I. This permit was, issued based on the information provided in the permit application and accompanying Flans and specifications and is
subject to the cdmpliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of

Project Type: Ramp
Description: Build concrete ramp in 2 sections within footprint of existing concrete front patio. First
section of ramp to be located at entrance to patio from driveway, to be cut into concrete, to have curb
toward yard, and to be approximately 4' wide x 6' long. Height of threshold is 5". To enable the ramp to
be 4' wide, will need to cut back decorative patio wall approximately 9'. Repair of wall will require new
framing and new stucco.
Second section of ramp to run from entrance to covered portion of patio to front door, to be
approximately 4' wide x 15' long, to be poured on top of existing patio (might need to do some demo of
patio below so do not have a sliver of concrete at start of ramp) and to have curbs on any open sides.
Going in, are planning on ramp not having a landing so it can be sloped as little as possible to make an
easy to step from ramp to balance of patio near front door. Height of threshold is approximately 12".
Install 1 %" high rubber threshold ramp inside door.

Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

The approximate cost of the services based on damages known at the time this contract is entered is $ 2. ('j• % 3 plus any and all periphery costs expended to
ensure that said services are in full compliance with all city; county and state building code requirements. If any further damage is discovered by Roofing the Rock-
ies upon tear off and installation of the new roof, the approximate cost will be amended to incorporate such damage and included in an amended scope of work to be
approved by your insurance company. Further, this estimate is based on labor and supply costs know as of the date of this agreement. If the costs of these services
increase from the date of this agreement to the date of service, Roofing the Rockies reserves the right to increase the approximate costs accordingly. Such supplemen-
tal claims billed by Roofing the Rockies, that are first approved by the insurance company for additional work, become a part of this contract. The final scope and
price agreed on between the insurance company and Roofing the Rockies shall become the final contract price for the final approved scope of work. Roofing the
Rockies will perform the scope of work approved by the insurance company for the amount of the insurance approved proceeds with no additional cost to you except
for your deductible. However, you will be responsible for any upgrades or additional work authorized by you that the insurance company determines is not covered
within the scope of coverage provided by your insurance policy. If your insurance company will not agree to the scope of work necessary to complete the job in

This letter is intended to describe the current situation regarding the two residential structures
located on Lots 46 and 47 in the Cambridge Park Patio Homes development.
A stop work order had been issued by the Chief Building official in regards to the garage
extensions which were added on illegally by the contractor. Due to the garage extensions, which
are an additional 2' encroachment into the 5' setback requirement, the buildings were non-
conforming and required the approval of a separation variance. The variance was applied for but
then withdrawn prior to the Board of Adjustment Public Hearing on March 23, 1998.
To date, the contractor has agreed to remove the 2' garage extensions, therefore allowing the
buildings to remain 9'6" apart.

Subject: Observation of Underdrain for House, at of 47
Cambridge Park, Wheat Ridge, Colorado.
Dear Mr. Hobbs:
At the time of our visit on February 10, 1998, the underdrain had been
installed. The perforated underdrain was surrounded with gravel, the drain and gravel
was underlain with a plastic membrane and was overlain with geofabric material. 'File
top of the gravel was 133 to 152 inches below the top of the foundation wall. A sump
was located in the northeast corner of the lower level and was about 30 inches deep.
The invert of the pipe entering the sump was about 168 inches below the top of the
foundation wall. At the time of our observation the sump was dry.
The drain appeared to be installed substantially in accordance with our
recommendations.

Dear Mr. Lyons:
At your request, on January 27' and February 11 1998, pier drilling and foundation
wall inspections were performed on the single-family residence located at 4002 Lee
Circle, Wheat Ridge, Colorado. The pertinent information regarding the inspection is as
follows:
PIER DRILLING INSPEC77ON - JANUARY 27. 1998
Soil Pro tle
The soil pro f le encountered during drilling consisted of 0' to 7' -0" of clay and 7' to
20'-0 " of claystone bedrock. Water was found in the holes at the time of drilling, the
holes were cased and pumped. Soils report requirements were met at all pier locations
inspected
Reinforcement Concrete
All of the piers were reinforced with three no. 5 bars, grade 60, for their entire length.
The concrete used consisted of a 3, 000 psi, 3/4" stone aggregate mix. All of the piers
were properly constructed according to our recommendations and the recommendations
in the soils report.

FOUNDA77ON WALL STEEL INSPECTIONS - FEBRUARY 11, 1998
The foundation walls were reinforced horizontally with four no. 5 bars, grade 60, and
two no. 5 bars, grade 60, at the top and bottom. All of the bars were properly lapped and
tied The foundation wall forms were properly centered over the drilled pier foundation
system, and the reinforcing steel from the piers extended to the top row of reinforcing in
the foundation wall. Paper void material was properly installed between the pier
locations inn the bottom of the forms. Reinforcing steel was properly placed at the
foundation wall step-downs and window openings. Additional vertical bars were placed
in all walls taller than 8'-0
