REGARDING ROOF VENTILATION: Roof ventilation shall comply with Building Division Policy BD -18-001 for Balanced Ventilation Systems, or manufacturer installation instructions, whichever is more stringent. MIDROOF & SHEATHING: Effective December 1, 2014, asphalt shingle installations require an approved mid -roof inspection, conducted when 25-75% of the roof covering is installed, prior to final approval. Installation of roof sheathing (new or overlay) is required on the entire roof when spaced or board sheathing with ANY gap exceeding one half inch exists. AGS subsequently conducted clearance inspection and monitoring as follows: • A visual inspection was performed to confirm the completeness of asbestos removal and cleanup. The visual inspection was performed in accordance with the American Society for Testing and Materials Standard E1368 — Standard Practice for Visual Inspection of Asbestos Abatement Projects. • Five (5) air samples were then collected inside the contained area utilizing aggressive air sampling techniques as directed in 40 CFR Part 763, Appendix A to Subpart E (EPA 1995). The air samples and two (2) quality control blanks were then analyzed utilizing Phase Contrast Microscopy (PCM) employing the NIOSH 7400 method. BTG is a certified asbestos laboratory by CDPHE and successfully participates in the American Industrial Hygiene Association's Proficiency Analytical Testing Program (PAT) for determining concentrations of airborne asbestos fibers. (CDPHE Laboratory I.D. #22890, AIHA Laboratory I.D. #226469). Results: Based on AGS's visual inspection, asbestos materials requiring abatement, including contaminated dust, debris and residues, were determined to have been removed satisfactorily.
