2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and

Official and may be subject to a fee equal to one-half of the original permit fee.

Work on the following systems require separate permitting through deferred submittals. This list is not all-inclusive
and all projects are subject to the requirements of the international fire code.

105.3.1 Construction permits shall automatically become invalid unless the work authorized by such permit is
commenced within 180 days after its issuance, or if the work authorized by such permit is suspended or abandoned for
a period of 180 days after the time the work is commenced.
105.3.3 Occupancy prohibited before approval. The building or structure shall not be occupied prior to the fire code
official issuing a permit and conducting associated inspections indicating the applicable provisions of this code have
been met.
105.3.5 Posting the permit. Issued permits shall be kept on the premises designated therein at all times and shall be
readily available for inspection by the fire code official.
105.3.6 Compliance with code. The issuance or granting of a permit shall not be construed to be a permit for, or an
approval of, any violation of any of the provisions of this code or of any other ordinance of the jurisdiction. Permits
presuming to give authority to violate or cancel the provisions of this code or other ordinances of the jurisdiction shall not
be valid. The issuance of a permit based on construction documents and other data shall not prevent the fire code official
from requiring the correction of errors in the construction documents and other data. Any addition to or alteration of
approved construction documents shall be approved in advance by the fire code official, as evidenced by the issuance of
a new or amended permit.
105.3.8 Validity of permit. The issuance or granting of a permit shall not be construed to be a permit for, or an approval
of, any violation of any of the provisions of this code or of any other ordinances of the jurisdiction. Permits presuming to
give authority to violate or cancel the provisions of this code or other ordinances of the jurisdiction shall not be valid. The
issuance of a permit based on construction documents, operational documents and other data shall not prevent the fire
code official from requiring correction of errors in the documents or other data.
105.4.4 Construction documents approved by the fire code official are approved with the intent that such construction
documents comply in all respects with the applicable edition of the fire code. Review and approval by the fire code official
shall not relieve the applicant of the responsibility of compliance with fire code.
106.3 Whenever any installation subject to inspection prior to use is covered or concealed without having first been
inspected, the fire code official shall have the authority to require that such work be exposed for inspection.
105.4.5 Amended construction documents. Work shall be installed in accordance with the approved construction
documents, and any changes made during construction that are not in compliance with the approved construction
documents shall be resubmitted for approval as an amended set of construction documents.
General Inspection Requirements
• Construction or work for which a permit is required shall be subject to inspection.
• Additional changes or modifications may be required upon field inspection.
• Such construction or work shall remain accessible and exposed for inspection purposes until approved.
• All failed inspections may result in a re -inspection fee based on the current West Metro fee schedule.
• All failed inspections must be re -inspected.
Requirements During Construction
• Permits and approved plans shall remain on site during all phases of construction.
• A fire extinguisher with a minimum rating of 2A-40BC shall be within 10 feet of all "HOT WORK OPERATIONS".
• All fire alarm and suppression systems shall remain in-service while work is being performed. The Fire District shall be
notified if any existing detection or suppression system will be off line for more than 4 hours.
• Trash dumpsters shall be kept greater than 20 feet from any combustible materials or structures.
• Fire Lanes shall remain clear at all times during construction.
• All utilities shall be clearly marked and protected from damage by construction equipment.
• Areas under construction shall have adequate protection in place to safeguard the public.
• An emergency phone shall be on site and accessible at all times during construction. Either a cell phone of a landline is
acceptable.
• Emergency contact numbers shall be posted on the front door during construction.
• All exit routes shall be clearly defined and posted during construction. Paths of egress shall remain clear of construction
material, debris, or other obstacles.
• Smoking shall be prohibited except in approved areas. No smoking within 25ft of a building, or portion of a building, under
construction. Signs shall be posted in accordance with Section 310. In approved areas where smoking is permitted,
approved ashtrays shall be provided in accordance with Section 310.
• Smoking shall be prohibited where conditions are such as to make smoking a hazard, and in spaces where flammable or
combustible materials are stored or handled.
• Any changes to the approved plans shall be submitted to the West Metro Fire Protection District for approval.
• Refer to Chapter 33 'Fire Safety During Construction and Demolition' of the IFC for all additional requirements for fire
safety during construction and demolition.
False Alarm Activations Durina Construction
• A $300 unnecessary response fee will be issued for every false alarm caused by construction work. It is a violation of
