Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.

1. This permit was issued based on the information provided in thepermit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.

company regarding damage to the Property. Owner understands that the Contractor
will assist the Owner in obtaining appropriate property insurance damage adjustments,
gaining approval of damage for repair and obtaining funds to repair Owner's Property.
Owner authorizes Owner's insurance company to discuss all matters directly with
Contractor, or their representative. Owner agrees to provide Contractor with any and all
insurance information, claim number, insurance loss data information and adjuster's
report provided to the Owner by the Insurance Company.
Upon authorization of the Owner's Insurance Company, for a price agreeable to the
Contractor, Owner agrees that the Contractor shall perform work prescribed by the
Insurance Company's loss data sheet or adjuster's report which includes the estimate
of repair as set forth in the insurer's claim file plus any upgrades selected by the Owner.
Owner is obligated to pay only the deductible required by the Owner's Insurance Company plus any upgrades requested by the Owner.
Owner further agrees to endorse insurance checks payable to the Contractor or to pay the Contractor an equal amount.
Owner owes the Contractor nothing in the event the Insurance Company does not authorize the repairs.
Contractor shall hold in trust any payment received from you until Contractor has delivered roofing materials at the site or has performed a majority of the
roofing work on your property.
Owner has the right to rescind this contract and obtain full refund of any deposit within 72 hours after entering the contract. Contractor cannot pay, waive or rebate
all or part of any insurance deductible applicable to the insurance claim for payment for roofing work on your property.
Otherwise, Owner understands that this is a binding agreement. If the Owner breaches this agreement before commencement of the work, Owner agrees to pay
the Contractor for the actual work performed.
Owner understands that, aside from upgrades, a summary of the work to be performed, a description of materials to be used or a list of standard features included,
the total contract price, or description of the basis on which the price will be calculated, is embodied in the insurance loss data information provided by the Owner's
insurer. Further, Owner authorizes the Contractor to submit supplemental requests to the Insurance Company for additional payment required for omitted items
from adjuster scope or due to price fluctuation caused by supply/demand issues orotherwise unknown at the time the contract was entered.
Owner warrants that this contract is signed without any reliance upon any representations or promises by the Contractor, or its agents, except those that are
specifically written in this contract, and that no additional promises or representations have been offered as inducement for signing. Owner also acknowledges

TERMS AND CONDITIONS
1. All contracts are subject to approval of Management and our Credit Department without exception.
2. All checks for payment must be made payable to Masterpiece Roofing. There will be NO exceptions. Owner will still be
liable to Masterpiece Roofing for payment of this contract if check is made out to any other entity or person.
3. If any provision of this agreement should be held to be invalid or unenforceable, the validity and enforceability of the
remaining provisions of this agreement shall not be affected thereby.
4• Owner further agrees that any insurance payments received by Owner are held in trust for the benefit of the Contractor.
5. Payment in full shall not be delayed while waiting for the city, county or appropriate building inspection department to
inspect the work. Owner understands that the city, county or appropriate building inspection department is not within the
control of the Contractor.
6. Contractor shall not be liable for failure of performance due to labor controversies, strikes, fire, weather, inability to obtain
material from usual sources or any other circumstances beyond the control of the Contractor, whether of a similar or
dissimilar nature.
7. Should a default be made in payment of this contract, charges shall be added from the date thereof at a rate of one and one
half percent per month (18% per annum) within a minimum charge of $2.00 per month and if placed in the hands of an
attorney for collection, all attorney's fees and costs shall be paid by the Owner.
8. If this Contract is cancelled by the Owner later than 72 hours from execution, and after insurance negotiations have begun,
the Owner shall pay Masterpiece Roofing twenty-five percent (25%) of the contract price as liquidated damages, not as a
penalty, to compensate Contractor for time and skill involved in the insurance claim process.
9. Owner may rescind this contract within 72 hours after the Owner receives written notice from Owner's Insurance Company
that the claim is denied in whole or in part and otherwise explained in C.R.S. §6-22-104. Owner shall give Notice of
Rescission in writing to Contractor by US Mail, email or personal delivery. Contractor shall return any deposit within to
days, minus amounts for actual work performed.
lo. The Contractor's Limited Warranty does not cover damage to roofs caused by lightning, gale (50 mph), hurricane, tornado,
hailstorm, impact of foreign object, other violent storm casualty or damage to roofs due to settlement, distortion, failure or
cracking of the roof sheathing, walls or foundation of the building.
11. The Contractor is not responsible for any damage below the roof due to leaks by excessive wind, ice or hail during the
period of the warranty. The Contractor's warranty covers the roof only.
12. The Contractor will not be held liable for any structural movement, settling, cracks in dry -wall, cracking or breaking of
driveway during the roofing or loading process and minor scratching or denting of gutters.
13. All pictures, wall hangings, lights, collectibles, etc., both indoors and out MUST be secured by the Owner. Any damage to
personal items during installation will not be the responsibility of the Contractor. The Contractor will not be responsible for
nail damage to tires, pets, etc. Pets should not be left unattended during installation.
14. Any alteration or deviation from specifications involving extra cost will be executed only upon written orders and will
become an extra charge over and above the estimate.
15. The Owner is to carry fire, tornado and other necessary insurance.
16. This contract and labor warranty shall not be assigned and are nontransferable unless specified in writing.
17. The Contractor shall revoke the labor warranty if this contract is not paid in full.
18. This Contract constitutes the entire understanding of the parties and no other understanding, collateral or otherwise, shall
be binding unless in writing and signed by both parties.
19. This agreement is composed of a front and back page and all materials incorporated herein by reference and shall be
considered the entire agreement by the parties. Any representations, statements or other communications not written in
this Contract are agreed to be immaterial and not relied on by either party and do not survive the execution of this contract.
2o. This Contractor has the right to order excess materials. All excess materials belong to the Contractor.
21. The Contractor reserves the right to file for supplemental insurance claims if the insurance adjuster's measurements are
used and prove to be incorrect.
22. At no additional cost to the Owner, the Contractor reserves the right to file for a supplemental insurance claim due to an
increase in costs.
23. Supplements paid by the Insurance Company for additional labor or materials needed beyond the original scope of repairs
are part of this contract as if contained herein and will be paid to Masterpiece Roofing either by the Owner or the Owner's
Insurance Company.
24. Owner acknowledges Masterpiece Roofing as a General Contractor and as such will be entitled to lo% overhead and lo%
profit if approved by the Insurance Company as part of the claim.
25. Owner is responsible for the Homeowners Association's (Covenant) approval of roofing products and color.
26. Masterpiece Roofing is not responsible for damage to stucco, siding or wall abutments as a result of installing new flashing.
27. Masterpiece Roofing is not responsible for satellite dish realignment.
28. Contractor carries workers' compensation, automobile and commercial general liability (bodily injury and property damage)
insurance. A Certificate of Insurance identifying our insurance carrier and the types and amounts of coverage and our
insurance contract information is attached or will be provided to Customer and is incorporated herein by reference and
should be considered as part of this contract.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard

