by any Sanitation District representative. Inspection and approval of work by any
Sanitation District representative DOES NOT grant authority to cover work without the

**For trenchless sewer replacements - The contractor will verify proper slope of new sewer
piping in conformance with the IRC and IPC of 1/4 inch per foot unless otherwise allowed
specifically by the applicable code. The plumbing contractor will provide verification of
proper slope and drainage in writing as a condition of approval of the permitted work.**
Both the front and back of this permit are required to be posted on the job site at all

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the
Homeowner.

Homeowner ("I/We") hereby retains Cenco Building Services, LLC ("Cenco") as our General Contractor ("Contractor"), for the
purpose of documenting the damage to our property in the capacity of Contractor and not an adjuster. If our insurance

complete such repairs with in like and kind materials as authorized by our insurance company and I/We agree to pay, if any,
any upgrades I/We request in materials or labor that are not paid from insurance proceeds.
The cost of the repairs shall be equal to the amount of the replacement cost value authorized and paid by the insurance
company, plus the insurance deductible paid by the Homeowner directly to Cenco, and increased or decreased by any
supplements authorized by the insurance company and/or upgrades authorized by us, to include all overhead and profit as
paid by the insurance company. First payment is due upon delivery of materials and/or commencement of work.
I/We agree that any information that is received from the insurance company will be delivered immediately to Cenco and that
any payments received from the insurance company shall be endorsed and delivered to Cenco.
If this contract is cancelled more than three days from the date of this Agreement and a full roof replacement is secured and
ready to roof, then I/We agree to pay Cenco a contract cancellation and administrative fee amount equal to 15% of this
contract as liquidated damages to Cenco.
Cenco and its Authorized Representatives are NOT Public Insurance Adjusters.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if

Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be

person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

2. The plans have been changed to read that the 8' ceiling in the gazage will be 16" O.C.,
thus requiring only one sheet of 5/8" type "X" fire code sheetrock.
New Foundation plan enclosed.
4. Exterior siding is 8" metal siding. Lateral bracing is provided by 3/4" OSB sheathing.
All outlets in the gazage will be GFCI protected and mounted +18" above finished floor.
6. Smoke detector removed from gazage. Gazage door labeled as solid core door with
closure.
7. All outlets in the bathroom will be GFCI protected.
8. There is a 3'x3' landing outside exterior door and a 3'x6' landing outside of the exterior
french doors (back of the addition).
9. Fuller's own the property, Brian Patefield is their son-in-law. The Patefield's rent the
home with plans to buy the home. Fullers are well awaze of the project. Kim at your
office knows the story.