the fire code and local ordinances to initiate a false fire alarm. Some circumstances may result in the issuance of a court
summons.
• It is the responsibility of the General Contractor to make sure that the fire system is offline and all smoke detectors in
the vicinity of any work being performed are covered with a manufacturer's approved cover or other suitable means of
protecting the detector head from contamination and unintended activation. A responsible party must be identified to
notify the fire department in case of a fire during the time the system is off line or inoperable by calling 911. Each day,
when work is complete, the system must be put back in service and the detectors uncovered.
• Detectors are easily contaminated with construction and drywall dust. The detectors can be very expensive to replace
if damaged or contaminated by debris or by blowing them out. Contaminated and damaged detectors do cause false
alarms even after working hours. A sensitivity or obscuration test of all detectors may be required before final system
approval.
• The fire alarm system is an important life safety tool that must be kept in good working order to be properly effective.
You will not be given any permits or appointments for rough and final inspections if you have any unpaid fees including
those due to false alarms.
IMPROVEMENT
GENERAL REQUIREMENTS
Current Fire Detection Reauirements
PERMIT REQUIREMENTS
Projects within the West Metro Fire Protection District (WMFPD) shall be built to the minimum requirements of the current
IFC and amendments adopted by the local City or County. Current amendments across our jurisdiction have no previsions
for fire alarm detection coverage beyond base code requirements. As a result, full detection may not necessarily be
required of the fire alarm system for a tenant improvement project. Tenant improvement projects may be designed to meet
current requirements provided there is no impact on the fire protection systems of adjacent tenant spaces.
If existing detection exceeds current requirements, and removal is approved by the fire district, then all non -required
devices must be removed in their entirety, including all relevant wiring and support systems. Approval via a permit from the
Fire District will be required for any removal or partial removal. All devices necessary to control or interface with other life
safety systems are required and shall be maintained (smoke control, opening protectives, elevator recall, etc.).
Approval will be contingent on the building owner submitting a letter acknowledging and accepting the reduction of fire
alarm protection given the insurance requirements for the property may stipulate the required level of protection.
Additional Tenant Improvement Inspection Requirements
The following are common code violations that the Fire District will be looking for during the final inspection of
tenant spaces. Please take the time to review your tenant space for these items. Compliance with these
requirements is mandatory in order to be approved to open for business.
EXITING
- Check to ensure that exit signs are above exit doors and that they are illuminated.
- Ensure that exit doors open freely, and do not lock in a manner that prevents the ability to exit at any time. (This means
no deadbolts, slide bolts, etc. that prevent exiting)
- Ensure that all push bars/panic door hardware functions and allows for free egress.
- If the main entry door to your tenant space locks then ensure it has a sign on or above the door stating: "THIS DOOR TO
REMAIN UNLOCKED WHILE BUILDING IS OCCUPIED".
- Check to make sure that all pathways through the tenant space are free of obstructions and that exits are readily
identifiable.
FIRE EXTINGUISHERS
- Check to make sure the space has at least one fire extinguisher. Depending on space size, more than one may be
needed so that an extinguisher is accessible from within 75ft from anywhere in the space.
- Ensure the extinguisher(s) have a tag with a date that is not older than one year of today's date.
FIRE PROTECTION
- Ensure that any sprinkler heads on the ceiling are not obstructed, damaged, or painted.
- Ensure that any smoke detectors on the ceiling are not obstructed, covered, or damaged.
- Ensure that the fire alarm strobe lights on the wall(s) or ceiling are not covered, damaged, or obstructed.
ELECTRICAL
- Check to make sure all electrical boxes, switches, light fixtures, and outlets are covered and secured.
- Ensure the electrical panel is closed, secured, and unobstructed.
- Make sure electrical outlets are not overloaded, using multiple extension cords strung together and that extension cords
are not being used as permanent wiring.
HOUSEKEEPING
- Be sure that any storage of combustible waste, trash, construction debris, packaging materials, and other products is
kept to a minimum and is stored in an organized and orderly fashion.
- No storage shall be located greater than 12ft. above the floor level, particularly in stock areas.
- Storage, displays, lighting, and signage shall not block or obstruct the sprinkler system, fire alarm system, or any exit
signs.
- There shall be no excessive accumulation of combustible or construction materials in or around occupied tenant space
or within any exit pathways from occupied tenant spaces.
- At no time shall designated exits from a tenant space be blocked, obstructed, or otherwise compromised. Notify the fire
district immediately if this occurs.
EMERGENCY ACCESS TO THE BUILDING AND TENANT SPACE
- Ensure that any exterior building access points are unobstructed for emergency personnel. Notify the Fire District
immediately if there are access issues to the building, access corridors, fire hydrants, or other
building safety features.