will assist the Owner in obtaining appropriate property insurance damage adjustments,
gaining approval of damage for repair and obtaining funds to repair Owner's Property.
Owner authorizes Owner's insurance company to discuss all matters directly with
Contractor, or their representative. Owner agrees to provide Contractor with any and all
insurance information, claim number, insurance loss data information and adjuster's
report provided to the Owner by the Insurance Company.
Upon authorization of the Owner's Insurance Company, for a price agreeable to the
Contractor, Owner agrees that the Contractor shall perform work prescribed by the
Insurance Company's loss data sheet or adjuster's report which includes the estimate
of repair as set forth in the insurer's claim file plus any upgrades selected by the Owner.
Owner is obligated to pay only the deductible required by the Owner's Insurance Company plus any upgrades requested by the Owner.
Owner further agrees to endorse insurance checks payable to the Contractor or to pay the Contractor an equal amount.
Owner owes the Contractor nothing in the event the Insurance Company does not authorize the repairs.
Contractor shall hold in trust any payment received from you until Contractor has delivered roofing materials at the site or has performed a majority of the
roofing work on your property.
Owner has the right to rescind this contract and obtain full refund of any deposit within 72 hours after entering the contract. Contractor cannot pay, waive or rebate
all or part of any insurance deductible applicable to the insurance claim for payment for roofing work on your property.
Otherwise, Owner understands that this is a binding agreement. If the Owner breaches this agreement before commencement of the work, Owner agrees to pay
the Contractor for the actual work performed.
Owner understands that, aside from upgrades, a summary of the work to be performed, a description of materials to be used or a list of standard features included,
the total contract price, or description of the basis on which the price will be calculated, is embodied in the insurance loss data information provided by the Owner's
insurer. Further, Owner authorizes the Contractor to submit supplemental requests to the Insurance Company for additional payment required for omitted items
from adjuster scope or due to price fluctuation caused by supply/demand issues orotherwise unknown at the time the contract was entered.
Owner warrants that this contract is signed without any reliance upon any representations or promises by the Contractor, or its agents, except those that are
specifically written in this contract, and that no additional promises or representations have been offered as inducement for signing. Owner also acknowledges

