Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in

Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

®INSURANCE AGREEMENT. The homeowner authorizes New Vision Roofing & Exteriors to represent their best interest in pursuit of full
replacement costs for damages believed to be covered under homeowner's insurance and allows New Vision Roofing & Exteriors to deal directly with the
insurance company regarding this claim. This contract does not obligate the homeowner or New Vision Roofing & Exteriors until the claim is approved by
the insurance company, and accepted by New Vision Roofing & Exteriors. Upon approval of claim, New Vision Roofing & Exteriors will complete all
specified work at a price agreeable to the insurance company (RCV only) and New Vision Roofing & Exteriors, with no additional cost to the homeowner
except for the deductible. You have the right to rescind this contract and obtain a refund of any deposit within 72 hours after entering contract. If you plan
to use the proceeds of a property and casualty insurance policy to pay for roofing work, you may rescind this contract within 72 hours after you receive
written notice from the property and casualty insurer that your claim has been denied in whole or in part. Final agreed upon price will become the final
contract price, regardless of estimated contract price above. New Vision Roofing & Exteriors shall hold in trust any payment received from homeowner
until roofing materials delivered at site or a majority of roofing work has been performed.

1. If the property owner plans to use the proceeds of a property and casualty insurance policy pursuant to part 1 of article 4 of
title 10, C.R.S., to pay for the roofing work, pursuant to section 6-2-105, the roofing contractor cannot pay, waive, rebate, or promise
to pay, waive, or rebate all or part of any insurance deductible applicable to the insurance claim for payment for work on the
covered residential property.
2. New Vision Roofing & Exteriors carries workers' compensation, automobile and commercial general liability insurance. A
copy of the Certificates of Insurance may be obtained by calling insurance carrier, Colorado Insurance Professionals at 303-755-8600.
3. All Contracts subject to approval by New Vision Roofing & Exteriors LLC management. Contract price is valid for ninety (90)
days.
4. New Vision Roofing & Exteriors reserves the right to perform specified work in accordance with generally accepted
construction guidelines, while adhering to local codes in effect at the time of contract.
5. Our standard payment terms are fifty percent (50%) upon delivery of materials, and the remaining fifty percent (50%) upon
completion of labor. All checks must be made payable to: New Vision Roofing & Exteriors.
6. All excess materials remain the property of New Vision Roofing & Exteriors.
Homeowner is responsible for Homeowner's Association approval of roofing products and colors.
8. Homeowner acknowledges New Vision Roofing & Exteriors as a general contractor; as such New Vision Roofing & Exteriors
will be entitled to ten percent (10%) overhead, and ten percent (10%) profit, (O&P) if approved by the insurance company, as part of
the claim.
9. An account is defined as past due when 30 days have elapsed from the date of completion of work. Past due accounts will
earn interest at a rate of one and one-half percent (1.5%) per month or eighteen percent (18%) annually. If New Vision Roofing &
Exteriors is required to turn the matter over to an attorney for collection, the homeowner also agrees to pay all costs incurred,
including attorney's fees.
10. Should default of payment occur on this contract, New Vision Roofing & Exteriors reserves the right to place a mechanic's
lien on the contract property.
11. New Vision Roofing & Exteriors shall not be held liable for failure of performance due to labor controversies, strike, fire,
hail, weather, inability to obtain material from usual source, or any other circumstance beyond the control of the company.
12. Our standard workmanship warranty for roofing is five (5) years.
13. New Vision Roofing & Exteriors is not responsible for any leaks or water damage below the roof line caused by excessive
wind, ice or hail during the period of the warranty.
14. You have the right to rescind this contract and obtain a refund of any deposit within 72 hours after entering contract. If you
plan to use the proceeds of a property and casualty insurance policy to pay for roofing work, you may rescind this contract within 72
hours after you receive written notice from the property and casualty insurer that your claim has been denied in whole or in part.
However, New Vision Roofing & Exteriors is entitled to retain payments or deposits to compensate New Vision Roofing & Exteriors
for roofing work actually performed in a workmanlike manner consistent with standard industry roofing practices. New Vision
Roofing & Exteriors cannot pay, waive, rebate, or promise to pay, waive or rebate all or part of any insurance deductible applicable
to the insurance claim for payment of roofing work on your property. If this contract is cancelled by the homeowner, after 72
hours of execution, the customer agrees to pay New Vision Roofing & Exteriors twenty percent (20%) of the contract price, set by
the insurance company, only if materials have been delivered or work has been performed, as liquidated damages. This payment is
not a penalty, but is fair and just compensation for services rendered to that point.
15. New Vision Roofing & Exteriors will not be held responsible for slight scratching or denting of gutters, droplets of oil in the
driveway, cracks in concrete, or damage to plants and trees. Furthermore, New Vision Roofing & Exteriors will not be held
responsible for exterior damages such as deteriorated trim pieces, crown molding, stucco, solar panels, evaporative coolers, existing
skylights, or interior damage such as falling dust and debris, cracked plaster, sheet rock or nail pops, as a result of normal re -roofing.
16. Replacement or repair of the deteriorated decking, fascia boards, swamp coolers, ventilators, chimneys, counter flashing, or
any other work completed for the homeowner not explicitly stated in this contract, are not included in quoted price and will be
charged as extra on a time and material basis.

Building Division and plans and/or other documents were required was recently completed and approved. In
accordance with the Building Division's document retention policy, the associated plans and/or other documents
are scheduled for disposal.
We are writing to offer you an opportunity to retrieve those approved plans and/or documents prior to their
disposal by the Building Division. The plans and/or documents are available for retrieval for a period of 20
working days (4 weeks) from the date of this letter and must be retrieved in person at the Building Division
offices. This letter must be presented at the time of retrieval of the plans and/or documents. Plans and/or
documents available for retrieval CANNOT be mailed and plans and/or documents will NOT be available for
retrieval after the expiration of the retrieval period, which is 60 days from the date of this letter.