FINAL INSPECTION AND TESTING
In an effort to assist contractors, as well as our inspectors, in assuring final inspections go smoothly, we respectfully ask
that you read and abide by the following measures:
• All rough inspections for fire safety systems shall be scheduled by that system's contractor. Rough inspections will not
be signed -off until mechanical, electrical and plumbing are in place.
• All rough inspections must be completed and passed before any final inspection will be scheduled. (Unless the fire
department has approved for the rough and final to be done at the same time.)
• It is the General Contractor's responsibility to ensure all rough inspections are completed, all pre-tests have been
completed, and any insufficiencies discovered in the pre-tests have been corrected. As well, that all benchmarks on the
G.C.'s permit have been signed off except those pertaining to the final inspection.
• All FINAL inspections for the life safety systems, as well as the New Building or Tenant Improvement FINAL inspection
shall be scheduled by the General Contractor. ALL be on site at the time of inspection. The space must be ready to turn
over to the owner, or "move in ready".
• All doors, glass and door hardware, for interior and exterior must be installed, and all flooring, carpet and painting must
be completed. All construction trades must be complete and out of the space, and the final clean-up has been completed.
• This form, appropriately signed and dated by all contractors, shall be given to the WMFPD inspector at the start of the
final inspection.

Pedestrian Signal Escrow/Letter of Credit Agreement
This agreement made this 15ti' day of January, 2009, by and between the City
of Wheat Ridge and Applewood Baptist Church (property owner) to provide a
letter of credit in the amount of eighty-three thousand three hundred eighty
dollars ($83,380.00) for design and construction of a mid-block pedestrian
signal being held by the City of Wheat Ridge until all improvements relating to
the installation of a pedestrian signal are completed at:
11225 W. 32nd Avenue
In the event that the design and installation of the mid-block pedestrian
signal required by the Wheat Ridge Public Works Department and/or as
agreed to by the property owner is not installed by November 30, 2009, the City
of Wheat Ridge or its designee shall have the right to enter upon said property to
construct and/or install such improvements or do other such work to accomplish
that purpose as may be necessary, provided that the City of Wheat Ridge shall
first give notice in writing to the property owner of its intent to do so.
If the City of Wheat Ridge exercises its right to perform work of any nature,
including labor, materials and use of equipment either by force account or
contract, the City shall have the absolute right to withdraw the escrow funds to
complete said improvements. The only precondition to the City's right to
withdraw the letter of credit funds for this purpose shall be a letter from the
Community Development Director, stating that the improvements have not been
completed or provided and that the compliance date of November 30, 2009 has
expired.

a pedestrian crossing that is adjacent to the church property, we hereby establish our Irrevocable Letter of Credit in
your favor in the amount of Eighty-Three Thousand Three Hundred Eighty and 00/100(US $83,380.00). This Letter
of Credit is available for drawings at our office located 233 Milwaukee Street, Denver CO 80206, at sight in the
format attached hereto as Exhibit A therein compiled and accompanied by your signed and dated statement worded
as follows:
Partial and multiple drawings are permitted under this Letter of Credit.
Drafts under this letter of Credit must be drawn and presented at our office at 233 Milwaukee Street,
Denver, CO 80206, no later than 01/12/2010.
Each draft must also be accompanied by the original of this Letter of Credit for our endorsement on this
Letter of Credit of our payment of such draft.
Each draft must be marked "Drawn under First Western Trust Bank, Letter of Credit No. 43011403".
Payment of any draft(s) drawn and presented under and in compliance with terms of this Letter of Credit
will be made by our issuance of our cashier's check made payable to Hastings Holding Corporation. Such cashier's
check will be forwarded to you at your address above via express courier or at your option via wire transfer pursuant

