scope of the Specifications. However, no changes are to be made except upon a prior
written "Change Order" (signed by both parties) consisting of the change, any additional cost or credit,
the Builder's Fee and the additional number of days to be added to the Completion Date, if any. If the
change reduces the cost, Owner will receive a credit, but Builder's supervision and overhead expenses and
profit will not be reduced. The Builder's Fee will be added to all change orders. This is an administrative
fee to cover time spent organizing, implementing and supervising a change. Either Owner may sign the
change order on his or her own behalf and on behalf of the other, or the signature shall be binding as if
both signed the change order.
SUBSTITUTION OF MATERIAL. Builder reserves the right to (a) make changes or substitutions of
materials of equal value or quality with notification to Owner and Owner's approval, (b) determine in
consultation with Owner the grading and elevation of lot, and (c) determine in consultation with Owner
the elevation of foundation.
INSURANCE.. Contractor has, and will continue to maintain insurance coverage sufficient to protect the
Contractor from the claims of workers under the Worker's Compensation Act and other employee benefit
acts for damages because of bodily injury, including death and from claims for damages other than to the
"Work" itself, to property which may arise or as a result of the Contractor's "Work" under this Contract
whether such operations are conducted by Contractor or by any subcontractor or anyone directly or
indirectly employed by the Contractor or any subcontractor. Owner shall be responsible to maintain
insurance against casualty and theft involving the materials that are or will be incorporated in the "Work"
while located on the Project site or the structure that is the subject of the "Work".
TAXES. Unless otherwise provided herein, the Contractor shall pay sales, consumer, use and other
similar taxes that are legally enacted as of the effective date of this Contract, whether or not yet effective
or merely scheduled to go into effect.
DE FAULT. If the Owner fails to make any required payment for a period of 30 days after it is due, the
Contractor may, upon seven additional days written notice to the Owner, terminate this Contract and
recover from the Owner payment for all "Work" performed and for all materials furnished to the Project

and for the use of all construction equipment and machinery in the performance of the "Work" including
reasonable overhead expenses, profit and damages applicable to the "Work".
If the Contractor (1) is judged to be bankrupt or insolvent, (2) makes a general assignment for the benefit
of its creditors, or if a trustee or receiver is appointed for the Contractor or for any of its property, (3) fails
to make prompt payments for undisputed amounts due to its subcontractors for labor and materials or
equipment, (4) disregards laws, ordinances, rules, regulations or orders of any public body or public
official having jurisdiction, (5) otherwise materially violates any provision of this contract, the Owner
may provide Contractor written notice of such default and, if such default remains uncured for a period
exceeding 30 days, the Owner may terminate the Contract and take charge of the site and complete the
"Work". In the event a lawsuit is filed for any default of the Contractor, Owner shall be entitled to an
award for Owner's attorney's fees, court costs, and other such fees incurred by Owner in bringing such
suit.
GOVERNING LAW. This Contract is governed by the laws of the State of Colorado.
JURISDICTION AND VENUE. Any claim relating to this Contract must be filed in a court having
subject matter jurisdiction over such claim located in County, State,
and any such court shall have exclusive jurisdiction over such claim. The parties consent to the personal
jurisdiction of said court to resolve any claims relating directly or indirectly to the Contract.
RESTRICTION ON ASSIGNMENT. Neither the Owner nor the Contractor may assign this Contract
without the express written consent of the other party, which consent shall not be unreasonably withheld.
FACSIMILE OR ELECTRONIC SIGNATURE. The Construction Contract may be executed by
facsimile or electronic signature.
LEAD PAINT DISCLOSURE: Owner's Indemnification of Contractor for EPA's Renovation, Repair,
and Painting Final Rule (40 CFR 745): In the event that Owner does not provide AG Contractors, Inc.
(Contractor) copies of documents proving that all occupants are free of all preexisting conditions
associated with lead exposure, Owner shall indemnify and hold harmless Contractor and their agents,
subcontractors, and employees to the fullest extent permitted by law, from and against all claims,
damages, losses and expenses. including but not limited to attorney's fees, arising out of or resulting from
the perfonnance of Contractor's duties under this Agreement associated with the EPA s Renovation,
Repair, and Painting Final Rule (40 CFR 745). Such obligation shall not be construed to negate, abridge
or otherwise reduce any other right or obligation of indemnity which would otherwise exist as to any
party described in this Estimate or Provision Statement.
FEDERAL ALLOWANCE: The owner is hereby advised that if the project is the owner's homestead,
federal law allows the owner to terminate the contract created when the owner signs this contract for any
reason within three (3) days after signing it.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
