Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

I. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, po Flans

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change, of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

THIS AGREEMENT IS BETWEEN GOLD STAR CONSTRUCTION, INC ("COMPANY") AND THE OWNER(S) NAMED ON THE FRONT
SIDE, AND IS SUBJECT TO ALL APPLICABLE RULES, REGULATIONS, LAWS AND ORDINANCES OF THE STATE OF COLORADO AND
THE LOCAL JURISDICTION(S) OF THE PROPERTY LOCATION, AND THE FOLLOWING TERMS AND CONDITIONS WHICH ARE MADE
A PART OF THIS AGREEMENT.ANY CONTROVERSY OR CLAIM ARISING OUT OF OR RELATING TO THIS AGREEMENT OR BREACH
THEREOF, SHALL BE EXCLUSIVELY DECIDED AND SETTLED BY BINDING ARBITRATION. VENUE SHALL BE EXCLUSIVELY DENVER, CO

practices. Any alterations or deviation from specifications involving extra costs will be executed only upon written orders in the form
of a ''written change order" and will bean extra charge over and/above the original contract price. Any representations, statement,
or other communication not written in this Agreement or the Disclaimer Agreement are agreed to be immaterial, and will not be
relied upon by either party, and do not survive the execution of this Agreement.
3. IN_It is agreed that the amount of the Agreement shall be the amount equal to the amount referred to as owner's insurance
claim summary on the front of this Agreement, plus any written change order(s), supplements and the terms contained within the
Disclaimer Agreement. Supplement money paid by the owner s insurance company for additional labor and materials needed
beyond the original scope of repairs shall be paid directly to the Company.
4. IN_You the owner(s) can cancel this transaction at anytime in writing prior to midnight of the third day after the date of this
transaction. To cancel this transaction, mail ordeliver a SIGNED, DATED, statement of cancellation to the above address as proof of
your intent to cancel prior to midnight of the third day after the date of this transaction.
5. IN_The final payment is due immediately upon work completion and shall not be held up while waiting for the city/county/state
to inspect the

but prior to the commencement of work, such cancellation, termination or failure to initiate shalIcon stitute breach of this

7. IN The Company has the right to order excess material. These materials will not be charged upon the above agreed upon price.
All excess materials belong to the Company. The Company is authorized to do work on a day that is mutually agreeable to customer
and Company.
S. IN All agreements are contingent upon "Force Majeure." It is mutually agreed to by both partiesthat Force Majeure means and
refers to acts of God: strikes, lockouts, material shortages, or other industrial disturbances: acts of public enemies; orders of any
kind ofthe government ofthe United States, The state of Colorado, or any civil or military authority: insurrections: riots: epidemics:
landslides: lightening: earthquakes: fires: hurricanes: storms: floods: washouts: droughts: arrests: restraint of government and
people: civil disturbances: explosions: break -age or accidents to machinery, pipelines, canals or other facilities: partial or entire
failure of necessary utilities or any other causes not reasonably within the control ofthe party claiming such inability.
9. IN_lf any ofthe provisions -4this Agreement should be held to be invalid or unenforceable, the validity and enforceability of the
remaining provisions of this .,reement shall not be affected hereby.
10. IN The Company is not responsible for any damages below the roof due to leaks caused by wind in excess of the manufacturer's
warranty limit, ice dams, hail, and pre-existing or future construction defects, caused by storms or lark of maintenance during the
warranty period.
11. IN Should default be made in payment of this Agreement, charges shall be added from the date thereof at a rate of one and a
half percent (1 % %) per month with a minimum charge of five dollars ($5.00) per month, in the event that referral to an Attorney for
collection ofthe amounts owed underthis Agreement becomes necessary, or in the event that arbitration or other legal action is
necessary to enforce the provisions of this Agreement, the prevailing party shall be awarded costs and reasonable attorney's fees.
12. IN_ANY PERSON OR COMPANY SUPPLYING LABOR MATERIALS FOR THIS IMPROVEMENT TO YOUR PROPERTY MAY FILE A LIEN
AGAINST YOUR PROPERTY IF THAT PERSON OR COMPANY IS NOT PAID FOR THE CONTRIBUTIONS.
13. IN_ Company's current written workmanship guarantee and applicable written manufacturer's warranties on materials shall
apply and are not transferrable or assignable, unless otherwise specifically stated or agreed to in writing. To obtain
warranty /guarantee services, the contract amount must be paid in full.
14. A property owner who enters into a written contract with a Roofing Contractorto preform roofing work on the property owner's
residential property, the payment for which will be made from the proceedsfrom the property and casualty insurance policy, may
rescind the contract with -in 72 hours afterthe property owner receives written notice from the insurer that the claim for payment
for roofing work on the residential property is denied in whole or in part. The owner's right of rescission does not apply when the
insurer denies in whole or in part, a claim related to a request for supplemental roofing services.
15. Waiver of insurance deductible prohibited. A roofing contractorthat preforms roofing work, the paymentfor which will be made
from the proceeds of a property and casualty insurance policy, shall not advertise or promise to pay, waive, or rebate all or part of
any insurance deductible applicable to the claim for paymentfor roofing work on the covered residential property.

