On 1-27-98 an inspection was completed on a water heater installation at the above address the
following was noted:
1 No down tube on P& T valve
2 Vent pipe was not secured with screws
3 All vent pipes were secured with adhesive tape
4 The main section of vent pipe consisted of flexible moisture duct which is not approved for
this type of installation
5 The connection of this vent to the main vent stack was completed with a non-approved side
inlet splice
6 No die-electric unions were installed
The following items need to noted
1 The occupants suffered form CO poisoning on 1-25-98
2 The occupants are seniors
3 The occupants son performed temporary repairs on 1-26-98
4 It is my opinion that the temporary repairs in conjunction with the newly purchased CO
detector that the occupants should be safe until permanent repairs can be completed
5 Permanent repairs are to be started no later than 1-28-98
6 Ifpermanent repairs are not started by 1-28-97 the city will contract with a local contractor to
complete repairs
7 The contractor that did this installation was Dan Schappaugh dba "Your One Call Remodel
Source" phone # 403-4155 pager #820-9181
8 Dan Schappaugh was instructed on 1-27-98 that he had 48 hours to contract with a licenced
contractor , that contractor is to obtain a permit and complete the above mentioned repairs