At your request, this office performed a structural analysis for the above listed residence. The purpose of the
analysis was to review and provide recommendations for the conditions described below. The pertinent information
regarding the analysis is as follows:
POST CAP MODIFICATION
It has come to the attention of this office that the front porch beam to post connection was modified to be a
concealed connection. The two 4x8 beams were mitered over the 6x6 post. Leger loks were installed through the
post and into each of the beams. Analysis by this office shows that the existing condition is structurally acceptable to
support the applied loads. No modifications are required.

performed on the residence listed above. The applicable information concerning the observations is as follows:

The footing widths and depths were formed according to the contract documents. Property sized horizontal reinforcing
was provided continuously in the forms. The reinforcing steel for the footings was properly lapped and tied and had been
placed in the forms prior to concrete placement. Footing dowels were on site. Footing forms were placed on undisturbed
natural soils and were clear of debris and loose soil.
FOUNDATION WALL REINFORCING OBSERVATION — April 28 2016
The foundation walls heights, thickness and reinforcing were. formed according to the contract documents, Reinforcing
was placed to provide adequate cover and was properly lapped and tied. The anchor bolts were on site and sized
according to plan. The foundation forms were centered over the spread footing foundation system. Vertical dowels
extended from the footing into the foundation wall at 24" on center. The foundation forms were clear of debris and loose
soit.

One ) exploratory test hole was drilled on dune 2, 2015 at the site shown on the
Location Map, Figure 1, The test holes were drilled with a four -inch ") diameter auger
advanced with o soil exploration drill rig.
At specific intervals, the drilling tools were removed from the test holes and soil samples
were obtained with a two-inch (2") diameter spoon sampling tube. The depth$ at which
soil samples were taken and a description of the soil encountered are shown on the Log
of Test boles, Figure 2, and the Summary of laboratory 'Besting, Table 1 .
All soil samples were carefully observed in the field during the drilling operation. These
samples were classified in the laboratory through visual observation and laboratory
testing to determine the pertinent properties. The natural moisture content and dry
density was obtained from relatively undisturbed drive samples of typical soils. Swell -
consolidation tests were performed on typical soil samples see Figure 3. These tests
indicate the behavior of the soil upon various loadings in a wetted condition.
Groundwater was not encountered at the time of the field investigation. These
observations represent the groundwater conditions at the time of drilling or
measurement and may not be indicative of the conditions at other tines. Groundwater
levels can be expected to fluctuate with varying seasonal weather conditions and if the
sites use irrigation for lawns..

single-family residence be founded on continuous concrete footings and pads.

I , The footings and pads shall be designed for a maximum soil bearing pressure of
1,500 pounds per square foot L+ . The entire foundation shall bear on the
fine-grained, medium dense, very clayey sand.
. No footing or pad shall bear upon topsoil or soils that contain organic material.
All loose and disturbed soil shall be removed before pouring the concrete for the
footings or bearing pads.
3. All continuous footings supporting perimeter concrete foundation walls shall be at
lest 16 inches vide or the width required for the maximum design loads. We
recommend footings be reinforced to bridge isolated soft areas op to 10 feet,
Exterior footings should be provided with at lest 3 feet of soil cover for frost
protection, or per county codes.
4. The foundation ells shall be designed for ars active horizontal pressure based
on an equivalent fluid density of 35 pounds per cubic foot plus any applicable
surcharge or hydrostatic loads.
. In place structural settlements are very difficult to predict with any reasonable
accuracy, dile to the large number of variable geotechnical parameters involved.
However, based upon the currently available methods of settlement prediction, it
is estimated that total structural settlement will be on the order of 1 " and
differential structural settlement will be on the order of 1/2",

The swell potential of the indigenous soils for the residence at this site mat the criteria
for lour risk of slab -on -grade mar l t. Concrete slabs may be used for basement
slabs, garage slabs and exterior surface (sidewalks, patios and aprons placed on the
native soil, Furthermore, intolerable movement of any slab on grade may occur at the
site as a result of future factors beyond the control of Colorado Geoscience and Design,
Inc.
If differential slab -on -grade movement is not acceptable to the owner/builder and,
if the owner/builder is unwillingto accept the risk f differential slab -on -grade
movement a structural floor above a crawl is required in the basement.
The Dwyer/builder shall be cautioned that problems with mold may arise when floors are
built over a crawl space or with a 'structural floor, In order to prevent such problems, it
may be necessary to take spade actions to mitigate the potential for mods, such as
installing actively controlled h rrlidistdt systems and devices, providing adequate
ventilation of enclosed spades below the floor, and/or treatment of the soils with anti -
mold, anti -fungal chemical agents. Colorado Geoscience and Design, Inc. will not b
responsible for any and all claims arising from issues of mold or fungal contamination.
It is the builder's responsibility to adequately address these issues during construction,
If the owner/builder accepts the risk ofslab-on-grade movement and chooses a slab -on -
grade floor, the following steps shall be part of the concrete slab design:
Any soil disturbed during construction shall be compacted by Esse of a vibratory
plate in the ease of loose granular soils or wheel rolled by heavy equipment in
the ease of soft clay or silt soils prior to placement of the concrete slab.
The soil should be kept moist but not wet during the compaction process as well
as immediately prig to the placement of the concrete slab directly onto the soil.
Steps shall be taken to ensure that subsurface moisture beneath the concrete
slab remains constant during the construction process.
The concrete slab shall be structurally isolated from all foundations and shall be
isolated from penetrations by suitable expansion material not less than %" thick.
