Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding AM -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€T'"s installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for or an approval of, any violation of any provision of any
applicable code or any ordinance or regulation of this jurisdiction. Approval of work is subject to field inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received. YOU WILL BE CONTACTED WHEN YOUR PERMIT
IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE AND TIME
WINDOW TO COMPLETE THE TRANSACTION. You will be notified if your o
contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit. Permits are currently being (r
processed within 3-5 business days, subject to change based on volume.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

former General Contractor: Tom Teehan, Jobe Costa, Sonjia Teehan, and Shazon Vlanzer
with Casa Bella Custom Homes, Inc. Our problems include in part:
1. As you aze aware, we came to realize that although we paid our General Contractor
for work performed, they did not pay the sub-contractors as per our contract and
subsequently, a number of liens have been placed against the property.
2. Our Genera( Contractor violated the Work SWp Order posted by the City of WYteat
Ridge by continuing to build on the house, failing to inform us of its existence for a
number of weeks or of their noncompliance.
3. we recently discovered that the water line that runs from the street to the house was
compromised. The line was crimped, folded back toward the house and buried, making it
unusable and unrepairable. We aze now currently required to insta(1 a new water line
from the meter to the residence because of ttus act. The water line was operational
during the initial stages of demolition and constnxction because we had a tenant living in
the property. Consolidated VJater has since infortned us that they were never contacted to
shut off the water during any phase of construction. In addition, a wad of linen was
discovered within the sewer ]ine approximately 4.5 feet from the house to the street,
purposefully placed to obstruct the sewer line. Because of these two issues, we are
having to have all utilities to the house examined.

31, 2006. Were were asked to review several items that were not in compliance with the construcrion
plans. The following is a lisf of the items and our recommendations:
1. The roof was sheathed with 7/16" OSB in lieu of the 5/8" sheathing specified on the plans. Based
on our review of the roof framing and the grade stamp of the installed sheathing, the sheathing as
installed is adequate, and no repair is required.
2.. The lower level rear wall of the addirion is framed with 2x6's, and the sill plate overhangs to the
inside of the foundation 2%z", leaving 3" of bearing. on the foundation. Based on our review, this
condition is adequate to support the design loads. The revised detail for lhis section is shown on :
sheet Rl and specifies the use of addxtional expansion material between the foundation wall and
the slab on grade as well as exterior flashing.
3. Original wall at walkout, 2x4 at 16" on center, will adequately support the design loads. Wall is
to be furred out 2" for insulation purposes. i
4. At the eastem most basement beam, replace wood shims with steel shims at both the north and the
south end of the beam. Plate is to be cut and stee] shims placed allowing the beam to be lowered ~
correcting the hump in the floor. i
5. At the stairway; provide addifional (2)-1 '/a" x 7 Ya" LVL, additional (2)-2x8, (2)-2x4 post, and i
hardware per sheeY R2. This framing revision will allow for 6'-8" of headroom and will ~
adequaEely support the design loads. ~

7. As reviewed, the garage door framing with (2)-2x6 king studs at center and a trim stud on each

Thank you for meeting with me and answering my questions concerning the house at 10689 W, 35t'
Place, W heat Ridge CO, 80033. To recap our conversafion:
1. Even ihough work has continued at 10689 W. 35~ Place, Wheat Ridge, CO by Casa Bella Custom
Homes, Inc. the "Stap Work Order" Issued on May 24, 2006 has never been lifted because The City of
Wheat Ridge has never received:
A) A letler to the Ciry of Wheat Ridge from Casa Bella Custom Homes, Inc. addressing the problem of
their over excavation and iheir commitrnent to remedy.
B) A copy of the letter to the neigh6or from Casa Beila Custom Homes, Inc, addressing the proWem of
their over excavation and thea commitrnent to remedy.
C} A copy of the survey of the project site (Chad, you said that you did go over to see the stakes and
pins placed}.
Following are copies Tomco Home Partners, Ina (ie. Tahla Stephens) received from Casa Bella
CusYOm Homes, Inc. (ie. Tom Teehan and Jobe Costa) gave to me representing to me ihat they had
been given to whom they were addressed. I see ihat Casa Bella Custom Homes, Inc. has faifed to
provide their business address on the letterwhich is: 20974 E. Princeton Place, Aurora, CO 80013.
We also discussed the projecPs inspections. Because I was unable to find the inspection card at the
project site, Cindy graciously printed me a copy of the City's Records. Thank you for walking me
through it. You brought to my altention that the foundation did not pass its inspection. Because Casa
Bella Custom Homes, Inc. faifed to call you to come and inspect before the concrete was poured, you
were unable to see that all the rebar was in place as needed. Therefore, the City of Wheat Ridge

requires an inspection and lefter from a structural engineer addressing its oompliance to all building
regulations. I have spoken to my architect, Kevin Stockton with Creo Designs, and he will make sure
that you have this. I believe Ascent Structural Engineering is the company we used for addressing this
issue. I do not have a copy of that paperwork.

Casa BeIla will restore this property to the best of our ability, with no charge to
the current owner on or before completion of work.
2. Staking of pmperty boundaries.
Once we have compleEed the survey staldng of the easf side of the property, Casa
Bella will contact the city to for its review.
3. Remova] or retention of the excavated dirt to prevent soil being washed away
inappropriately.
Casa Bella will either retain the needed soil with netted and staked blankets,
or remove t3ie excess that will not be needed for backfill and finish of the
project.
