Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding k/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€1Ms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

I. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This.permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalermit fee.
3, If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and_procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, any violation of any provision of any
applicable code or any ordinance or regulation of this jurisdiction. Approval of work is subject to field inspection.

Dear Ms. Willis:
At your request KLP Consulting Engineers, Inc. has visually reviewed the above referenced address for
a potential addition of an upper floor. The house was built around 1956 and is a ranch style house with
a crawlspace, with the south end of the house being on a slab-on-grade. The house faces east. The
exterior structure consists of wood frame walls with a brick veneer that is supported by a concrete
foundation wall and conventional spread footing. There appears to be a center load-bearing wall that
runs through the house, which is evident by the multi-wythe brick wall in the basement.
The proposed addition is approximately twenty-feet wide by forty four-feet deep and will be above the
south end of the house, and will be constructed over foundation walls on all four sides. It is proposed
for the existing roof structure in this area to be removed and a second story constructed.
Some minor cracking was noted in various areas of the ceiling. The southwest comer of the structural
has experiences some very minor settlement, which appears to be caused by poor drainage on the south
side.
There is an intermediate foundation wall approximately twenty-feet north of the south end of the house,
which has a vertical crack approximately four feet in length.
Given the age of the house the structure is in very good condition.
Based on this brief visual review and to the best of our knowledge, we make the following comments:
I. Based on the second floor addition, the above noted conditions are structurally adequately
to support the additional loading.

2. It is recommended that the crack in the intermediate foundation wall approximately twenty-
feet north of the south end of the house be epoxy injected.
3. Improve the landscaping at the south side of the house to provide positive drainage away
from the foundation.
4. The gutter and downspouts should be check and cleaned if necessary and checked
periodically to ensure that proper discharge is maintained.
In as much as the site review of an existing structure for the purpose of observing the structural
conditions requires that certain assumptions be made regarding existing conditions, and because some
of these assumptions may not be verifiable without expending additional sums of money or destroying
otherwise adequate or serviceable portions of the building, the Client agrees that, except for
negligence on the part of the engineer, the client will hold harmless, indemnifY and defend the
Engineer from and against any and all claims arising out of the professional service we have provided.
