Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

1. REVISIONS: This Contract constitutes the entire understanding of the Owner and Contractor with respect to the Work, and no other understanding, collateral or otherwise,
shall be binding upon either of them unless memorialized in a separate written document signed by both of them.
2. CHANGE ORDERS: Replacement of Owner's deteriorated decking, fascia boards, roof jacks, vents, flashing, or other material, unless otherwise expressly stated in this
Contract, are not included in the figures contained within the front side of this Contract (hereinafter referred to as the "Figures"), and will be charged to Owner by Contractor,
and paid by Owner to Contractor in addition to the Figures, on a time and materials basis and pursuant to a written change order signed by owner and Contractor.
3. PAYMENTS:All payments shall be made by Owner to Contractor at Contractor's address listed on the front side of this Contract. Any warranty on the services provided by
Contractor in connection with this Contract, whether expressed or implied, are not effective unless and until the Owner has paid the Figures plus the sums contained within
any change orders due and payable by Owner to Contractor under this Contract have been paid in full.

4. CASH SALES: All monies are due and payable upon completion of the Work. Any amounts left unpaid after 15 days will be subject to lien and will incur late charges at
1.8% per month for open balances, retroactive to date of completion.
5. COLLECTIONS: Owner shall be liable to Contractor for all of Contractor's costs of collection, including, but not limited to, all court costs, expert witness fees, attorney's
fees (both hourly and contingency fees) and all collection agency fees, in the event that Contractor seeks to enforce this Contract against Owner, whether or not Contractor
institutes a lawsuit against Owner.
6. WARRANTIES: Contractor warrants all roof replacements performed by it with a 5 -year workmanship warranty. Repairs to any roof replacements are covered by a 90 -day
workmanship warranty. Any labor warranty, express or implied, given by Contractor to Owner in connection with the Work, is void in the event that the Owner's roof is
subjected to gale force winds (70 MPH); lightning striking the roof or any surface adjacent to it; hurricane; tornado; hail damage; impacts of foreign objects; other violent
storm or casualty; or damage to the Owner's roof due to settlement, distortion, failure or cracking of the roof deck, wall or foundation of the Owner's home.
7. LABOR WARRANTY: The Owner and Contractor agree in the event of breach of any warranty, either expressed or implied, by Contractor for the Work, the liability of the
Contractor shall be limited to the labor and material costs of replacing any such defective portion of the Work. The Contractor shall not be liable for any other damages
incurred by Owner, whether direct or consequential.
8. MATERIAL WARRANTIES: Owner agrees that material warranties are limited warranties issued by material manufacturers and are subjected to terms and limitations from
each manufacturer. Materials used for the Work are covered exclusively by the manufacturer(s) of such materials, and depend on product selection. Contractor will submit a
claim on the Owner's behalf with the manufacturer of any such Materials after Owner provides it with notice as to same, and will complete warranty repairs per any
manufacturer's request and approval.
9. DAMAGE: The warranty provided by Contractor to Owner for the Work, applies to Owner's roof only. Contractor is not responsible for any damage to interior, rafters,
and/or any and all structures of the Property resulting from Contractor's removal of excess layers and/or the vibrations caused by Contractor during its performance of the
Work. Contractor is not responsible for any water damage to the interior of the Property or its contents due to water leaks from the Owner's roof after the Work has been
performed. In the event of water leaks during the warranty period, Contractor liability shall be limited to labor and material costs of repairing defective Work on Owner's roof
only. Contractor shall not be liable to Owner for any cracks or damage to driveways, walkways, or sidewalks, whether or not caused by Contractor and/or its Sub -Contractors.
Contractors shall not be liable for any bowed, cracked, or sagging rafters during or after the Work is performed, whether or not caused by Contractor and/or its
sub -Contractors, or caused by the material supply company.
10. RESCISSION: Owner has the express right to rescind this Contract and obtain a full refund of any deposit paid by Owner to Contractor in connection with this Contract,
within 72 hours after Owner signs this Contract. In the event a contract is cancelled by Owner after this 72 hour period, Owner shall remit 25% of Contract price as stated on
this Agreement.
11. INSURANCE CLAIMS: By signing this contract I do hereby give Limited Power of Attorney to Formula Roofing & Remodeling, authorizing my insurance company to
speak with my contractor Formula Roofing & Remodeling on my behalf with any and all dealings regarding the current claim listed in this contract. Limited Power of
Attorney is to be withdrawn when Formula Roofing & Remodeling is paid in full. Formula Roofing & Remodeling will bill my insurance company for all work, including
code required work necessary, which, in most cases, is payable by my insurance company. The insured, will not be responsible for any monies not paid by the insurance
company except for the deductible and optional upgrades listed on contract, unless notification is made prior to work being completed, or in cases of "No Code Coverage" or
"Non -Recoverable Depreciation" on my insurance policy. Code items are required to comply with city/county code and are not optional. Formula Roofing & Remodeling will
request the final inspection with the appropriate City/County Authority. Any additional funds allocated for additional work as part of this claim will be payable to Formula
Roofing & Remodeling once the work has been completed and funds have been released. Contractor shall hold in trust any payment received from Owner until Contractor has
delivered roofing materials to the site or has performed a majority of the roofing work on the property, whichever occurs first.
This estimate may be withdrawn if not accepted within 30 days.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if

