FOR ROOF PITCH 6/12 AND OVER: Midroof & Final Roof inspections for ROOFS with 6/12 PITCH &
OVER REQUIRE A 3RD PARTY INSPECTION. The 3rd party inspection report must be dropped off to
the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with Building Division Policy
BD -18-001 for Balanced Ventilation Systems, or manufacturer installation instructions,
whichever is more stringent.
MIDROOF & SHEATHING: Effective December 1, 2014, asphalt shingle installations require an
approved mid -roof inspection, conducted when 25-75% of the roof covering is installed,
prior to final approval. Installation of roof sheathing (new or overlay) is required on the
entire roof when spaced or board sheathing with ANY gap exceeding one half inch exists.
Sheathing and mid -roofs may be called in at the same time, one hundred percent of the
sheathing must be complete and 25-75% of the mid -roof may be complete. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required.
LADDER MUST BE SET FOR ALL ROOF INSPECTIONS: A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections.
COMMERCIAL ROOFS: In order to pass a final inspection of elastomeric or similar type roof
coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

Re: structural Observation of Structural Damage due to Car Impact at
3741 Oak Street Wheatridge, CO
EVstudio was requested by Eric Paul to observe the reported structural damage
and provide detailed structural repair recommendations.
On June 15, 2011, EVstudio observed the reported structural damage at the
north apartment building. This report will provide a summary of observed
damage and recommendations to correct structural concerns,
Observations
North Building — North West corner
• The car impacted the building at the north west comer, hitting it at roughly
45 degree angle.
• The impact was primarily focused at the window. The trimmer and king
stud on the west side of the window and the next adjacent stud to the west
are damaged.
• The header above the window appears to be undamaged.
• The main floor sheathing was damaged directly behind the impact area.
• The rim board has slight damage.
• The brick veneer was damaged at the window and displaced slightly
towards the west wall.
Conclusion and Recommendations
The Impact only damaged a few framing components around the window
opening. There is no concern for further damage or collapse.
The following items are in need of correction:
• Replace the 2X4 trimmer and king and the next adjacent stud on the west
side of the window framing.
• The window sill and framing between the window sill and floor sheathing
are in need of replacement.
• The floor sheathing should be replaced for a distance of roughly 24" from
the north wall along the impact area.
• The damaged rim board needs a single 2x8 block added between the
joists at the area damaged oust a single joist bay).
• Replace any damaged wall sheathing, drywall and any other wall finishes
around the damaged area.
• The brick veneer is non - structural. Replace around the impact area and
replace or repair as needed towards the west wall.
Limitations
• The recommendations of this report are based on the available data and
the limited accessibility of the elements and components of the structure.

IRC & 2005 NEC. Per IRC Sec. R313, smoke detectors are required to be installed

Dear Mark and Robbie:
At your request, a representative of Mountain States Structural, Inc. visited the subject
property on August 6 and 7, 2009. The owners of the apartment complex art: having the
(6) buildings repaired and remodeled and asked us to assess the porches/stain that arc in
various stages of disrepair.
Each building has a near 2"d floor haicmry/fire escape that serves (2) units (see attached
sketch). The balconies are steel feamed with concrete slabs cast upon corrugated metal
decks. The decks are supported on stoci pipe columns that bear upon concrete porch slabs
on grade- We found that several of the posts do not bear on the slabs at all or are set ton
close to the slab edges. Several of the hose plates arc not adequately attached to the slats;
base plates are missing in some k catitna. These deficiencies will be resolved by the
planned construction of new ground floor concrete porch slabs.
General Observations and Comments:
• The concrete docks appear to be built on shallow, light gage metal decks. Such
slab systems use the metal decks to act as formwork; reinforced slabs, once cured,
no longer treed the deck to carry their design loads (the type and size of
reinforcement should be determined by examining the slab from Building #9,
which is to be replaced). Consequently, the fact that the metal decks have badly
deteriorated does not necessarily compromise the integrity of the porches. The
slabs should be evaluated after the metal deck is rumoved.1lre portion of the

metal deck that lies on the edge angle cannot be removed easily. Ater the exposed
portion of the metal deck has been removed, the area between the edge angle and
the deck must be evaluated; the extent of nLWdetetioration will determine if the
edge angles must be replaced.
• the metal decks will be completely removed due to extensive rusting. A new,
non-st uctural soffit can be applied.
• The stairs on all of the buildings appear to be in satisfactory condition and Will
require scraping and painting- However, per the City of Wheat Ridge, as a life-
safety issue, the spaces between the stairs and the buildings and the riser spaces
are too larger they need to be retrofitted up meet the standard 4-inch ball test. The
basic stair struchnre is allowed to remain. 7taue are some treads that have
deteriorated and need to be replaced. These will be noted later in this report.
• Most ofthe tops of the concrete deck slabs appear to be acceptable. There are
some cracks, but they are fairly small and may be scaled with an epoxy or
comparable-
• There are some concrete door thresholds that have spoiled pieces and need to be
repaired or replaced.
• The edge angles are tied together by a series of narrow (approximately 1/81 x 1-
1/2") straps. These: are in direct contact with the metal deck slabs and many have
rusted. Most of these are likely to need only soaping and painting. They should be
evaluated after they are scraped. A splice can be designed as needed.
• The stairs are made of steel stringers and steel treads with concrete infdl. The
stairs bear an concrete pads at the base and these bases are scheduled for
replacement.
The Porches will be addressed individually, beginning with the north-most building, #9,
and continuing sequentially to the south-most building, #4.
Porch #9.
• This porch is in the worst condition and needs extensive repairs. The stairs and
posts an: salvageable but require scraping and painting. Prrrtionc of the concrete
slab need to be replaced When the metal dock is removed, the slab should be
evaluated to deternine which areas will W replaced.
° The edge angles appear to be badly rusted. If thoroughly scraped, they may be
found to be reusable to some extent.
• The tailing needs to be re-attacbed.

• The metal pans of the slab are rusted and completely deteriorated in many

On February 19, 2002, I inspected the property located at 3767 Oak Street, Wheat Ridge also known
as parcel 39 282 01 012. The inspection was at the request of the current tenant. The purpose of the
inspection was to discover if any deficiencies existed in the duplex, as were reported by the tenant.
Deficiencies that violate the Uniform Codes as adopted by the City were found to exist.
Some of the violations that were noticed are as follows:
Section 3029 "Uniform Code For The Abatement Of Dangerous Buildings" (1997),
Whenever, for any reason, the building or structure, or any portion thereof, is manifestly unsafe
for the purpose for which is being used. Specifically, the window frame in the front living azea
has warped and worn to the point that it no longer secures the sliding window in position. Also,
the sliding glass door is worn and will not move without lifting the entire door.
The above violation constitutes cause to determine the property substandard as defined by the Uniform
Codes(1997) as adopted by the City of Wheat Ridge and is hereby declared to be a unsafe and shall
be abated by repair or replacement. Please regard this letter as official notice and order to repair or
replace the window and door and obtain proper permits.
Work and permits shall commence within two weeks from receipt of this notice.
