Conclusions: The footings were formed with the Big Foot forms at 36 inches of depth and the
piers on those were reinforced in compliance with the construction documents.

Observations: Framing of the deck.
Conclusions: The cantilever of the framing that was done to reposition a footing is acceptable.
The lateral loading from the code requirement for two 1500 lbs. attachments to the building can be
accomplished be using 4-1/2" diam. x 8" Hilt! HY270 epoxied rods or inserts (6" embedment) into.
the existing two course wide composite masonry wall.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in

By signing this agreement I/We authorize Modern Roofing LLC to pursue my best interest for my claim at a fair and accurate price
according to policy coverage. When a fair and accurate claim is agreed upon between the insurance company and Modern Roofing LLC,
it will become the contract price (Below) for all work completed by Modern Roofing LLC. This amount could be different from the
original authorized amount by your insurance company due to approved supplemental work as per policy coverage. All approved

insurance company denies the claim this contract will terminate and neither Modern Roofing LLC nor Property Owner/Insured are
bound to one another.
I hereby authorize Modern Roofing LLC to prepare/send their estimate and/or supplement to my insurance company to negotiate a "fair and
accurate" price. If [/We hire a public adjuster or attorney to assist in negotiating a fair and accurate price, I/We authorize said representation to
communicate directly with Modem Roofing LLC in order to effectuate a prompt settlement to the claim and restoration of the property. If the
insurance policy is ACV only, or if the settlement of the claim is not agreeable to the homeowner and/or Modem Roofing LLC, either party
reserves the right to rescind this agreement at no cost to either party. A fax, photo, pdf or any other electronic copy of this document, as well as

staring into a rea] wood fire. What appears to be a beautiful antique wood stove is actually the most technologically
advanced gas burning stove ever designed. The cast iron body (#1) conceals a sophisticated steel firebox (#2).
A convection heat exchanger (#3) circulates warmed room air (#4). An optiona] convection blower (#5) is available

The most outstanding feamre of this stove is the realism and efFiciency (86.5 oP the
Ember-Fyre'"burner. When natural gas (or propane) reaches the srove (#10), it
is piped into a high quality European SIT gas valve 11). This valve has a variable
turn down rate from 31,000 to 16,000 BTU's. The lower pressure gas flows
from the valve into the burner orifice (#15), a small opening specificaily designed
and machined to allow a precise gas flow into the mixing tube (#16). Outslde
air (#8) is drawn inro the mixing tube as well. The fuel/air mixture is adjusted a

and set using the air shutter control (#7 7). This control allows the stove ~
installer to compensate for variations in al6tude, fuel rype, and line pressure.
With the Ember-FyreT' burner, the wmbustion air (#8) is drawn from outside `
the house via the direct vent flue. This air is preheated and drawn into
the lower portion oP the sealed firebox 18). This gas/air mix[ure flows toward ~
the lower bum chamber (#19). The bum chamber is a hollow area Formed ~
between a metal pan and the patented Embeo-Fyre (#20) burner. From the sealed
chamber, the mixture flows through a unique array of precise burner ports (#21). ~
As this mixture ignites and bums, the Ember-FyreT"' burner heats to a deep red
1200° F degrees, realistically mimicking the appearance of wood embers. Dancing
yellow flames (#22) to further enhance the look and feel of a wood fire. Unlike
other gas burning stoves, the Ember-Fyre burner and logset is designed to provide
a beautiful realistic Pire and burner setting. R.

A unique option for the Berkshire Is the state-oF-the-art Modulating Remote
ControL You simply replace the manual regufaror with a servo motor 12)
and add the ll0 volt receiver (#13). This allows you to use the Modulatlng
Remote Control (#14) to vary heat output and flame height in 20% increments
(5 settings), and to adjust the output of the whisper quiet 130 CFM blower (3 set[ings).
The Modulating Remote Convol incorporates a programmable thermostat that automatically
turns the appliance off and on, maintaining your desired temperature setting.
Self Balancing Flue System
The Berkshire Stove installer is able to fur[her dial in the appearance of the Fire
by se[ting the Restrictor Link (#23) (patent pending). A single adjustment arm
allows the installer to finely tune both the intake (#24) and exhaust (#25) restrictors
at the same time, creating that "Showroom Fire° in your home. The Exhaus[ Siphon
(#26) (patenC pending) allows some of the ou[side air to be drawn directly into
the exhaust vent, allowing dilution air to regula[e the exhaust draft pressure during
gusty winds, extreme cold, and varying atmospheric conditions.
The burning gases are drawn [hrough an elaborate heat exchange system (#27).
As the Exchanger heats up, air in the chamber gains buoyancy and naturally
expands and rises ou[ of the front of the stove into the home. The heated air
is continually replaced by cooler room air being pulied into the base of the stove,
crea[ing a natural convection current.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