Study and Final Drainage Report & Plan for the Applewood Baptist Church Parking Lot Project,
located at 11200 West 32nd Avenue.
Dear Mr. Harris,
I have completed my review of the third submittal of the Civil Construction Plans received on May 8,
2009. On October 30, 2008 I (previously) approved the second submittal of the Final Drainage Report and
Traffic Impact Study which were received on September 8, 2008 with a Building Permit Application.
This letter is to inform you that all of the above-mentioned documents for this development have been
reviewed and are hereby approved for construction by the Department of Public Works.
Please be aware of the following items regarding the construction of the project:
NPDES/CDPS Permit
The area of proposed area of disturbance for this construction project is greater than one acre and
therefore requires an NPDES (CDPS) Permit for construction activities. A copy of the CDPS Permit
was submitted to the Department of Public Works on January 15, 2009. Erosion control for the
project shall be in place prior to the commencement of any grading activities, and function as stated
in the CDPS Permit and accompanying Stormwater Management Plan and maintained as needed
during the course of construction. A copy of the CDPS must be kept onsite during construction.
Please be advised that additional erosion control measures may be required if deemed necessary by
the City Inspector.
2. Application for Grading/Fill Permit
Prior to the commencement of any onsite grading, an Application for a Minor Grading/Fill Permit,
along with the fees due will need to be submitted for review and approval.
3. Public Improvement Restoration/Debris Tracking
It will be the responsibility of the of the contractor for the project to repair any damage to the existing
public improvements along 32nd Avenue as a result of related construction traffic in the area. Also,
the contractor will be responsible for maintaining 32nd Avenue on a regular basis such that it remains
free of construction debris and tracking from construction traffic accessing the site.
4. Drainage Certification Required Prior to C.O.
Upon completion of the drainage improvements, the Engineer-of-Record Mr. Wayne Wray Harris,
P.E., P.L.S. shall provide to the City of Wheat Ridge a written, signed and sealed Drainage
Certification Letter stating that the site grading was completed per the approved Grading & Erosion
Control and Drainage Plans, all drainage facilities were constructed and shall function as defined in

the approved Final Drainage Report/Plan, and that the site has been accurately surveyed to confirm
that the grading and the construction of all drainage facilities was completed in accordance with these
documents. The Drainage Certification Letter shall be submitted to the City for review and approval,
and shall be accompanied with As-Built Plans for all constructed drainage facilities prior to issuance
of a Certificate of Occupancy (or Certificate of Completion).
Two (2) copies of the "As-Built" Plans shall be submitted as follows:
a. 1 copy shall be on full-size 24" X 36 "bond paper, signed and sealed, and
b. 1 copy shall be on electronic media (CD-ROM) in AutoCAD 2000-2007.dwg format only.
5. Stormwater Maintenance Agreement and O & M Plan Required
All post-construction Best Management Practices (BMP's) for water quality must be maintained and
inspected by the property owner(s) per Section 20-34 of the City of Wheat Ridge Municipal Code of
Laws. For the stormwater quality facilities including, but not limited to, above-ground or
underground detention facilities, porous landscape detention areas, porous pavement, centrifuge type
devices such as a Rinker Stormcepter®, or for hydrodynamic separation systems such as a Contech
Vortechs®, a Stormwater Maintenance Agreement accompanied by an Operations and Maintenance
(O & M) Schedule/Log will be required prior to issuance of the Certificate of Completion. The O &
M Schedule/Log must be kept current and will be inspected on a regular basis by City staff to ensure
compliance with City of Wheat Ridge requirements.
6. Right-of-Way Construction Permit(s), Licensing, and Bonding
Prior to any construction within the public right-of-way, the necessary Right-of-Way Construction
Permit(s) and respective licensing will need to be submitted for processing by the City. Right-of-Way
Construction Permits are issued only after approval of all required technical documents, including but
not limited to, the Final Drainage Report & Plan, Final Plat, Final Development Plan, Traffic Impact
Study, Storm Sewer Plans, Street Construction Plans, Grading & Erosion Control Plan, Traffic
Control Plan, and easement or ROW dedications. In addition, if the cost of the proposed public
improvements exceeds $10,000, additional warranty bonding shall be required.
2-Year Warranty Period for Public Improvements
The contractor shall warrant any work performed within the public Right-of-Way for a period of two
(2) years, commencing upon completion of all constructed public improvements and Final
Acceptance by the City Inspector. Prior to the expiration of said 2-year Warranty Period, if the City
Inspector at any time deems that the constructed public improvements are in such condition as to
require replacement or repairs, the contractor shall complete such repair work upon request per the
City of Wheat Ridge Permit Testing and Inspection Requirements.
8. Materials Testing for any Construction within the Public Right-of-Way
It will be the responsibility of the developer/owner, to provide the necessary testing, as applicable for
the scope of this project, for sub-grade compaction and other related material tests for any
improvements to be constructed within the public Right-of-Way.

