Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

1. Owner's Authorization & Contractor's Obligations. I hereby retain and authorize Design Roofing LLC and its joint
venture partners SF Construction Management and K1 Roofing and Restoration ("Contractor") to perform certain
reconstruction and repair work ("Work") on my Property made necessary by the above-described Loss event. Contractor
will furnish all labor, services, equipment, and materials to make the necessary restoration repairs. All Work will be
completed in a workmanlike manner according to industry standard practices.
2. Work Scope and Price. The scope of Work to be performed is in the attached estimate prepared by my insurance
company dated S ►S I ("Estimate") in the amount of $ i b,y7`l , Z ("Price"), which may be marked to exclude
certain work items.,The Price does not include any additional work that may be necessary to make the repairs to correct the
damage caused by the loss event giving rise to this claim, nor does it include any additional work authorized by the Owner.
3. Contract Documents. This Authorization for Repair & Reconstruction Work ("Work Authorization"), along with the
Estimate, any insurance company -approved supplemental estimates, estimates prepared by Owner's adjuster with
Contractor's advice, and/or change orders shall constitute the contractual obligations with respect to the scope of work to be
undertaken by the Contractor and shall be incorporated by reference herein as if fully restated. Contractor reserves the right
to decline performance of any line item in the insurance company -approved estimates.
4. Price and Payment Terms. I authorize my Insurance Company to release payment directly to Contractor for the Work
performed on my Property. I authorize Contractor to process all paperwork through my Insurance Company as necessary to
complete this Work Authorization. For any additional work authorized, I understand Contractor may invoice me for portions
of Work completed. I agree to pay Contractor the amount of each invoice no later than three (3) days following my receipt of
the invoice. Interest shall accrue on payments not received within such time at the lesser of (i) the maximum lawful interest
rate or (ii) one and one-half percent (11h%) per month. Any remaining balance due after progress payments are made shall
be paid no later than three (3) days following completion of the Work. Punch list items identified during the final walk
through will be placed on a warranty list and will not be subject to subtractions from final payments. Fifty per cent (50%) of
the total contract price (i.e. total replacement cost value of the Work in the Estimate) is due no later than three (3) business
days prior to the scheduled commencement of the Work. Should Contactor retain the assistance of a third party, including
without limitation an attorney, to assist with collection of unpaid amounts due and owing, Owner agrees to pay Contractor's
costs associated therewith including without limitation reasonable attorneys' fees, court costs, and interest at the maximum
legal rate.
5. Right to Cancel & Acknowledgement. I understand that I may cancel this Work Authorization at any time before
midnight on the fifth business day after I have signed this work authorization. This right to cancel is in addition to
any other rights of cancellation, which may be found in state or federal law or regulation. I have read and agree to
this Work Authorization and the Standard Terms and Conditions on the backside of this Work Authorization.
