Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding k/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer&4Erms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final

At the time of our visit on June 26, 1997, the underdrain had been installed.
The gravel was placed on a plastic membrane and covered with lapped rooting paper.
The top of the gravel was 116 to 122 inches below the top of the foundation wa1L A
sump was located in the northeast corner of the lower level and was about 37 inches
deep. The invert of a drain line entered the sump about 135 inches below the top of
the foundation walL No water was noted in the sump.
The drain appeared to be installed substantially in accordance with our
recommendations.

At your request, on June 121h and 20th, 1997, a pier drilling and foundation wall
inspections were performed on the single-family residence located at 4065 lee Circle,
Wheat Ridge, Colorado. The pertinent information regarding the inspection is as
follows:
PIER DRILLING INSPF,CTION - JUNE' 12, 1997
Soil Profile
The soil profile encountered during drilling consisted of 0 ' io 10'-0 " of clay with gravel
and rock and 10' to 18'-0" of claystone bedrock in the ripper level, and 0' to 4'-0 ° of
clay with gravel and rock and 4' to 18'-0" of claywone bedrock in the lower level Some
water was found in the holes at the time of drilling. Concrete was on tine site tine/ was
immediately placed in the holes. Soils report requirements were met at all pier locations
inspected
Reinforcement Concrete
All of the piers were reinforced with three no. 5 bars, grade 60, for their entire length.
The concrete used consisted of a 3, 000 psi, 3,4 " stone aggregate tnix. AI! of the piers
were properly constructed according to our recommendations and the recommendations
in the soils report.

FOUNDATION WALL STF.FL INSPF.CTION.S - JUNE 20 1997
The fo nndation walls were reinforced horizontally with two no. 5 bars, grade 60, at the
top and bottom. All of the bars were properly lapped and tied l he fou ndation wall
forms were properly centered over the drilled pier foinndation s}es7enn, and the reinforcing
steel from the piers extended to the top row of reinforcing in the.famdation wall. Paper
void material was properly installed between the pier locations inn the bottom of the
forms. Reinforcing steel was properly placed at the foundation wall step-clowns and
window openings. Additional vertical bars were placed inn all walls taller than 8'-0

This leuer will cont7rm our observations of Lot 72 made on June 11, 1997.
At the time of our visit the excavalion was dug in lwo levels, the upper level
was 1- 2 1'eet below grade and the lower Icvel was 7- 10 feet below lhe exisling
grade. The soil exposed at the upper levcl consisled of 1ill containing a mixture oF
claystone and s1nd. 'Che soils exposed a[ lhe lower levcl consisled oP moist natural
silty to clayey sand. The exposed soils are similar to lhose encountered in our nearby
borings. It is expected tha[ bedrock is dose to lhe botrom of the excavation.
In our opinion lhe house shoulcl be foundcd on a'Cype 5 foundation, consisting
of picrs. A s1aU-on grade basement may be utilized, realizing there is moderate risk
of movement. The other recommenda[ions in our report 296 131, dated Augus[ 30,
1996 should also be observed.
