On September 23`d, 2020 at approximately 12:00 p.m. a site observation was conducted for the purpose
of observing the post -to -beam hardware installed by Services Rendered, Inc. at the above referenced
address. This was conducted by Dillon Fox, P.E.
1. 5" 90 degree OWT LITE flush outside, 12-guage steel plates were observed to have been
installed to attach the patio cover roof beams to the supporting columns. The plates were
attached per the manufacturer's specifications. This hardware is adequate for the applied loads
and use.
Please provide this letter as proof of observation by a licensed engineer.

observing the depth, width, and anchor bolt embedment of (4) concrete caissons by Ser.'icc-- pc -a__ -=a,
Inc. at the above referenced address. The observation was conducted by Dillon Fox, P.E.
1. (4)- 24" diameter caissons were observed to be dug to a depth of 36" before concrete was
poured.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding k/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturericlms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.