2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees and procedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE

THIS AGREEMENT IS BETWEEN GOLD STAR CONSTRUCTION, INC ("COMPANY") AND THE OWNER(S) NAMED ON THE FRONT
SIDE, AND IS SUBJECT TO ALL APPLICABLE RULES, REGULATIONS, LAWS AND ORDINANCES OF THE STATE OF COLORADO AND
THE LOCAL JURISDICTION(S) OF THE PROPERTY LOCATION, AND THE FOLLOWING TERMS AND CONDITIONS WHICH ARE MADE

of a ''written change order" and will bean extra charge over and/above the original contract price. Any representations, statement,
or other communication not written in this Agreement or the Disclaimer Agreement are agreed to be immaterial, and will not be
relied upon by either party, and do not survive the execution of this Agreement.
3. IN_It is agreed that the amount of the Agreement shall be the amount equal to the amount referred to as owner's insurance
claim summary on the front of this Agreement, plus any written change order(s), supplements and the terms contained within the
Disclaimer Agreement. Supplement money paid by the owner s insurance company for additional labor and materials needed
beyond the original scope of repairs shall be paid directly to the Company.
4. IN_You the owner(s) can cancel this transaction at anytime in writing prior to midnight of the third day after the date of this
transaction. To cancel this transaction, mail ordeliver a SIGNED, DATED, statement of cancellation to the above address as proof of
your intent to cancel prior to midnight of the third day after the date of this transaction.
5. IN_The final payment is due immediately upon work completion and shall not be held up while waiting for the city/county/state
to inspect the

All excess materials belong to the Company. The Company is authorized to do work on a day that is mutually agreeable to customer
and Company.
S. IN All agreements are contingent upon "Force Majeure." It is mutually agreed to by both partiesthat Force Majeure means and
refers to acts of God: strikes, lockouts, material shortages, or other industrial disturbances: acts of public enemies; orders of any
kind ofthe government ofthe United States, The state of Colorado, or any civil or military authority: insurrections: riots: epidemics:
landslides: lightening: earthquakes: fires: hurricanes: storms: floods: washouts: droughts: arrests: restraint of government and
people: civil disturbances: explosions: break -age or accidents to machinery, pipelines, canals or other facilities: partial or entire
failure of necessary utilities or any other causes not reasonably within the control ofthe party claiming such inability.
9. IN_lf any ofthe provisions -4this Agreement should be held to be invalid or unenforceable, the validity and enforceability of the
remaining provisions of this .,reement shall not be affected hereby.
10. IN The Company is not responsible for any damages below the roof due to leaks caused by wind in excess of the manufacturer's
warranty limit, ice dams, hail, and pre-existing or future construction defects, caused by storms or lark of maintenance during the
warranty period.
11. IN Should default be made in payment of this Agreement, charges shall be added from the date thereof at a rate of one and a
half percent (1 % %) per month with a minimum charge of five dollars ($5.00) per month, in the event that referral to an Attorney for
collection ofthe amounts owed underthis Agreement becomes necessary, or in the event that arbitration or other legal action is
necessary to enforce the provisions of this Agreement, the prevailing party shall be awarded costs and reasonable attorney's fees.
12. IN_ANY PERSON OR COMPANY SUPPLYING LABOR MATERIALS FOR THIS IMPROVEMENT TO YOUR PROPERTY MAY FILE A LIEN
AGAINST YOUR PROPERTY IF THAT PERSON OR COMPANY IS NOT PAID FOR THE CONTRIBUTIONS.
13. IN_ Company's current written workmanship guarantee and applicable written manufacturer's warranties on materials shall
apply and are not transferrable or assignable, unless otherwise specifically stated or agreed to in writing. To obtain
warranty /guarantee services, the contract amount must be paid in full.
14. A property owner who enters into a written contract with a Roofing Contractorto preform roofing work on the property owner's
residential property, the payment for which will be made from the proceedsfrom the property and casualty insurance policy, may
rescind the contract with -in 72 hours afterthe property owner receives written notice from the insurer that the claim for payment
for roofing work on the residential property is denied in whole or in part. The owner's right of rescission does not apply when the
insurer denies in whole or in part, a claim related to a request for supplemental roofing services.
15. Waiver of insurance deductible prohibited. A roofing contractorthat preforms roofing work, the paymentfor which will be made
from the proceeds of a property and casualty insurance policy, shall not advertise or promise to pay, waive, or rebate all or part of
any insurance deductible applicable to the claim for paymentfor roofing work on the covered residential property.
