In order to pass a final inspection on commercial elastomeric or similar type roofing, a
letter of inspection and approval from the manufacturer's technical representative
stating that "the application of the roof at (project address) has been applied in
accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection. Effective December 1,
2014, asphalt shingle installations require an approved midroof inspection, conducted when
25-75 percent of the roof covering is installed, prior to final approval. Installation of
roof sheathing (new or overlay) requires an approved inspection prior to installation of
ANY roof.coverings and is require on the entire roof when spaced or board sheathing with
ANY gap exceeding k/2 -inch exists. Asphalt shingles are required to be fastened to the roof
deck with a minimum of 6 nails per shingle. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€-,Ms installation
instructions, whichever is more stringent.

2. This. permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and maybe subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired pen -nits is at the sole discretion of
the Chief Building Official and is not guaranteed.

5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, any violation of any provision of any

ADDITIONAL CONTRACT PROVISIONS: All of the provisions on the front and back of this contract are material terms of this
contract and apply to the work performed under this contract between the property owner and Falling Rocks Construction, LLC.
Jnder the federal Three (3) day Right of Rescission Law, you have 3 business days from the signing of this contract or the denial of
your claim, whichever is later, to cancel the contract by notifying us in writing, by mail, or email. After this time, all monies shall
be held in trust in accordance with Colorado State Law. By signing this contract, I am aware of that right.
I. Payment is due in full to Falling Rocks Construction, LLC (herinafter
referred to as "FRC, LLC" in this document) upon completion of work,
unless otherwise agreed to in writing by both parties.
II. In the event that payment is not received within 5 days after completion,
any remaining balance shall be subject to a late fee of 2% per month from
the day of completion. Customer shall also be responsible for the cost of
collecting said debt and will be additionally liable for all court costs,
collection agency and attorney/legal fees.
AVOIDING HAZARDS TO YOU AND YOUR PROPERTY
III. Roofing work can result in falling debris which may cause injury or damage
property. You agree not to enter or permit guests or invitees to enter the
work area while work is ongoing. FRC, LLC and our subcontractors do
carry the required worker's compensation insurance and general liability
insurance to cover work-related accidents. GL Carrier: United Specialty
Ins. Co. - 303-429-3527. FRC, LLC is not responsible for injuries to you or
others who enter the work area without proper protection unless
determined to be negligent in a court of law.
IV. Dust and debris may enter your attic and the space between your roof
and internal ceiling. You should protect valuable items in attic prior to
construction and use care in lowering the attic stairs or in removing ceiling
tiles after reroofing to avoid injury from fallen debris.
V. Your landscaping is important to FRC, LLC and we will try to avoid
damaging it, however, some minor damage may occur during roof tear -off
and construction.
VI. You agree to allow FRC, LLC and material distributors unobstructed access
to your driveway and parking areas for material delivery and while work is
in progress. The equipment and materials used in replacing a roof are
heavy. You agree that FRC, LLC is not responsible for driveway or curb
damage that results from settling, compression, or inadequate support of
your driveways and/or curbs.
CONSTRUCTION MATTERS
VII. Replacement of a roof often exposes pre-existing structural problems. You
agree that the replacement or repair of pre-existing structural problems
and/or prior unpermitted construction is NOT a part of this contract and
FRC, LLC is not obligated to make such repairs at our expense. The
appearance or pre-existing deflections from sagging rafters or trusses may
be accentuated by a new roof system and you agree that FRC, LLC is not
responsible if this occurs due to structural imperfections in your home.
VIII. Building codes and good building practices require air conditioning, water
and electric lines and pipes be located a safe distance below roof deckj_rte. -

not a reason to withhold final payment. Warranty active when balance
has been paid in full.
XV All contracts are subject to approval of our credit department and
management approval without exception.
XVI. Roofing materials are separately warranted by the manufacturer. FRC,
LLC shall not be responsible for manufacturer's defects.
XVII. You agree to notify FRC, LLC within 24 hours of discovering a leak and to
take reasonable immediate action to prevent future damage. FRC, LLC
will send a representative to investigate any reported leaks. Multiple
investigations maybe necessary to determine the cause of a leak. You
agree to cooperate with such inspections by providing FRC, LLC access to
the interior and exterior of your property at reasonable times.
VIII. You release FRC, LLC from and agree not to pursue claims against FRC,
LLC for pre-existing environmental conditions including, but not limited
to, mold, water intrusion/damages, etc.
XIX. You agree that FRC, LLC is not responsible for damages from fire, water
intrusion, windstorm, hail, tornadoes, hurricanes, or other hazards to
work in progress or completed work. You agree to keep in force a
homeowner's insurance policy covering damages to your property due to
such conditions or other acts of God. FRC, LLC's limited warranty does
not apply to damages or repairs resulting from any of the foregoing
events and FRC, LLC shall have no obligation to repair or replace roof
components damaged by such events. Except as specifically written
here, FRC, LLC makes no warranties, expressed or implied.
CANCELLATION AND ASSIGNMENT
xx. You may cancel this contract and get a refund only if you do so by
written notice to FRC, LLC at the address on the front of this contract ON
OR BEFORE THE THIRD BUSINESS DAY after you sign this contract or after
your claim has been denied, whichever is later. All funds shall be
returned within 10 days of said notice. We provide a valuable service to
our clients. If you prevent us from replacing your roof after you have
signed the contract or preliminary agreement and the claim was
approved, we have met your insurance adjuster onsite, or written a
supplemental estimate to negotiate with your insurance company on
your behalf, you agree that FRC, LLC is entitled to invoice and be paid for
their time spent working on your behalf at $75/hour for all work done up
to that point in time as reasonable and just compensation for work
performed. We do not offer free services to the public nor are we public
adjusters. You agree to reimburse FRC, LLC for all roctn_r4s—
