Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

2. This. permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and

Residential Roofing Permit Application
This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE
AND TIME WINDOW TO COMPLETE THE TRANSACTION. You will be notified if
your contractor's license or insurance has expired, and you may update those
documents at the time you are issued your permit.
For all other requests:
Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in
person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

This contract is between the client and Integrity Building Solutions
of 6041 Demott Ave Commerce City, Co 80022.
Description of Services
iNTEGRITY
■U�LDINB SOIUTIOMS
Upon the signing of the contract Integrity Building Solutions will begin the services as described in the
attached estimate. These services will be performed on the property that is noted in the estimate under
the address.
Scope of Work
Integrity Building Solutions shall provide all of the labor and materials, to do the services described in
the attached estimate. The roofing work will be performed Monday through Saturday, statutory
holidays excluded, unless the parties mutually agree otherwise, provided that the services will be
performed only if weather conditions are favorable, in order to ensure an acceptable finished product.
Work Site
The client hereby authorizes Integrity Building Solutions to commence and complete the usual and
customary excavation and grading on the work site as may be required in the judgement of Integrity
Building Solutions to complete the roofing work. Unless called for in the drawings or specifications, no
landscaping, finish grading, filling or excavation is to be performed at the work site by Integrity Building
Solutions.
Payment
Payments shall be made payable to Integrity Building Solutions. The client agrees to the amount set
forth in the attached estimate. Integrity Building Solutions does not take any payment until the roof has
been completed. We will accept cash, check, and in most cases credit card. Everything is due upon
completion of the project. If any invoice is not paid when due, interest will be added to and payable on
all overdue amounts at a compounding 5% a month, or the maximum percentage allowed under
applicable laws, whichever is less. The client agrees to pay all costs of collection, including without
limitation, reasonable attorney fees.
In addition to any other right or remedy provided by law, if the client fails to pay for the services when
due, Integrity Building Solutions has the option to treat such failure to pay as a material breach of this
contract, and may cancel this contract and/or seek legal remedies.
Permits
Integrity Building Solutions shall apply for and obtain such permits and regulatory approvals as may be
required by the local municipal/county government, unless the client otherwise indicates, the cost
thereof shall be included as part of the payment to Integrity Building Solutions. However, the client is
responsible for getting all work approved through their HOA.

Insurance
Integrity Building Solutions shall maintain general liability insurance and worker's compensation
insurance in accordance with the minimum requirements of the state throughout the duration of the
services. Integrity Building Solutions shall provide the client with proof of insurance upon their request.
Survey and Title
The client will indicate the property lines to Integrity Building Solutions and will provide boundary stakes
by a licensed land surveyor if the clients are in doubt about the property boundaries.
Change Orders
The client may make changes to the scope of the work from time to time during the term of this
contract. However, any such changes or modification shall only be made by written Change order
signed by both parties. Such change orders shall become part of the contract. The Client agrees to
pay any increase in the cost of the roofing work as a result of a change order. In the event the cost of a
changed order is not known at the time a change order is executed, Integrity Building Solutions shall
estimate the cost thereof and the client shall pay the actual cost whether or not is is in excess of the
estimated cost.
Access
The client will allow free access to work areas for workers and vehicles and will allow areas for the
storage of materials and debris. Driveways will be kept clear for the movement of vehicles during work
hours. Integrity Building Solutions will make reasonable efforts to protect driveways, lawns, shrubs, and
other vegetation, however it is common for some damage to occur during construction and Integrity
Building Solutions is not responsible for fixing it.
Term
This contract will terminate automatically upon completion by Integrity Building Solutions of the services
required by this contract.
Default
The occurrence of any of the following shall constitute a material default under this contract:
A. The failure to make a required payment when due.
B. The insolvency or bankruptcy of either party.
C. The subjection of any of either party's property to any levy, seizure, general assignment for the
benefit of creditors, application or sale for or by any creditor or government agency.
D. The failure to make available or deliver the services in the time and manner provided for in this
contract.
Remedies
In addition to any and all other rights a party may have available according to law if a party defaults by
failing to substantially perform any provision, term or condition of this contract (including without

