Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in

the order they are received and due to the volume of requests, time to process

varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if

documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

TERMS AND CONDITIONS
The proposal appearing on the reverse side hereof is subject to the following terms and conditions. Upon acceptance by Purchaser
by
signature on the reverse side hereof, this proposal shall be deemed a contract between Purchaser and C&M Construction and Roofing, ,
hereinafter ("Contractor"). This contract is based upon specifications indicated on the reverse side hereof and, if applicable, upon plans and
specifications which are attached hereto and incorporated herein by this reference.
1. PURCHASER ACKNOWLEDGES: (a) Purchaser agrees to pay Contractor or its assigns the unpaid balance due upon substantial
completion to be paid upon all terms and conditions set forth herein; (b) Contractor will perform all work in a workmanlike manner, (c) Purchaser
is entitled to an exact copy of the signed contract; (d) Contractor is not responsible or Fable for any work performed in addition to the items
shown and described on this contract.
2. PURCHASER'S IMPLIED WARRANTY: (a) Purchaser agrees to cooperate and not hinder, delay or interfere with the progression
of work; (b) Purchaser agrees not to dictate the means or method of work; (c) Purchaser agrees that all concealed and unknown conditions such
as, but not limited to, electrical wiring, plumbing, telephone wiring, or defects discovered during performance are not the responsibility of
Contractor; (d) Purchaser is obligated to inform Contractor if personal finances or career changes occur during the time frame of the execution
and completion of this Agreement.
3. SURPLUS MATERIALS, All surplus materials are the property of C&M Construction & Roofing, LLC. All materials delivered by the
Contractor to the Purchaser's premises shall be stored and safely kept by the Purchaser and no rental or storage charges therefore shall be
made or assessed by Purchaser.
4. UTILITIES: --Purchaser will supply without charge, any power, electricity, water or other utilities to perform the work provided for
herein, Purchaser understands the removal and replacement of objects and window coverings on interior walls are their responsibility.
5. SUBCONTRACTOR COMPENSATION: Contractor assumes no responsibility or liability for payments made to its salesmen,
employees or subcontractors unless checks are made payable to C&M Construction and Roofing, LLC.
6. SIGN ING OF DOCUMENTS: If the contract is to be financed with a lending institution, Purchaser agrees to sign any and all required
documents, including but not limited to any credit application, note or deed of trust. The act of signing shall be done on the lending institutions's
contract, documents and forms. The Purchaser agrees to produce any and all required documents, including but not limited to tax returns and/or
pay stubs. These documents and forms shall merge with and become part of this contract as though more fully shown on the face hereof.
7. CONTRACTOR'S LIENS: Contractor retains all rights under the Colorado mechanic's lien laws. Contractor's acceptance of a note
as evidence of a debt or their taking of security shall not deprive the Contractor of rights under the Colorado mechanic's lien laws or any other
state's mechanic's lien laws to which said document(s) apply.
8. COMPLETION CERTIFICATE: Purchaser agrees to sign the Completion Certificate provided by Contractor immediately upon
substantial completion of work. The execution or failure to execute a Completion Certificate shall in no way effect the rights of the Contractor
to enforce this Agreement.
9. UNCONTROLLABLE DELAY: Contractor is not responsible for delay or inability to perform caused by strikes, acts of God, war, riots,
shortages, weather conditions, public authorities or other causes or casualties beyond its control or due to Purchaser's conduct.
10. BREACH OF CONTRACT: In the event of a breach of this contract by Purchaser, Purchaser will be liable for all damages caused
by the breach including, but not limited to, loss of gross profits and costs of litigation including attorney's fees. The election of remedies
hereunder shall be at the sole discretion of the Contractor.
11. LIMITED WARRANTY: Contractor warrants the materials are of the quality specified and will transfer to the Purchaser all
manufacturers' written warranties. Contractor warrants workmanship for two c2 years after the date of substantial completion and will remedy
substantial defects without charge to Purchaser on written notice from the Purchaser within such two (2) year eriod. A service call and

12. LATE CHARGES: If Purchaser fails to make any payment required hereunder, a late charge of one and one-half (1% %) percent
per month but in no event hi her than the maximum legal rate allowable under the laws of the State of Colorado will be assessed on any balance
remaining unpaid for ten (10) days or longer.
13. SUBSTANTIAL COMPLETION: For the purposes of this Contract, "substantial completion" shall be deemed to mean that point in
time when the Purchaser has normal usage of the products delivered or installed by the Contractor.
14, MODIFICATION: No modification to this Agreement shall be valid unless and until a writing reflecting the same has been signed
by both the Contractor and Purchaser. No agent of Contractor or Purchaser shall be authorized to execute any such modification unless
specifically authorized in writing by the Contractor or Purchaser as the case may be,
15. This Agreement shall be construed and performed in accordance with and governed by the laws of the State of Colorado. The
parties agree that for the purpose of any legal proceeding, they shall submit to the non-exclusive jurisdiction of the courts of the State of
Colorado and of the United States located in the City and County of Denver, State of Colorado.
16. THIS CONTRACT REPRESENTS THE ENTIRE UNDERSTANDING OF THE PARTIES HERETO. THERE ARE NO WRITTEN
OR ORAL UNDERSTANDINGS OR REPRESENTATIONS IN ADDITION TO OR MODIFYING THIS CONTRACT. NO OTHER WRITTEN OR

1. This permit was issued in accordance with the provisions set forth in your application and is subject to the laws of the
State of Colorado and to the Zoning Regulations and Building Codes of Wheat Ridge, Colorado or any other applicable
ordinances of the City.
2. This permit shall expire 180 days from the issue date. Requests for an extension must be received prior to expiration

3. If this permit expires, a new permit may be acquired for a fee of one -half the amount normally required, provided no
