Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR

RE: Hail Damage Foam Roof Rehab From May 8th Hail Storm.
NATURE OF WORK
We examined your roof in May and have worked out the details for redoing the roof with your
carrier, The Hartford. That effort was finalized around mid June, with some amended follow-up
regarding fireplace flashing and perimeter trim painting.
With the type of damage sustained, this is going to be a complete removal and replacement type
effort. A roll -off dumpster will be brought on site and positioned next the East side of the residence
to provide for ease of removal of all roof debris.
We will remove the polyurethane foam roofing in sections, prime the deck and follow with new
foam placement to match what was the previous foam thickness on the roof. By doing the work
phases in sections, minimal exposure for unexpected weather events shall be eliminated, since roof
work shall be dried in on a daily basis.
Foam thickness shall again be at a nominal one and one-half inches (1-1/2") for both the flat and
low sloped middle section on the roof. On the flat elevations we will add additional foam between
the internal roof drains to enhance water drainage. This likely will constitute an additional one-half
inch (1/2") in average foam thickness, so that polyurethane foam insulation thickness will at some
locations be as much as two and half -inches (2-1/2") at the high points between the roof drains.
Additionally, we will create a slightly rounded foam edge detail along the perimeter of the roof, so
that water does not run over the edges, due to the added foam thickness in some areas.
Some flashing will be replaced as necessary, as will one skylight dome. Two coats of acrylic roof
coating are planned for the completed foam installation. Manufacturer shall be United Coatings
with product tradenamed Diathon (25 to 30 dry mils), with roof color to be white.
Project includes the repainting of trim around the perimeter of the residence. We will need access to

residual paint that you have on hand, so that color matching can be properly handled at the time
additional paint is purchased.
Job includes all applicable permit fees for the City of Wheat Ridge. We anticipate scheduling in the
month of August, due to some equipment upgrades that were undertaken which delayed some of the
foam work on our schedule, plus a little more time lag for processing permits due to the volume of
work resulting from the May hail storm.
ITEMIZATION OF CHARGES
Total cost for job to be a lump sum total of $45,231.00 (FORTY -FIVE -THOUSAND TWO -
HUNDRED -THIRTY-ONE AND NO ONE -HUNDREDTHS DOLLARS).
Above quotation shall be valid for only thirty (30) days from date of issuance.
WARRANTY
A new ten (10) year leak -free warranty shall apply to the completed foam roof system rehab
installation.
Excluded from warranty shall be all unusual acts of nature, structural -related problems, animal, bird,
or man-made damages.
PAYMENT TERMS
Fifty percent (50%) of contract total or $22,615.50 (TWENTY -TWO -THOUSAND SIX -
HUNDRED -FIFTEEN AND 50/100 DOLLARS), shall be due upon start of work. The remaining
fifty percent (50%) balance of $22,615.50 (TWENTY -TWO -THOUSAND SIX -HUNDRED -
FIFTEEN AND 50/100 DOLLARS), shall be due within ten (10) days of completion of work,
based upon client's receipt of final billing.
Please note that The Hartford people have provided instructions for our submittal of the final
invoicing for your roof job, so that the recoverable portion of your depreciation can be claimed
while our work is in process. Denis Ogle, your claim representative, will give it personalized
processing, so that those funds can be received by the time we are at full completion on the roof.
DEFAULT AND DISPUTE RESOLUTION
All sums due Counce Kemper Specialty Contractors, Inc., under this Agreement not received on or
before their due date, shall draw interest at the rate of one and one-half percent (1-1 /2%) per month,

or eighteen percent (18%) per annum. In addition to all other relief to which Counce Kemper
Specialty Contractors, Inc. may be entitled, upon breach by client, Counce Kemper shall be entitled
to full reimbursement for all costs and expenses of collection, including time spent by Counce
Kemper personnel and for all attorneys' fees incurred calculated at $300.00 (THREE -HUNDRED
AND NO ONE -HUNDREDTHS DOLLARS) per hour. At the option of Counce Kemper, any
disputes between the parties shall be resolved either by binding arbitration or by litigation. If
litigation, the jurisdiction and venue of the Jefferson County/District Court is hereby acknowledged
and agreed. If arbitration, the parties stipulate and agree that their dispute will be resolved by
binding arbitration in accordance with the Commercial Arbitration Rules of the American
Arbitration Association at Denver, Colorado and that the award of the arbitrator(s) may be entered
as a judgment of the Jefferson County/District Court pursuant to C.R.S. Section 13-22-201, et_ sea.

thru Denver, visit the jobsite located at 30 Hillside Drive in Wheat Ridge, CO.
V His report to me was that the installation of the polyurethane foam and coating system looked very good. The
application of the foam showed positive drainage and the coating application
was uniform and within United' specifications. Issuance of the 10 Year Warranty will be expedited once the
"Request for Warranty" form is recieved by United, if in fact it hasn't already.