Dear Ray and Jennifer:
On December 8, 2008 I inspected the pier installations at the above referenced property. The pur-
pose of the inspection was to determine compliance with the repair design dated October 21,
2008. At the time of inspection it was observed that the steel piers had been installed in conform-
ance with the repair design. Pier depth and pressure charts were provided by the contractor. In my
opinion the piers were installed according to the repair design and approval was given to the con-
tractor for backfilling the excavations. Also observed was the shimming of the central beam,
grouting of the sill plate, and installation of the moisture barrier in the crawlspace. All work was
in conformance with the repair design:
You indicated concern with the rear sliding glass door. It is apparent that the door opening is
somewhat out of square, which may be the result of minor foundation movement and/or pressure
from the exterior deck on the rim joist. In any event, some foundation movement is to be ex-
pected given the shallow type of foundation and changing climate/moisture conditions. The
movement is not sufficient to warrant structural intervention, however. At this time, you may
want to have a door installer reset the jambs and threshold to improve the closure of the door.

the inspe ation of the home to determine my professional opinion
of the strl ° nspection did not involve any other system or component
of the res tl sampling, testing, or analysis was performed.
General:
• The hot nine structure, assumed to be facing West. The home was
reportedly constructed in 1950. The foundation is of poured concrete construction, supported
on spread footings, enclosing a crawlspace and the original attached garage that has been con-
verted to living space. Central bearing for the home is via wood beams and concrete piers.
The detached garage was not inspected.
• No soils report or other engineering documents were available for review.
• I understand that you have owned the property for 5 years and that the sewer was replaced at
the time you acquired the property.
Observations, Opinions and Recommendations:
The slope away from the foundation appears to be deficient by Soils Engineering Standards, at
the front fireplace area, the North side, the rear near the NE corner, and at the South side be-
tween the foundation and the driveway. The drainage condition below the rear deck could not
be determined, and the disposition of the downspout extender at this location is unknown. It is
recommended to expose this area and improve as needed per the notes below. Poor drainage
allows excessive moisture to infiltrate into the foundation soils, which can cause structural
damage and/or moisture penetration into the structure. Soils Engineers typically recommend
12" of fall in the first 10 feet away from the foundation, though 6" fall in the first 5 feet is al-
lowed in some cases as a minimum. Thereafter landscape areas should have a minimum slope
of 1/4" per foot. Downspout extenders should discharge at least 5 feet from the foundation. Ir-
rigation within 5 feet of the foundation should be limited to drip systems. The downspout ex-
tender at the SE corner discharges into the flower bed between the foundation and the drive-
way. This roof drainage cannot escape this area and infiltrates into the foundation soils. Some
minor settlement at the SE corner was observed that is associated with this condition.