MATERIALS AND TESTING REQUIREMENTS
FOR
CONSTRUCTION OF PUBLIC IMPROVEMENTS
DESCRIPTION
Materials sampling and testing for public improvements shall be performed by an independent
materials testing company at the expense of the developer, and/or utility district. Unless
otherwise designated, all referenced specifications, standards or policies shall be the latest
edition as revised or updated by approved supplements published and issued prior to the date of
the building permit.
REQUIREMENTS
All materials sampling and testing shall be performed by certified, experienced and qualified
materials testing technicians who work under the supervision of a registered professional
engineer in the State of Colorado. All materials sampling and testing equipment shall be
serviceable and have current calibrated certifications.
Soil classifications and moisture-density curves shall be provided to the Department of Public
Works inspector prior to in-place density testing. Materials testing technicians shall furnish
copies of failed test results to the inspector as promptly as the results become available. On a
weekly basis when testing is being performed, the developer shall furnish the inspector with
copies of all test results taken during the prior week and a cover letter, signed by the supervising
registered professional engineer, which summarizes the results and discusses any failed tests or
inconsistencies.
The City materials testing requirements are provided in Table 720-1. All testing procedures of
verification and central lab requirements shall be as specified in the Frequency Guide Schedule
of the Colorado Department of Transportation Field Materials Manual. One test is required for
any fraction of the specified frequency. The reference to Section 203.07 in the table is from the
Colorado Department of Transportation Standard Specifications for Road and Bridge
Construction.

