Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

E. PAYMENT TERMS: If the work is completed within thirty (30) days of its commencement, payment in full is due on or before the 15' day of the month
following the completion date. In all other cases, payment of 90 percent of the work completed shall be made on or before the 151 day of the following month,
with final payment due thirty (30) days after completion. Non-payment in accordance with the above shall be cause for terminating performance. If payments
are not made when due, interest, costs incidental to collection and attorney fees (if an attorney is retained for collection) shall be added to the unpaid balance.
Interest shall accrue at the rate of 1-1/2 percent per month on the unpaid balance.
F. Contractor shall hold in trust any payment received from you until Contractor has delivered roofing materials at the site or has performed a majority
of the roofing work on your property.
G. You have the right to rescind this contract and obtain a full refund of any deposit within 72 hours after entering the contract. If you plan to use the proceeds of a
property and casualty insurance policy to pay for the roofing work, you may rescind this contract within 72 hours after you receive written notice from the
property and casualty insurer that your claim has been denied in whole or in part. However, Roof Check is entitled to retain payments or deposits to compensate
Roof Check for roofing work actually performed in a workmanlike manner consistent with standard roofing industry practices. Roof Check cannot pay, waive,
rebate, or promise to pay, waive or rebate all or part of any insurance deductible applicable to the insurance claim for payment for roofing work on your
property.

TERMS AND CONDITIONS
1.Roof Check, Inc. ("Roof Check") assumes no responsibility for structural integrity of the roof deck or the building on which the roofing is
to be installed. Customer represents all structures to be in sound condition capable of withstanding normal roofing construction and
operations.
2.This proposal and contract is based upon the work to be performed by Roof Check not involving lead paint, toxic or asbestos -containing
materials and that such materials will not be encountered. In the event that such materials are encountered, Roof Check shall be entitled to
reasonable compensation for additional expenses due to lead paint, toxic or asbestos -containing materials.
3.Roof Check cal -ries worker's compensation, automobile and commercial general liability (bodily injury and property damage) insurance.
Ewing -Leavitt Insurance Agency is Roof Check's insurance carrier and can be contacted through Ewing -Leavitt Insurance Agency is RCI's
insurance carrier and can be contacted through Ewing -Leavitt Insurance Agency. 4532 Boardwalk Drive Suite 200. Ft. Collins, CO. 'Owner
is to carry property and casualty insurance, including coverage for fire, wind damage and vandalism.
4.Customer may order changes and the contract price adjusted accordingly. When removing the existing roof, conditions requiring additional
work, such as rotten or deteriorated wood, are often found. If the scope of work stated on the face of this proposal does not specifically cover
repairs or replacement of decking or similar defective conditions, Roof Check will report the condition to the Customer, take such steps as are
reasonably necessary to protect the building and the contract scope and price will need to be adjusted. Any alteration to or deviation fiom the
specifications described on the face of this proposal involving extra costs will be billed as an extra.
5.If gutter and/or downspouts are to be removed to facilitate a re -roof, Roof Check shall not be held responsible for damage caused by
removal and/or re -installation of gutter and/or downspouts.
6.Roof Check is not responsible for loss, damage or delay caused by circumstances beyond its reasonable control, including but not limited to
acts of God, weather, accidents, fire, regulation, strikes, failure or delay of transportation, shortage of or inability to obtain materials. Roof
Check is not required to remove water or snow from the roof.
7.Customer acknowledges that re -roofing may cause vibration, disturbance, dust or debris to fall into the interior. Customer agrees to remove
or protect property directly below the roof in order to minimize potential interior damage. Roof Check shall not be responsible for
disturbance, damage, clean-up or loss to interior property that Customer did not remove or protect prior to commencement of roofing
operations. Customer shall notify tenants of re -roofing and the need to provide protection underneath areas being re -roofed. Customer agrees
to hold Roof Check harmless from claims of tenants who were not so notified and did not provide protection.
8.All materials and work shall be furnished in accordance .with normal industry tolerances for color, variation, thickness, size, weight,
amount, finish, texture and performance standards. Specified quantities are intended to represent an average over the entire roof area.
9.Roof Check and Customer are committed to acting promptly so that roof leaks are not a source of potential interior mold growth. Customer
will make periodic inspections for signs of water intrusion and act promptly, including notice to Roof Check, if Customer believes there are
roof leaks. Roof Check is not responsible for indoor air quality. Customer shall hold harmless and indemnify Roof Check from claims
arising out of or relating to property damage, personal injury, illness, allergic reactions, disease and /or death resulting from the occurrence or
exposure to interior mold, mildew and fungi.
10.Roof Check is not responsible for adequacy of drainage or ponding on the roof. Roof Check does not provide engineering, consulting,
architectural or design services. It is the Owner' s responsibility to retain a licensed architect or engineer to determine proper design and
code compliance. Roof Check is not responsible for design. Additional costs due to code changes and code requirements will be charged
over and above this contract price.

11.Roof Check's work will be warranted by Roof Check in accordance with its standard warranty. A copy of Roof Check's standard warranty
is attached or, if not, will be furnished upon request. ROOF CHECK SHALL NOT BE LIABLE FOR SPECIAL, PUNITIVE, INCIDENTAL
OR CONSEQUENTIAL DAMAGES. The acceptance of this proposal by the Customer signifies his agreement that this warranty shall be
and is the exclusive remedy against Roof Check pertaining to the roof installation.
12.If a dispute shall arise between Roof Check and Customer with respect to any matters or questions arising out of or relating to this
Agreement or the breach thereof, such dispute, other than collection matters which may be handled through arbitration or normal collection
procedures, shall be decided by arbitration administered by and in accordance with the Construction Industry Arbitration Rules of the
American Arbitration Association and must be initiated no later than two (2) years after Roof Check completed the roof installation. This
Agreement to arbitrate shall be specifically enforceable under the prevailing arbitration law. The award rendered by the arbitrators shall be
final, and judgment may be entered upon it in any Court having jurisdiction thereof.

(1) This permit was issued in accordance with the provisions set forth in your application and is subject to the laws of the State of
Colorado and to the Zoning Regulations and Building Codes of Wheat Ridge, Colorado or any other applicable ordinances of
the City.
(2) This permit shall expire if (A) the work authorized is not commenced within sixty (60) days from issue date or (B) the building
authorized is suspended or abandoned for a period of 120 days.
(3) If this permit expires, a new permit may be acquired for a fee of one-half the amount normally required, provided no changes
have been or will be made in the original plans and specifications and any suspension or abandonment has not exceeded one
(1) year. If changes have been or if suspension or abandonment exceeds one (1) year, full fees shall be paid for anew permit
(4) No work of any manner shall be done that will change the natural flow of water causing a drainage problem.
(5) Contractor shall notify the Building Inspector twenty-four (24) hours in advance for all inspections and shall receive written
approval on inspection card before proceeding with successive phases of the job.
(6) The issuance of a permit or the approval of drawings and specifications shall not be construed to be a permit for, nor an
approval of, any violation of the provisions of the building codes or any other ordinance, law, rule or regulation.
