Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding k/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer&tlms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

2. This, permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made -be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Offial and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.

warramy date of December 5,1993 is approaching, we decided to have this inspection in

1) The crawl space below our basement's structural floor is very weT. According to
our inspector, Gary Cantrell, an active vent system should be installed to cuculate
air below the structural floor (Pagge 2). There is no check valve on the discharge

Note:The key to preventing or minimizing structural movement is to prevent moisture accumulation in the foundation
subsoils. The following recommendations should be observed as a minimum:
1 Maintain or ptovide positive slope away from the foundatioa 1" of fal] per foot for the first 5 feet is
recommended. If soil is to be added around the foundation it should be impenneable, clay type material.
2. Maintain or provide splash blocks or downspout extenders at al] gutter downspouts to discharge at least 5 feet

Note: Heat exchanger cracks and/or tlue blockage can resuli in carbon monoxide poisoning or death. Since cracks or
blockage can devefop at any time, annual inspection and testing of the heat plant is recommended. Headaches or
dizaness may be symptoms indicating possible carbon monoxide leakage that should be investigated. The visual
inspection of the heat exchanger is an attempt to identify cracks, holes or other defects However, it is generally not

Subject: Observation of Underdrain for House with proposed crawlspace and
basement at Lot 17ANOW.WCOM Cambridge Park, Wheat Ridge,
Colorado.
Dear Mr. Odean:
At the time of our visit on July 29, 1997, the underdrain had been installed.
The perforated underdrain was surrounded with gravel, the drain and gravel was
underlain with a plastic membrane and was overlain with lapped roofing paper. The
top of the gravel was 41 to 123 inches below the top of the highest foundation wall. A
sump was located in the northeast corner of the basement level and was about 30
inches deep. The invert of the pipe entering the sump was about 137 inches below the
top of the foundation wall. At the time of our observation there was approximately 18
inches of water in the sump, problably due to previous heavy rains. The water should
be removed from the sutnp and water should be removed &om the footing area.
The drain appeared to be installed substantially in accordance wich our
recommendations.