All permits shall be applied for through the Public Works Department. An approved permit is effective for a
period of one (1) year from date of issue and may be renewed. All fees shall be in accordance with those fees
established by the Uniform Building Code or other applicable City adopted Resolutions or Ordinances.
Note. All public improvements, when constructed, shall be maintained (by the respective individual(s) and/or
company responsible for the construction of that public and/or private improvement in the respective
development) on a daily basis, or as needed, such that they are free of mud and other construction debris
tracking from the site. Failure to comply with this requirement will result in the enforcement of Article III,
Specified Nuisances, Section 15-16 (4) of the City of Wheat Ridge Code of Laws.
MINOR GRADING. EXCAVATION. AND FILL PERMITS
1: 1-50 Cubic Yards: No permit is required for dumping or excavation of earth materials, which do not exceed
50 cubic yards, provided, however, that any fill deposited is on natural terrain of less than three (3) horizontal
to one (1) vertical slope, or such fill is less than three (3) feet in depth and is not intended to support
permanent structures, and in addition, such fill or excavation does not obstruct or otherwise adversely affect
any drainageway. Should any of the above standards be exceeded, or a drainage way be affected, a permit
shall be required under the guidelines for No. 2.
2. 51-500 Cubic Yards: Dumping or excavation of earth materials not exceeding 500 cubic yards may be
allowed with a permit approved by the City Engineer. All applications are to be submitted on the appropriate
completed application form and shall be accompanied by the appropriate fee and sketch plan indicating the
following information before the permit will be issued.
A. Location and dimensions of all property boundaries and structures on the site.
B. Location and extent of areas to be filled and/or excavated.
C. Location of existing and proposed drainageways, irrigation ditches, etc., and indication of how and where
historic run-off will be maintained on and through the site.
D. Cross-section area to be filled and/or excavated indicating original slope, new slope and depth of fill.
E. Statement that indicates the proposed use or purpose for said fill or excavation.
F. Relative elevation of adjacent properties.
G. Erosion control plan showing placement of control devices such as hay bales, etc.
3. 501-10,000 Cubic Yards: Operations in which earth material fill or excavation exceeds 500 cubic yards, but
does not exceed 10,000 cubic yards, may be allowed by a permit issued by the City Engineer after review of
an application and supporting information. The following information shall be submitted with the required
application form:
A. _A site plan, prepared and signed by a Colorado registered professional engineer, at a scale of no less than
1 inch to twenty feet (1:20) which illustrates the following:
B. Location and dimensions of all property boundaries and structures on the site.
C. Location and extent of areas to be filled and/or excavated.
D. Location of existing waterways and drainage courses indicating any changes. (for a site containing an
established irrigation ditch, a letter of approval from the appropriate ditch company shall be required).
E. Location of existing and proposed points of ingress.
F. Location and extent of existing vegetation, proposed changes in such vegetation and methods of
rehabilitation on site vegetation after earthwork operations are complete.
G. Erosion control plan.
H. Grading plan with existing (dashed lines) and proposed (solid lines) ground contours with contour
intervals of 2 feet and spot elevations.
1. At least two cross sections (east to west and north to south) through the site showing depth of fill and/or
excavation.
I A drainage report may be required if site runoff characteristics are changed.
K. Relative elevations of adjacent properties and structures.
L. Statement of purpose of intended fill and/or excavation.
4. Performance Standards: The following provisions shall apply to all Minor Grading, Excavation and Fill
Permits:
A. Rehabilitation: Within thirty (30) days after cessation of filling and/or excavation, rehabilitation for the
site shall have been completed in accordance with the approved plans. Rehabilitation shall consist of
leveling, grading, landscaping or any combination thereof to minimize potential erosion and be acceptable
to the City Engineer.
B. Debris: Debris and/or contaminants shall not be used except with the approval of the City Engineer.
"For the purpose of this Section, the term debris shall have the same meaning as the terms "garbage, trash
or junk" as defined in the Wheat Ridge Code of Laws, Section 15-4."
In no case shall debris or contaminates identified or classified as hazardous waste by local, state or federal
agencies be used as fill in any fill areas within the City of Wheat Ridge.
C. Excavation and/or fill areas shall be graded to facilitate weed control until final grades are set and site
rehabilitation and use occur (must be leveled with side slopes not to exceed 3 to 1).

Dear Gary,
This letter is to inform you that the final drainage study, and
the erosion control plan for the above referenced project have
been reviewed and approved for construction. Please be aware of
the following items regarding the construction of the project:
l. The proposed drainage facilities shall be constructed and
approved by the City of Wheat Ridge prior to issuance of the
Temporary Certificate of Occupancy.
2. Erosion control for the project shall be placed in conformance
with the approved drainage/erosion control plan and shall be
maintained as needed during the course of construction.
3. All drainage improvements shall be constructed in conformance
with the approved drainage report and plan: - -
4. It will be the responsibility of the contractor for the
project to repair any damage to the existing public
improvements on West 32nd Avenue as a result of related
construction traffic in the area. Also, the contractor will
be responsible for maintaining West 32nd Avenue on a regular
basis such that it is free of construction debris and tracking
from construction traffic accessing the site.
5. Upon completion of the drainage improvements the engineer,
Mountain States Engineering Associates, P.C. shall provide to
the City of Wheat Ridge a letter of certification stating that
the various improvements, as defined in the drainage report
and plan, have been accurately surveyed to confirm that their
construction is in accordance with these documents. The
certification letter shall be written and stamped by the
engineer who prepared the drainage report, and shall be
submitted for review and approval by the City prior to

I have observed the detention pond and our survey crew has collect data
pertaining to the detention pond contours. Suroey information indicates that the
defention pond volume exceeds the detenfion pond design volume as indicated
and designed in the City approved Final Drainage Report for the Applewood
Baptist Church North Parking Lat dated Ocfober 25, 1996.
Visual observation indicates that the outlet control structure and PVC pipe are

pazking lot detention pond that we spoke about last April. I have enclosed a copy of your April

plamung to have this problem corrected. It has now been several months since I brought this to
your attention and the problem still exists.

