Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding A/2-inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€rms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

2. This.permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180. days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3, If this permit expires, a new permit may be required to be obtained. issuance of a new permit shall be subject to the standard
requirements, fees and_procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, any violation of any provision of any
applicable code or any ordinance or regulation of this jurisdiction. Approval of work is subject to field inspection.

PIERING INSPECTION
Dear Ms, Hardy:
On July 9, 2008 1 inspected the structural repairs at the above referenced property. The purpose
of the inspection was to determine compliance with the repair design dated May 27, 2008.
The inspection did not involve any other system or component of the residence. No soil or other
material sampling, testing, or analysis was performed.
At the time of inspection the steel piers had been installed at the proper locations and torque
specifications. The concrete pier cap reinforcing steel was in place and ready for placement of
concrete. The work was determined to be in compliance with the repair design and approval was
given for placement of concrete and backfill.

On March 5, 2008 I inspected the above referenced property at your request. The purpose of the
inspection was to observe the foundation of the home to determine my professional opinion of
the shvctural condition thereof. The inspection did not involve any other system or component of
the residence. No soil or other material sampling, testing, or analysis was performed.
General:
• The home is a 1 story residential structure, reportedly wnstructed in 1999. The foundation is
of poured concrete construction, supported on footings, enclosing a crawlspace and attached
garage. Central beazing for the home is via steel beams and steel posts, supported on footing
pads. The upper structure is of conventional wood frame conshvction.
• The soils report that you provided for my review, by Hepworth-Pawlak, dated 7/19/1996
(#296 131) dces not cover Uus pazYiculaz property. Attempts to extrapolate the data to make
sense of yow current condition were unsuccessful.
• The Foundation Plan that you provided for my review, by Martin Design dated 1/27/98 (#962-
4397) is labeled for your address and lot/block, but dces not represent the foundation as con-
structed (has a different perimetet layout and is for drilled piers).
• The Footing and Foundation Wall Inspeclion letter, provided for my review, by Martin Design
dated 3/4/98 references footings, but has a different project number (982-5507) than the Foun-
dation Plan provided.
Observations, Opinions and R: :..ndations:
• The central steel beams have deflected downward due to settlement of the footing pads. The
steel posts supporting the beams are excessively off center on the pads.
• The beam deflections have caused the main floor system to sag, causing drywall cracking and
doorway distoRions.
• Based on these observaUons it is apparent that [he footing pads supporting the steel beams are
undersized for the loading conditions and the soil capacity. It is possible that some unintended
roof load is being placed on these footings, which would overload the footings if not origi-
nally anticipated. Also, given the discrepancies between the soils repor[, foundation plan, and
as-built condiUOns, it is possible that the footing pads as-built were not intended for this prop-
erty.
• Given the above, it is recommended to remove the existing footing pads and install steel heli-
cal piers at each of the steel post locations. The piers should have a wncrete pier "cap" pad

with a new adjustable steel post at each location, per the attached diagrams. This action will
provide permanent support of known capacity at the beam support locations, thus removing
any uncertainty regarding the original design and/or attempting to modify the existing footings
without confidence in the actual soil capacity and/or potential for upheaval.
It is assumed that each new pier will achieve penetration into bedrcek and can be installed to
refusal. Should bedrock and refusal not be reached within reasonable depth, this office should
be noUfied for fuRher inshuction.
All work to be done in compliance with local and national safety standazds, and building de-
partment regulations.
Limiffitions:
• This inspection was visual and cursory in nature, but represents the professional opinions of
this firm for the area in question.
• No warranties are made or implied as to the continued structural integrity of this property, as
soil conditions are unpredictable and unknown conditions could exist. The opinions ex-

At the time of our visit on March 2, 1998, the underdrain had been installed
on the exterior of the foundation walL The perforated underdrain was surrounded
widi gravel, Uie drain and gravel were underlain witli a plastic membrane and were
overlain with geofabric material. The top of the pipe was 42 ro 52 inches below the
top of the foundation wall. A sump was located in the northwest corner of the lower
level and was about 30 inches deep. 'Che invert of the pipe entering the sump was
aUout 60 inches below the top of the foundation wall. The sump was dry at the time of
our visit.
Tlle dcain appeared to be installed substantially in accordance witli our
recamnenclations.
