(3) If this permit expires, a new permit may be acquired for a fee of one-haif the amount normally required, provided no changes have been orwill be made in the

On June 4, 1999 at 0336 hours I responded to 7800 W 35' Ave 9206 to assist American Medical Response
on a report of a first aid. Upon arrival I found that the above pazamedics (Brewer and Jackson) at the scene talking to
the victim of an appazent heart attack. The victim, later identified as Stanley Campbell, was a white male in his sixties.
He stood about 6'2 in height and weighed about 3001bs. He complained about having chest pains since the morning of
June 3, 1999. He had ignored the pain and stayed in bed thinking that it would go away. He was dressed in underweaz
and I could see scars on his legs as well as his chest that were consistent with scars found on a person who had
undergone heart bypass surgery. There was also paraphemalia in his kitchen that was consistent with a person who was

taking insulin. I realized that the victim was located on the third floor of the apartments. (Apartments consisted of a
garden level, main level and second story level.) In order to get him out to the ambulance it would be necessary to get
him down three flights of stairs. There were two stair cases located in the building. I checked the stair cases and found
that the turn around area of the east stair case had been constructed too small to allow a pram to be easily moved from
one flight of stairs to the other. I checked the west stair case and found that the turn around azea was originally lazge
enough to easily allow for a pram to be easily moved from one flight of stairs to the other. However the buildina had
been remodeled and a locked glass partition had been installed making part of the staircase into an office. If a key
could be obtained to unlock the partition and move furniture it would have been possible to move the pram from stair to
stair. I checked with the paramedics to see if they had a"stair chair" to assist them in getting the victim down to the
ambulance. I told them that it would be very hard to get the victim down to the ambulance using a pram. They told me
that they did not think that they had a"stair chair". I called on the radio to dispatch telling them that two to three
firemen would be needed to assist getting the victim down stairs in the apartment. I also told dispatch that they should
also brin-, a"stair chair". I went to residents on the lower level of the apartments and inquired where I could find the
manager or keys to open the above office door. Two residents answered their doors and informed me that the
management did not live in the comples and as faz as they knew no one at the building had access to keys. I then went
with pazamedic Brewer to the ambulance and retrieved their pram. We were able to get the pram up the east stairs but
only after I raised it arms length above my head to get it azound the tum. It would be impossible to inove the victim on
the pram. I asked dispatch to send me another officer to the scene for more ma n power. Dispatch also paged the fire
department another time to get them to the scene. I then went back to room #206 I heazd pazamedic Jackson tell the
victim that he had suffered a heart attack and needed to go to the hospital. The pazamedic gestured to the EKG tape
hooked up to the victim when he made his statement. Finally fireman Bob Dowling of the Wheat Ridge Fire
Department arrived at the scene. Officer Berkowitz arrived at about the same time. Dowling said that he did not have
access to a"stair chair". At this time pazamedic Jackson told me that he had decided to walk the victim down the stairs
to the door landing and have him sit on the prim. I checked the above partition doors again with Officer Berkowitz. I
tried to get them to jump off their track or jar the locking mechanism loose from it's position. The manoeuver did not
work. I then helped paramedic Brewer getting the pram to the west door. Jackson then walked the victim down the
stairs. After the second flight of stairs the victim appeared woozy and I held his one arm to steady him. He then
walked down the last flight of stairs. He sat down on the pram and appeared to pass out. The color on his face tumed
to blue and he showed signs of not breathing. Jackson and Brewer took him into the ambulance and began CPR.
Fireman Dowling then got into the ambulance and assisted them with CPR as one of the firemen drove the victim to
Lutheran Hospital.
Fire Chief Rob Applehance arrived at the scene and I showed him the problems with the stair cases at the
apartments. He will notify Fire Marshall Roberts about the problem. I will notify the code enforcement department at
the city about the apparent violation also.
I went to Lutheran Hospital and discovered that no one knew the victim's name. I took keys that belonged
to the victim and went back to his residence. After leaming that he had died I gathered his medication as well as the
above information, plus information about his identity and took it back to the hospital. I found out that Dr Murphy had
pronounce the victim dead at 0442 hours. I turned the above items and information over to Deputy Coroner Kim

notifications made by the police department:
0336 Hours - AMR notified WRPD that they needed assistance on a first aid call requested by Kaiser Permenente.
0333 Hours I arrived at the scene to find AMR personal present.
0338 Hours - Fire Department paged by dispatch to respond to the scene. (No Response)
0339 Hours - Fire Department paged by dispatch a second time to respond to the scene. (No Response)
0345 Hours - Fire Department paged by dispatch a third time to respond to the scene.
0357 Hours - Fire Department Officer powling arrived at the scene.
Aprox. 0407 Hours - Victim "Goes Core 0" after walking down stairs.
0411 Hours - Officer Wacker notified Lutheran Hospital of the "Core 0"
0414 Hours - AMR leaves the apartment complex with the victim to go to the hospital.
On the morning of June 4, 1999 I responded to 7800 W 35"' Ave and took photographs of the stair cases at
the apartment.
Foreword copies of this report to the building inspection department as well as the fire department.
