The identified suspect ACMs were each assigned a homogeneous area (HA) number and randomly sampled in accordance with regulatory guidelines. Homogenous areas are defined as an area of material that is uniform in color and texture. The HA list for this project including the description, location, categorization, and condition of each material on the HA list is attached to this report Also attached is a digital photo of each HA. Specific sample location, material type, condition, classification, and material quantification can be found on the attached charts. Materials that were not observable, accessible, or not part of the scope of work communicated to Prokop Environmental, LLC are not included in this limited survey. If additional materials that are not included in this survey are found, samples of these materials should be tested for asbestos content Methods: The samples collected were delivered under chain of custody to EMSL Analytical a National Voluntary Laboratory Accreditation Program (NVLAP) and CDPH&E certified laboratory. Laboratory accreditations and certifications are attached to this report. Laboratory analytical method used: EPA Method 600/1\44-82-020; 600/R-93/11. Limitations: See the Scope of Work Limitations section on page 3 of this report for details on the specific area(s) and material(s) that the asbestos testing for this project and report is limited to. This limited asbestos survey was conducted following federal and CDPHE protocols for identifying ACM. Areas of suspect ACM may be present in areas not visible or accessible during this limited asbestos survey. Prior repairs or remodeling of the subject property and building components may have altered building materials so that suspect ACM could not be identified during this inspection. If any additional suspect ACM is discovered these materials should be properly sampled and tested for asbestos content. Reasonable attempts have been made to ensure that this report is complete and accurate with respect to Prokop Environmental, LLC's authorized scope of investigation. Our site visit was of a limited nature with emphasis on bulk asbestos testing based on the scope of work communicated to Prokop Environmental, LLC at the subject property identified herein. It appears the house has experienced differential vertical movement. This movement was indicated by the cracks in the interior finishes, the cracks in the foundation, and the cracks in the brick veneer. It appears that there has been soil subsidence and moisture accumulation on the north wall of the house that likely exacerbated the foundation movement. Additionally, zip -level measurements taken by Peak Structural show differential vertical movement throughout the house. The north wall of the house is approximately 1.4" lower than the south wall. The below recommendations will help prioritize items by severity. Recommendations The cracks observed in the walls were likely caused by vertical movement of the foundation. These movements are most likely caused by contraction and expansion of the soil due to changing moisture content. The solution is to drain water away from the house and prevent it from infiltrating the soil next to the foundation. The following is a prioritized list of recommended repairs that should be made to stabilize the foundation: • Install (7) new helical piers under the south wall foundation, (5) new helical piers under the west wall foundation, and (6) new helical piers under the north wall foundation to help mitigate vertical movement of these areas. • Monitor the home for further signs of vertical movement, particularly at the center of the house supports. These signs may include new orworsening cracks in the interior finishes, new or worsening cracks in the foundation, floors that become more out of level, and/or doors and windows that become difficult to operate. If further signs of vertical movement are observed, the homeowner should contact DL Engineering for further assessment. • The homeowner may consider having the framing above the slabs floated if they are not currently floated to help mitigate vertical movement of the framing as the slabs move. • Patch and monitor cracks in concrete, brick veneer and drywall. This will give a baseline for any new movement. If new movement is observed, contact DL Engineering Inc. for further assessment. • With any foundation movement, the only way to ensure complete stability is to fully underpin the entire foundation. This observation only addresses the areas that appear to be actively moving. Best Practices for Foundation Maintenance The following best practices are actions to maintain the service life of the foundation that can be applied to all structures. Proper site drainage and placement of vegetation alone may not prevent movement of a structure; however, they are prudent steps to avoid the contraction and expansion of the soil due to changing moisture content. The purpose for the site visit and observation was to review the structure for movement of the foundation. The subject property is a split-level home (consisting of an upper level, main level, and lower level) with a basement and addition located in an established neighborhood. The building is assumed to be built in 1962 (based on an online search of the Jefferson County Assessors records). The foundation is constructed of concrete and is assumed to be on spread footings. The house appears to be constructed of wood with brick veneer. The observation was visual and limited to the exterior and the interior house. No observation was performed on the roof structure. For purposes of this report we assume that the house faces south. Observations • Cracks in the interior finishes on the main, upper, and lower levels, multiple areas • Separation around the lower level wall finishes, several places. Lack of proper drainage is often a contributing factor to foundation damage observed. The site drainage around the structure must be maintained such that precipitations will quickly drain away. The use of proper slope slabs -on -grade, moisture barriers, swales, and surface/subsurface drainage systems are strongly recommended. Extensions should be attached to properly functioning downspouts and roof drains such that they discharge a minimum of 10 feet away from the structure. Site Vegetation • The vegetation around the perimeter of the structure can eventually lead to foundation movements. Vegetation can hold moisture in soil and create excessive pressure on the foundation system. In order to reduce the risk, bushes, shrubs, and trees should be carefully removed by an experienced professional. Care should be taken to not further disturb or damage the existing foundation system. • Planters and other surface features which could retain water should be eliminated and properly graded to reduce the possibility of moisture infiltrating around the foundation.
