1. This ermit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subj ct to the compliance with those documents, and all applicable statutes, ordinances, regulations, po Flans

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3, If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
6. The issuance or grantor of a permit shall not be construed to be a permit for, or an approval of, an violation of any provision of any

Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be

Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE

person at City Hall. The Building Division will be open from 7:30-10:30 a.m., \
Monday through Friday to process these types of requests.

Dear Ms. Slater:
At your request, I have inspected the recently installed shingle roof on the residence
located at 3501 Parfet Street, Wheat Ridge, Colorado. The foilowing is a summary of
my findings:
FINDINGS:
The roof on this residence consists of a Tamko Heritage dimensional asphalt shingle
roof installed over felt underlayment installed over the existing wood roof deck. The
previous roof covering was apparently removed to the existing roof deck prior to
installation of the new dimensional shingle roof. The following discrepancies were
noted:
1. The owner reported "numerous" ►eaks at the south side of the residence around
the solar panels. Inspection reveals that the shingles at the tie-in to the sheet
metal so►ar panel cover were not properly sealed to the "pan metal" flashing. It
appears that the shingles were laid directly over the existing metal flange, and
sealant was applied to the bottom of the shingle as the finai sealant. This flange
should be properly "stripped in" with a waterproofing membrane higher than the
outside edge of the solar panel box to insure proper weatherproofing.

2. The shingles at the "gambrel roof' transition are not properly installed. There are
numerous exposed fasteners where the shingle tabs were "face nailed". This
transition detail should have received a metal edge detail similar to an eave
detail. Photographs 7- 4
3. The metal "lap joints" at the solar collector cover do not appear to be watertight.
These joints should be inspected and resealed as necessary. Photographs 4& 5
4. A wire penetration at the top of the solar panel collector is poorly sealed with
plastic roof cement. A metal "bonneY" or "roof jack" should be installed at this
detail. Photograph 5
5. There is no "saddle metal" installed at the high side of the chimney. Pan flashing
metal was not visible either. It appears that the shingles were "butted" to the
back side of the existing chimney flashing, and "sealed" with a bead of plastic
roof cement. Photograph 6

6. The shingles at the "sidewalls" and the solar panel "sidewali flashing" have not
been installed properly. No "step flashing" was installed, and shingles were
butted to the existing "pan flashing" and not properly "shingled" into the roof.

7. An area on the garage roof was observed as having no felt underlayment
instailed under the roof shingles. Also, the shingle starter strip is only five (5)
inches in width. No felt was apparent under the starter strip, also. Photograph 9
8. In the field of the roof a number of exposed fasteners was observed. Also many
roof nails were observed as being "underdriven" Photographs 9 8 10
9. The counterflashing at the base of the solar panel has been "nailed off" with no
sealant and/or neoprene washers installed at the fastener heads.
RECOMMENDATIONS
The newly installed roof has not been installed according to manufacturer or ARMA
(Asphalt Roofing Manufacturers Association) recommendations and/or requirements.
Of particular concern is the lack of underlayment at some locations, and the improper
flashing details at roof transitions, and the solar panels. Underdriven fasteners are a
concern relating to wind "blow-off'. It is possible to repair this roof to compiy with code
and manufacturer requirements. However to properly inspect the substrate to confirm
that felt underlayment has been installed over the entire roof substrate, and to inspect
and repair all underdriven nails, many of the newly installed shingles would have to be
torn up and replaced to accomplish these repairs. I recommend that this roof is properiy
repaired and/or replaced by a professional roofing contractor, and inspected and
"signed off", prior to payment of any funds regarding this reroof project.
