Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or beard sheathing with ANY gap exceeding Alt -inch exists. Asphalt shingles are required to
be fastened to the. roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€�s installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has kfeen applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT

3. Deposit; If Homeowner intends to pay for the services of Company using the proceeds of a claim made under his or her property and casualty insurance policy, the entire deposit must
be paid to Company within three (3) days of receipt of such proceeds. The entire amount of the first insurance checkor'ectual cash value" check; Must'be paid before work commences.
4: Cencellation: Homeowner has a right to rescind this Contract pursuant to C.R.S. § 6.22-104. Within 72 hours of entering this Contract, Homeowner may rescind the Agreement and
obtain a full refund of his or her deposit,

pay for the roofing work, Company acknowledges that -under C.R.S. § 6-22-105--i1 cannot pay, waive, rebate, or promise to pay, waive or rebate all or part of any insurance deductible
applicable to the insurance claim for payment for roofing work on the covered residential property.
6. Liability Coverege Insurer In accordance with C.R.S. § 6-22-103(e), Denver Roof Systems identifies its liability coverage insurer as follows.

7. Homeowner further agrees that the equity in this property is security for the contract.
8. Company shall not be liable for failure of performances due to labor controversies, fire, weather, inability to obtain material from usual sources. Acts of God or any other circumstances
beyond the control of Company.
9. Should default be made in payment of this Contract, charges shall be added from date thereof at a rate of one and one half percent per month (18% per annum) with a minimum
charge of $2.00 per month, and if placed in the hands of an attorney for collection, all reasonable attorney's fees and legal filing fees shalt be paid by Homeowner accepting said contract.
10. If this Contract is cancelled by the customer later than three (3) days from execution, the Homeowner shall be liable to Company for ten (10) percent of the contract price as liquidated
damages as. reasonableand just compensation for labor, time and expenses incurred in Company's performance of the Contract.
11. If material must be reordered or restocked because of a cancellation of this agreement by Company. Homeowner wip be assessed a restocking fee equal to fifteen (116) percent of the
contract price to cover Company's cost of such reordering and/or restocking.
12. Replacement of deteriorated decking, roof jacks, ventilators, flashing, or other materials, unless otherwise stated in this Contract are not included and will be charged as an extra on a
time and material basis.
13. Company's Limited Warranty does not cover damage to roofs caused by lightning, fire, gale force winds (50 MPH), hurricane, tornado, hailstorm, Impact of foreign object or other
violent storm casualty, or damage of roofs due to settlement. distortion, failure/cracking of,the roof deck, walls or foundation of a building.
14. Company not responsible for any damagebelow the roof due to leeks by excessive wind, ice or hail during the period of the warranty, The Company's warranty covemroof only.
15. Company will not be held liable for any structural movement, settling cracks in dry -wall, cracking or breaking of driveway during the roofing or loading processes, minor scratching or
denting of rain gutters, nail pops, oil droplets or televisionslelectronics,
16. Ail pictures, vrall hangings, lights, collectables, televisions,.etc., both indoors & out, MUST be secured by Homeowner. Any damage to personal items during installation isnot the
responsibility of Company. Company will also not be responsible for nal damage io tires, pots, etc. Pets should not be left unattended during Installation.
17. Any alteration or deviation from specifications involving extra cost will be executed only upon written orders, and will become an extra charge over and abova the, estimate.
18. This Contract and labor warranty shall not be assigned and are nontransferable unless specified in writing and agreed to by Company.
19. If Homeowner fails to fully satisfy the amount of compensation owed under the Contract within a reasonable time, then the laborwarranty is revoked without any further action by
Company.
20. This document constitutes the entire agreement of the parties; and no ether understanding; collateral or otherwise, shall be binding unless in writing and signed by both parties.
21. Company reserves the right to order excess materials. All excess materials belong to Company.
22. Company reserves the right to file for supplemental insurance claims if insurance adjustor measurements are used and prove to be incorrect.
23. At no additional cost to the customer, Company reserves the right to file for supplemental insurance claim due to an increase in costs.
24. Homeowner's final payment In full shall not be delayed while the city, county or appropriate butlding inspectlon department to inspect the work.
25. Homeowner acknowledges Company as a General Contractor. As such, Company will be entitled to 10% profit and 10% overhead if approved by Homeowner's insurance company
as part of the claim.
26. Homeowner is responsible for Homeowners Association (Covenant Approval) of roofing products and color.
27. Company is not responsible for damage to stucco, siding, soffits, and or wall abutment due to flashing and re -roofing.
28. Company will not replace furnace caps.
29. All Starr Roofing Is waived of all liability in regards to damages done to gutters, landscaping, lawns, garden stones, trees, paver stones, propane grills, hoses of any kind,
outdoor furniture, and flower pots due to work performed by All Starr Roofing outlined in this contract.
30. Deductible collected by All Starr Roofing is non refundable if contract is cancelled after three days of signing and insurance company has approved proceeds for work.
31. Any alterations made to:the roof after completion not done by the Company will nullify said warranty. This includes removal of all flashings and wall abutments.
32. This agreement is composed of front and back page end-all materials and labor incorporated herein by reference and shall be considered the entire agreement by the parties.

Wheat Ridge, Colorado.
Dear Mr. Odean:
We observed the excavation for the subject lot on October 18, 1996. The
proposed house will have a basement.
A[ the time of our visit the excavation had been completed. It was dug to a
~ , . q9s
depth of 2t ft. in the upper level an up to about 8 feet deep in the lower level.
About three feet of old till was noted around the sides of the excavation. The soil
exposed on the bottom consisted of slightly moist, stiff, very sandy clay and was
similar to tha[ encountered in our borings. Several inches of loose soil was prosent on
the bottom of the excavation. The garage excavation was bottomed in [he old till.
The loose soil should be removed from below the tbotings and the old till removed
from the garage area. In our opinion the house should be founded on a Type 3
foundation consis[ing of spread footings wilh a maximum soil pressure of 4,000 psf
and a minimum dead load pressure of 2,000 psf. Footings should be placed on tirm,
undisturbed natural soils. A slab on grade lower iloor will have a moderate risk of
movement. The other recommendations in our report 296 131, dated August 8, also
should be observed.