limitation the failure to make a monetary payment when due), the other party may terminate the
contract by providing written notice to the defaulting party. This notice shall describe with sufficient
detail the nature of the default. The party receiving such notice shall have 14 days from the effective
date of such notice to cure the defaults. Unless waived by a party providing notice, the failure to cure
the defaults within such time period shall result in the automatic termination of this contract.
Force Majeure
If performance of this contract or any obligation under this contract is prevented, restricted, or interfered
with by causes beyond either party's reasonable control (force majeure), and if the party unable to carry
out its obligations gives the other party prompt written notice of such event, then the obligations of the
party invoking this provision shall be suspended to the extent necessary by such event. The term
"force majeure" shall include, without limitation, acts of God, fire, explosion, vandalism, storm or other
similar occurrence, orders or act of God, fire, explosion, vandalism, storm or other similar occurrence,
orders or acts of military or civil authority or by national emergencies, insurrections, riots, or wars or
strikes, lockouts, work stoppages, or other labor disputes, or supplier failures. The excused party shall
use reasonable efforts under the circumstances to avoid or remove such causes of nonperformance
and shall proceed to perform with reasonable dispatch wherever such causes are removed or ceased.
An act or omission shall be deemed within the reasonable control of a party if committed, omitted, or
caused by such parry, or its employees, officers, agens or affiliates.
Arbitration
Any controversies or disputes arising out of or relating to this contract shall be resolved by binding
arbitration in accordance with the then -current commercial arbitration rules of the American Arbitration
Association. The parties shall select a mutually acceptable arbitrator knowledgeable about issues
relating to the subject matter of this contract. In the event the parties are unable to agree to such a
selection, each party will select an arbitrator and the two arbitrators in tum shall select a third arbitrator,
all three of whom shall preside jointly over the matter. The arbitration shall take place at a location that
is reasonably centrally located between the parties, or otherwise mutually agreed upon by the parties.
All documents, materials, and information in the possession of each party that are in any way relevant
to the dispute shall be made available to the other party for review and copying no later than 30 days
after the notice of arbitration is served. The arbitrator(s) shall not have the authority to modify an
provision of this contract or to award punitive damages. The arbitrator(s) shall have the power to issue
mandatory orders and restraint orders in connection with the arbitration. The decision rendered by the
arbitrator(s) shall be final and binding on the parties, and judgement may be entered in conformity with
the decision in any court having jurisdiction. The agreement to arbitration shall be specifically
enforceable under the prevailing arbitration law. During the continuance of any arbitration proceeding,
the parties shall continue to perform their respective obligations under this contract.
Entire Agreement
This contract contains the entire agreement of the parties, and there are no other promises of
conditions in any other agreement whether oral or written concerning the subject matter of this contract.
This contact supersedes any prior written or oral agreements between the parties.

Severability
If any provision of this contract will be held to be invalid or unenforceable for any reason, the remaining
provisions will continue to be valid and enforceable. If a court finds that any provision of this contact is
invalid or unenforceable, but that by limiting such provision it would become valid and enforceable, then
such provision will be deemed to be written, construed, and enforced as so limited.
Amendment
This contract may be modified or amended in writing, if the writing is signed by the party obligated
under the amendment.
Governing Law
This contract shall be construed in accordance with the laws of the State of Colorado.
Notice
Any notice or communication required or permitted under this contract shall be sufficiently given if
delivered in person or by certified mail, return receipt requested, to the address set forth in the opening
paragraph or to such other address as one party may have furnished to the other in writing.
Waiver of Contractual Right
The failure of either party to enforce any provision of this contract shall not be construed as a waiver or
limitation of the that party's right to subsequently enforce and compel strict compliance with every
provision of this contract.
Signatories
This agreement shall be signed on behalf of the client by whomever will be issuing payment and on
behalf of Integrity Building Solutions by Andrew Goslovich and effective as of the date given in the
attached estimate.