• Add and compact soil adjacent to the foundation to provide a minimum fall of 6" in the
first5 feet. However, care must be taken notto,raise the soil too high relative to sid-
ing/framing. Several inches of clearance should be maintained (most building codes re-
quire 6" clearance to deter moisture and pest access). At the South wall, consider instal-
ling concrete between the foundation and the driveway. In the front yard it is recom-
mended to remove the sidewalk. Improve all downspout extenders per the above notes
and eliminate sprinkling near the foundation.
• The upper structure exhibits evidence of foundation movement and movement of the central
beam system. These movements have caused plaster cracking, floor sloping, and doorway dis-
tortions. I understand that recent movement has been observed in the front fireplace area.
• In the,crawlspace, foundation wall cracking and effloresence was observed. The cracking is
the result of minor foundation movement over the years; undoubtedly associated with the poor
drainage. Similarly the effloresence is indicative of moisture penetration resulting from the
poor drainage. Making the recommended drainage improvements will help to stabilize the
foundation movement and deter moisture penetration. However, at the front wall fireplace
area, the movement is more significant and deserving of structural stabilization at this time.
• Foundation stabilization against vertical movement is recommended at the front wall
fireplace area, via installation of steel piers per the attached diagram. Some foundation
realignment may be possible using this method but should not be assumed.
• The central beam has experienced deflection caused by movement of the support piers. This
movement is largely responsible for the floor sloping, plaster cracking and doorway distor-
tions noted in the upper structure. It is recommended to adjust the beam to level, as much
as possible, via adjusting the shims between the beam and the piers.
The foundation sill plate has been shimmed, either at the time of construction or subsequently,
in order to level the sill plate. This is typical construction practice and includes adding grout
to seal the gap between the sill plate and the foundation created by the shimming. However, in
some areas the grout is missing. It is recommended to regrout the sill plate wherever the
grout is missing.
• The c'rawlspace has a partial moisture barrier. It is recommended to complete the moisture
barrier, including sealing all overlaps and sealing the barrier to the foundation.
• It is assumed that the new steel piers will achieve penetration into bedrock and can be in-
stalled to refusal. Should bedrock and refusal not be reached within reasonable depth, this of-
fice should be notified for further instruction.
• Care needs to be taken in the areas of underground and overhead utilities. All work to be done
in compliance with local and national safety standards, and building department regulations.

• This inspection was visual and cursory in nature, but represents the professional opinions of
this firm for the area in question.

· Before you begin nailing, layout your lumber with the best-looking face exposed. Decide which pieces
you want for visible areas, and which pieces for understructure.
· Separate deck boards as follows to allow for expansion and contraction. If heavy and wet, separate boards
no more than 1116" as some shrinkage will occur. If light and dry, separate boards no more than 118".
· Avoid long spans in consttuction The greater the
distance between supporting points, the more force
developed within the wood as it dries. Also avoid de-
signs with long cantilevers unsecured at one end.
· Nails and other hardware should be hot -dipped
zinc-roated or equally well protected material. Other-
wise, weather may cause them to rust, leaving streaks.
· Use enough nails. Skimping doesn~ pay. Use two
nails across a 2 x 4 and three across a 2 x 6. Drive
nails at a slight angle toward each other.
· With a pneumatic nailer; use no more pressure than necessary to drive nails flush with the swface.
· Screws take somewhat longer to drive than nails, but they hold boards securely, allow for easier removal,
and eliminate unsightly indentations from hammer blows.
· To reduce splitting, drill a pilot hole about three quarters the diameter of the nail. For dense or brittle
wood, grind sharpness from nails or blunt the points by striking them carefully with a hammer. Blunt nails cut
through; sharp ones pry apart.
. Use 12d nails on nominal two-inch decking. Use two at each joint with 2 x 4s laid flat; use three for 2 x 6s
laid flat. 10<1 nails are recommended for 5/4" decking.
· Lumber wider than six inches should not be used as a flat swface. Wide, flat boards are subject to ponding
of rain water, which can lead to cupping problems. It is better to use two 2 x 6's than one 2 x 12.
· If a board is bowed, install it with the crown up. Gravity and the weight of people and furniture will flatten
it.
· If a board has a slight bend to it, it sometimes can be straightened as it is nailed in place.

Dear Mr. Gordon,
Please find attached our structural calculations veridying the design of the wood deck
where a new hot tub is proposed to be placed. Please note we have amended your
drawings to reflect the additional construction requirements. All aspects of your
construction not covered by our comments should follow UBC-2000/IRC-2000 and/or
the local building officials requirements.
The deck joists directly under the tub may be constructed of double 2x12 D-Fir (larch)
no.2 or better. If a different of species of wood is to be used, please ensure its physical
properties are the following minimums or more:
Bending Stress, Fb = 875 psi
Shear Stress, Fv = 95 psi
Modulus of Elasticity, E=1600 ksi
Your lumber supplier should be able to document these values. All lumber must be
treated for exterior use.