Dear Mr. Perri,
This letter is in response to your letter dated July 24, 1997
requesting the status of the drainage improvement corrections
needed at the above referenced site.
As of this date these corrections, per the letter dated April 14,
1997 from the City of Wheat Ridge, have not been completed.
For your information, Mr. Gary Theander, P.E. with Mountain
States Engineering, was reminded approximately two weeks ago of
that this correction work is to be completed as soon as possible.
Mr. Theander responded by stating that he is in the process of
addressing this work however, he could not give me a specific
date as to when this would be completed.
Thank you for your letter. The City will continue to monitor the
progress of the correction work needed, along with pursuing all
reasonable options available to complete the work as quickly as -
possible.

Dear Mr. Lloyd,
The Wheat Ridge Public Works Department inspected the new pazldng lot at the above site and found
that it is not constructed in accordance with the approved plan and hereby request that corrective
measures be taken. There has been a cross access constructed connecting the north end of this
parking lot with the existing lot to the east. There is no indication of such a connection on the
approved plan. This, in effect, creates one large parking lot with two access points to 32nd Avenue
which aze too close to each other. This configuration creates a traffic hazard on 32nd Avenue when
both access points are used simultaneously during the busy hours by allowing a lazge number of
conflicting traffic movements within these two closely spaced driveways.
To restore the site access to a safe and efficient condition, the City requests that one of the three
following options be taken as a corrective measure. Option one would be to remove the cross access
between the parking lots to be in conformance with the approved plan. Option two would require
one of the two western driveways be closed off to uaffic. Option three would be that both of these
driveways be converted from full access to one-way in and out access. This option would require
these two driveway widths be narrowed to a maximum of 14 feet each with appropriate signing and
marking.
Please examine the options discussed above and inform me of your decision by August 25, 1997, in
correcting the site access to this location. I will be glad to meet with you and/or the Church Building
Corrunittee to discuss the above options in detail if you wish. I can also be reached at 235-2862 to
answer any questions you might have.

Dear Mr. White:
It is my understanding that Applewood Baptist Church will not be required to submit
application for a Temporary Use Permit for the upcoming event which we will be having
on our church property at 11225 W. 32"d Avenue from July 26-29, 1998. We aze
submitting this letter in lieu of a permit application so that the City will have something
to sign offon.
With regard to the Church's plans for a TendCamp Meeting I would like to offer the
following information:
The meetings are for Applewood Baptist Church members and guests, including
especially invited neighbors in the Church's vicinity.
2. The lazge tent (100' x 60') labeled Worship Tent would contain seating for 672-
792 which is slightly less than the capacity of our sanctuary.
The tent (20' x 20') labeled Food Service would be for just that, as we intend to
serve meals to the congregation in the evenings starting at 6:00 p.m. No cooking
will be done in the tent.
4. Times for the event will be as follows:
July 26 (Sunday)
10:30 am to 12:45 pm and 6:00 pm to 9:45 pm
July 27 (Monday)
6:00 pm to 9:45 pm
7uly 28 (Tuesday)
6:00 pm to 9:45 pm
July 29 (Wednesday)
6:00 pm to 9:45 pm

A meal will be served to those who attend from 6:00 pm to 7:00 pm each evening,
with the service held from 7:00 pm to 9:00 pm. We would anticipate that the
property would not be completely vacated until approximately 9:45 each evening.
6. There will be lights and sound equipment set up in the Worslup Tent. This power
will be supplied by a temporary drop provided by Public Service.
7. We plan to notify neighborhood representatives within the neat few days about
this proposed church function and we will address and mitigate any concerns they
may have regarding traffic, noise, parking, etc. I am the point of contact at the
church should problems arise.
We have made contact with the Wheat Ridge Fire District and are addressing the
concerns that they have. Per my telephone conversation yesterday, with Dave Roberts,
Wheat Ridge Fire Deparhnent, we will haue six fire extinguishers in the Worship Tent
and one fire extinguisher in the Food Tent. These fire extinguishers will be in the same
locations as in 1997.
We haue also contacted the Lakewood Police Department and will have an extra duty
police officer on site during any activities.
Attached is a sketch of the locations of the tents and access roads for your file. As you
can see in the sketch, this year we have added a trailer for the purpose of telescopic
viewing of the night sky. This trailer will not require electricity, lighting or sound
equipment.

