Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding Ai�-inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€T"s installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

This is a contract between AGR Roofing and Construction, Inc. ("Contractor") and the
Customer identified above and signing below ("you" or "Customer"). Customer wishes to engage
Contractor as a general contractor to perform work on property owned by the Customer, or by a
person that Customer represents, as Contractor recommends. Customer will pay Contractor for this
work as calculated in this contract. Contractor agrees to communicate with Customer's carrier to

1. PAYMENT. You are personally obligated to pay the final price for all work and imteriuLs

improvements to the property, supplements requested by you or recommended by Contractor.

instruct your carrier to cooperate with Contractor, and agree that you and your insurer shall
ccopemte fully and in good faith with Contractor. You authorize and ircsnuct your carie, to
include Contractor as a payee on all payments related to this contract. You instruct any
mortgage company who may come into possession of insurance lands to immediately convey
any insurance funds received to Contractor. These directives are irrevocable and you
understand Contractor is relying at them
4(b). Scope of Work Because the services rendered by Contractormay be intended to address
an emergency situation, it may no he possible m deemiine the final stupe of work and price
boom work begins. Contractor will undertake masonableefforts toagree rot a scope ofwork and
Price with your carrier, but cannot guarantee that your carrier will agree or approve the work.
In theevent that Contractorand vourcarrieragreeon a scapeof work- you agree that Contractor -
will perform all work in that scope for the prices identified in Thaz scope unless you and
Contractoragree, in writing, which specific items of the scope of weak will rat he undertaken.
4(c). Adequacy. Contractor's obligation to perform the scope of work tinder this contract is
contingent upon you carrier authorizing sufficient funds to complete the project. If, in
Contractor's sole discretion. Contractor detclani,a s that your carrier cannot be made m pay
funds wnummially reasonable and sufficient to com plec the project, Contractor may cancel
this cermet without penalty to Contractor.
4(d) Assignment. You today assign to Contractor the right to collect funds that my be owing
to you or paid by your carrier to you related to Contractor's scope of work Commctw, will
receive all finds paid by your carrier, unless you and Contractor reach a specific written
agreement, signed bybotb parties, specifying which portions ofthe insurancescope ofwark will
mol be performed and the total amount of related insurance funds that will conveyed m you.
4(e) Funds Held in Trust tfyou Or any third party, including any mortgage company, receive
any money from a career, to the extent you owe or will owe funds to C i ntractor, that money is
owned by Contractor and shall be held in trust for Contractor. The funds will not he subject to
any claim s lien by my Other person, and you indemnify Contractor against any damages or
costs related to the same. You agree that you w anyone else pn . sing o disbursing these
funds wail be subject to liability, including liability under C.R.S. §§ 11F4-401, et seq.
4(f) Na Withholding by Mortgage Company. Ifany insurance funds come intothe possession
of or must be endorsed by an entity holding a mortgage or aha interest in the real property
otherthan you ("morgagewmpany'), then you agree that this nortgagecompany is ran entitled
to milihold any amount of tial payment from Contractor. Since you might ask Contractor to
do less than all of the work on the insurance scope• yam mortgage company is not permitted to
+adnhold money until final completion of walk. You will fully cooperate in ensuring any
mortgage company s compliance with this provision, including signing letters instructing it to
mime funds o provide is endorsement. You acknowledge that delays in payment caused by

5(b) Humird Testing. You represent to Contractor that, to the best of your knowledge,
understanding, and bctief, you home does wtcmtain asbe lm or envoherhatardous anterial.

necessary, you agree to pay the costs for such a test and approve delay related to such testing.
5(c) Incidental and Consequential Damages. You agree that Contractor is not liable fa any
damages of ally kind, whether direct, consequential, incidental, or otherwise, resulting from the
rctions of Contractor or is subcontractors, and you agree to indemnify and hold harmless
Contractor and any of its employees, agents, or subcontractors from any claim This includes
arnages related to Homeowners Association rules or penalties, disruption to business, tenant
:omplains, damage to utility lines located underneath decking, mold or water damage, many
atheralleged damage miumnresulting from Contractor's work. Contractor's liability is limited
o is warranty.
5(d). Pre -Existing or Hidden Conditions. Contractor is not responsible for inspecting the
roperty for structural deficiencies, or other preexisting hazards, Or those occurring as an
inevitable consequence ofproperwork. includinghut not limited todeileaiunc, Protnaions,
damage tostuceoor sidinc, problems with dcekiii ,fascia boards, bufles.Jacks. ventilntff,
gutters. or flashing. Root'work can change the load on the suucure, leading to cracking,
distortion, changes in the fiwndavun, or other manrfistmion of hidden defects or
