Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding Al -z -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturerac—s installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer&439;s technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

We hereby submit specifications and estimates for:
1 Remove and carry away existing roofing materials. (Note: If plywood sheeting needs to be repaired or
replaced, cost will be an additional charge.
2. Dispose of roofing trash and clean property of all job related refuse.
3. Install GAF Timberline HD dimensional asphalt shingles over 301b. felt, (ice and water shield in valleys), ridge shingles
and wind protection starter strip. Shingles and all accessories fastened with high wind application (six nails per shingle).
4. Install ice and water shield at eaves, per code
5. Install 2" X 4" painted steel drip edge metal at eaves and 2"x2" painted steel drip edge at gable ends.
6. Remove and replace corrugated fiberglass roofing.
7. Install 26 gauge new vertical step flashing at walls.
8. Replace plumbingjack storm collars, furnace and flue vents. Paint all to match new shingles.
9. Caulk and seal all protrusions and flashing to insure watertight roofing.
10. 5 year 100% workmanship warranty covering any problems that may occur due to faulty workmanship.
11. Installed in accordance with all applicable building code requirements. Permit fees included in contract price.
Option to upgrade shingle to GAF Armorshield 11 Impact Resistant Class dimensional asphalt shingles: $400.00
We Propose to furnish material and labor in accordance with above specifications for the sum of:
Eight Thousand Two Hundred Seventy Five dollars $8,275.00

fees and costs, even If there Is no litigation. Payment due upon completion of roof, no deposit required. 18% annum interest will be charge on past due balances.
All material is guaranteed to be as specified. All work to be completed in a workmanlike manner according to standard practices. Any alteration or deviation from above specifications
involving extra costs will be executed only upon written orders, and will become an extra charge over and above the estimate. All agreements contingent upon strikes, accidents,
or deiays beyond our cuntrol. Owner to carry fire, tornado and other necessary insurance. White Roofing, Inc carries worker's compensation through Pinnacol Assurance and
automobile, commercial general liability insurance through National American Insurance Company and can be contacted through Forsberg Engerman Company at 303762-1717.
Acceptance of Proposal - The above prices, specifications and conditions are satisfactory and are hereby accepted. You are authorized to do the work as specified. Payment will be made
as outlined above. You have the right to rescind this contract within 72 hours after entering the contract If you plan to use the proceeds of a property and casualty Insurance policy to pay for
the roofing work, you may rescind this contract within 72 hours after you receive written notice from the property and casualty insurer that your claim has been denied in whole or In part
However, White Roofing, Inc. is entitled to receive compensation for roofing work actually performed in a workmanlike manner consistent with standard roofing practices.
White Roofing, Inc. cannot pay, waive, rebate, or promise to pay, waive or rebate all or part of any insurance deductible applicable to the Insurance claim for payment for roofing