Use Permit for the upcoming event which we will be having on our church property at
11225 West 32nd Avenue the week of July 13, 1997. Upon subsequent conversation with
Meredith Reckert, we will be submitting a permit application (similar to a building
permit) so that, according to Meredith, the City would have something to sign off on.
As you indicated, we will make contact with the Wheat Ridge Fire District and address
any concerns that they may have. We will also make every effort to reduce or eliminate
the loss of any parking as a result of erecting the one tent (20'x20') which would
currently be located on the west parking lot.
We plan to meet with neighborhood representatives within the next few weeks to discuss

concerns they may have regarding traffic, noise, parking, etc.

Re: Religous Revival Event

Pastor Larry Fulton of the Applewood Babtist Church contacted me on May 7, 1997
regarding any requirements which the City may impose on the church regarding a Summer
Revival Event which the church is scheduling for one week in June. He indicated that it would
primarily serve the existing congregation, and would involve the use of three tents to be
placed upon the vacant portion of church property at 11225 W. 32nd Ave.
As Zoning Administrator I made the decision that an event such as the church has
planned, is a traditional and customary accessory use to a permitted church use. The use of
tents would need to meet the following :
1. The Wheat Ridge Fire Protection District should review plans for the erection and
placement of said tents.
2. The tents should not be placed upon any parking areas which are necessary to meet the
minimum requirements of the church, per the Zoning Code.
3. No building permit is required for the placement of such temporary shelters.

This new dual chamber ionization smoke
detecror represanis Ihe state ol the art.
Designed for simple two-wire and four-wire
operation, it senses both visible and invisible
products of combustion. Maximum smoke
response from any direction is assured by its
360° smoke entry design.
Quick, easy visual superoision of Ihe detec-
tor's function is provided by a pulsing LED (six
flashes per minute). Under alarm conditions,
the LED lights constantly al full brilliance.
The new CPD-7021 teatures unique, gated
output circuitry ihat requires three differenl
and simultaneous signals to actuate the detec
toc This, coupled with improved transient sup-
pression design, provides unmatched reliabilily
and stabiliry ofbperalion.
Installation becomes quick and easy with
nonpolarized connections in the detector
head. The detector head is simply fifled to the
base with a twis4lock ac[ion. The locking tab
feature provides additional security for a
vandal-resistant installation.
Functional lesling may be done easily with
a test magnet that causes a currenl change
through the detector chambers to simulate the
effect of smoke and put the device inro alarm
condition.'
Sensitivity of the CPD-7021 may be moni-
tored by a 10 megaohm meter without remov-
ing the detector trom its base. Should
conditions warrant a change in the presel
sensitivity calibrations, the device is easily field
adjusted.
To avoid potential nuisance alarms, the
chamber area is protecled by an insect screen.
Optional bases are available for auxiliary
relay, remote alarm LED, and four-wire
applications.

conshuction with die cast metal housings

power suppiies and are polarized for su-

This new dual chamber ionization smoke
detector represents the state of the art.
Designed for simple two-wire and four-wire
operation, it senses both visible and invisible
products of combustion. Maximum smoke
response from any direction is assured by its
3600 smoke entry design.
Quick, easy visual supervision of the detec-
tor's function is provided by a pulsing LED (six
flashes per minute). Under alarm conditions,
the LED lights constantly at full brilliance.
The new CPD-7021 features unique, gated
output circuitry that requires three different
and simultaneous signals to actuate the detec
tor. This, coupled with improved transient sup-
pression design, provides unmatched reliabiliry
and stabiliry of operation.
Installation becomes quick and easy with
nonpolarized connections in the detector
head. The detector head is simply fitted to the
base with a twist-lock action. The locking tab
feature provides additional security for a
vandal-resistant installation.
Functional testing may be done easily with
a test magnet that causes a current change
through the detector chambers ro simulate the
effect of smoke and put the device into alarm
condition.'
Sensitiviry of the CPD-7021 may be moni-
tored by a 10 megaohm meter without remov-
ing the detector from its base. Should
conditions warrant a change in the preset
sensitivity calibrations, the device is easily fieid
adjusted.
To avoid potential nuisance alarms, the
chamber area is protected by an insect screen.
Optional bases are available for auxiliary
relay, remote alarm LED, and four-wire
applications.