TERMS AND CONDITIONS
1. All contracts are subject to approval of Management and our Credit Department without exception.
2. All checks for payment must be made payable to Masterpiece Roofing. There will be NO exceptions. Owner will still be
liable to Masterpiece Roofing for payment of this contract if check is made out to any other entity or person.
3. If any provision of this agreement should be held to be invalid or unenforceable, the validity and enforceability of the
remaining provisions of this agreement shall not be affected thereby.
4• Owner further agrees that any insurance payments received by Owner are held in trust for the benefit of the Contractor.
5. Payment in full shall not be delayed while waiting for the city, county or appropriate building inspection department to
inspect the work. Owner understands that the city, county or appropriate building inspection department is not within the
control of the Contractor.
6. Contractor shall not be liable for failure of performance due to labor controversies, strikes, fire, weather, inability to obtain
material from usual sources or any other circumstances beyond the control of the Contractor, whether of a similar or
dissimilar nature.
7. Should a default be made in payment of this contract, charges shall be added from the date thereof at a rate of one and one
half percent per month (18% per annum) within a minimum charge of $2.00 per month and if placed in the hands of an
attorney for collection, all attorney's fees and costs shall be paid by the Owner.
8. If this Contract is cancelled by the Owner later than 72 hours from execution, and after insurance negotiations have begun,
the Owner shall pay Masterpiece Roofing twenty-five percent (25%) of the contract price as liquidated damages, not as a
penalty, to compensate Contractor for time and skill involved in the insurance claim process.
9. Owner may rescind this contract within 72 hours after the Owner receives written notice from Owner's Insurance Company
that the claim is denied in whole or in part and otherwise explained in C.R.S. §6-22-104. Owner shall give Notice of
Rescission in writing to Contractor by US Mail, email or personal delivery. Contractor shall return any deposit within to
days, minus amounts for actual work performed.
lo. The Contractor's Limited Warranty does not cover damage to roofs caused by lightning, gale (50 mph), hurricane, tornado,
hailstorm, impact of foreign object, other violent storm casualty or damage to roofs due to settlement, distortion, failure or
cracking of the roof sheathing, walls or foundation of the building.
11. The Contractor is not responsible for any damage below the roof due to leaks by excessive wind, ice or hail during the
period of the warranty. The Contractor's warranty covers the roof only.
12. The Contractor will not be held liable for any structural movement, settling, cracks in dry -wall, cracking or breaking of
driveway during the roofing or loading process and minor scratching or denting of gutters.
13. All pictures, wall hangings, lights, collectibles, etc., both indoors and out MUST be secured by the Owner. Any damage to
personal items during installation will not be the responsibility of the Contractor. The Contractor will not be responsible for
nail damage to tires, pets, etc. Pets should not be left unattended during installation.
14. Any alteration or deviation from specifications involving extra cost will be executed only upon written orders and will
become an extra charge over and above the estimate.
15. The Owner is to carry fire, tornado and other necessary insurance.
16. This contract and labor warranty shall not be assigned and are nontransferable unless specified in writing.
17. The Contractor shall revoke the labor warranty if this contract is not paid in full.
18. This Contract constitutes the entire understanding of the parties and no other understanding, collateral or otherwise, shall
be binding unless in writing and signed by both parties.
19. This agreement is composed of a front and back page and all materials incorporated herein by reference and shall be
considered the entire agreement by the parties. Any representations, statements or other communications not written in
this Contract are agreed to be immaterial and not relied on by either party and do not survive the execution of this contract.
2o. This Contractor has the right to order excess materials. All excess materials belong to the Contractor.
21. The Contractor reserves the right to file for supplemental insurance claims if the insurance adjuster's measurements are
used and prove to be incorrect.
22. At no additional cost to the Owner, the Contractor reserves the right to file for a supplemental insurance claim due to an
increase in costs.
23. Supplements paid by the Insurance Company for additional labor or materials needed beyond the original scope of repairs
are part of this contract as if contained herein and will be paid to Masterpiece Roofing either by the Owner or the Owner's
Insurance Company.
24. Owner acknowledges Masterpiece Roofing as a General Contractor and as such will be entitled to lo% overhead and lo%
profit if approved by the Insurance Company as part of the claim.
25. Owner is responsible for the Homeowners Association's (Covenant) approval of roofing products and color.
26. Masterpiece Roofing is not responsible for damage to stucco, siding or wall abutments as a result of installing new flashing.
27. Masterpiece Roofing is not responsible for satellite dish realignment.
28. Contractor carries workers' compensation, automobile and commercial general liability (bodily injury and property damage)
insurance. A Certificate of Insurance identifying our insurance carrier and the types and amounts of coverage and our
insurance contract information is attached or will be provided to Customer and is incorporated herein by reference and
should be considered as part of this contract.

(2) This permit shall expire if (A) the work authorized is not commenced within sixty (80) days from issue date or (8) the building authorized is suspended or

(3) If this pertnit expires, a new permit may be acquired for a fee of one-half ihe amount normaily required, provided no changes have been orwill be made in ihe
original plans and specifcations and any suspension or abandonment has not exceeded one (1) year. If changes are made or if suspension or abandonment
exceeds one (7) year, fulffees shall be paid for a new pertnit (4) No work of any manner shall be done that will change ihe naturel flow of water causing a drainage problem.
(5) Contractor shall notiry the Building Inspector hventy-four (24) hours in advance for all inspedions and sha0 receive written appmval on inspection card before
proceediing with successive phases of the job.
