Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR

Homeowners wishing to obtain a roofing permit must apply for the permit in person
at City Hall. Revisions to existing permits (for example, to add redecking) must be
completed in person at City Hall. All other non -roofing permits must be completed in

person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

on the front of this contract; or (b} any additional work that maybe ordered by the
building department having jurisdiction over the site of this project.

CONTRACTOR on this project, CUSTOMER agrees, upon the request at
CONTRACTOR to execute a Certificate of Substantial Completion in the form
submitted by CONTRACTOR upon substantial completion of the project herein

any obligation pursuant to this acceptance shall be deemed a breach of this
Acceptance or create any liability if the same shall arise from any cause beyond the

storm, flood, earthquake, strikes or other work stoppage, war, difficulties from any

state, county, city and local building permits.
Governing Law/Cholce of Venue: This acceptance and the legal relations between
CONTRACTOR and CUSTOMER shall be construed In accordance with the laws of
Colorado. CUs I OmER'S signature warrants that for purpose of determining vel ue
for bringing any legal action on the account that venue shall be either DENVER or
ADJAms county at the election of CONTRACTOR,
Waiver/ Severability. Any delay, waiver or omission by either party to exercise any
night or power arising from breach or default by the other party of the terms,
provisions or covenants of the Acceptance shall not be construed, except as herein
otherwise set forth, as a waiver by either patty of subsequent breach or default of
the same or other terms provisions or covenants of the other party. Should any
provision hereof be deemed contrary tuthe laws of Colorado such provision shaft
be deemed severable from the acceptance and not affect the enforceability sof the
balance of this Acceptance.
Warranty- Work Includes a three (3) year warranty ort labor and workmanship
unless stated otherwise in the Contract. Warranties on materials furrushed by
CONTRACTOR are limited to the manufacturer's warranty and do not cove abuse,
neglect alterations or damages caused by others_

damages arising out of, connected with or resulting from the work performed by

the project to which the claim relates:
Premise Availability. CUSTOMER will make the property upon which the Work is to
be performed available to CONTRACTOR from 7:30 a.m. until 1:30 p.m. Monday
through. Friday and all agreed upon times as modif e.d and agreed to in writing
on page (1) of this contract. CUSTOMER understands that roofing activities will result
in elevated noise levels and debris. CONTRACTOR will insure that the work area will
be cleaned up upon final completion of the work as outlined in this contract.
Materials. if materials for this Project/Work are ordered and cancelled before work
on the Project commences, CUSTOMER is responsible for any restocking costs and
inbound and returned shipping charges to CONTRACTOR. If materials are specially
ordered to CUSTOMER'S specifications, CUSTOMER is responsible for the entire
value ofthe order as soon as production of said materials begins- CONTRACTOR will
retain title to any materials that may be furnished until final payment is made and if
settlement is not agreed to, CONTRACTOR will be held harmless for any damages
resulting from the removal thereof. CONTRACTOR will retain title to ail unused
materials upon completion of the work.
Assignment and Delegation. The CUSTOMER may not assign any rights or interest to
This Contract without the prior- written consentt and approval of CONTRACTOR, No
delegation of any obligation owed to CONTRACTOR nor the performance of any
obligation of CUSTOMER to CONTRACTOR shall be made without CONTRACTOR's
prior written consent
Indemnifications CUS fOMER agrees to indemnify and hold CONTRACTOR harmless
from any and all costs (including attorney's fees), expenses and damages resulting
from any claim brought against CONTRACTOR by virtue of any negligence or fault of
CUSTOMER including, but not limited to, claims of personal injury.
Entire Agreement Amer dmertts: This acceptance constitutes the entire final
binding agreement between CONTRACTOR and CUSTOMER with respect to the
project (i.e. work and materials) itemized on page 1 of this fora i and supersedes any
and all prior orders and agreements between the parties, either verbal or written.
Any alteration or deviation from thc* provisions on front of this contract will be
valid only if made upon written Change Order signed on behalf of CUSTOMLR and
CONTRACTOR, and will be subject to extra charges raver and above the amount set
forth in the Contract. This Contract shall become a contract between CONTRACTOR
and CUSTOMER upon signature by CUSTOMER and is subject to the force Majeure
provisions hereinabove set forth, Copies of this document disclosing signatures of
the parties shall have the legal effect of any original. This Contract may he executed
in counterparts with the same ford and effect as though all signatures appeared on
one original document. Facsimile signatures are binding and enforceable.
Tearoff CfaM. CONTRACTOR will exercise reasonable care and diligence
in the removal of the existing materials. We strongly advise customer to
remove sensitive wall hangings, ceiling suspended articles, to cover furniture
and otherwise protect their property since CONTRACTOR cannot be
responsible for replacement or repair of these items, CONTRACTOR cannot.
unless otherwise specifically stated, be responsible for replacement or repair
of associated areas such as fascia, gutter. siding, trim. old flashings or any
previously damaged or deteriorated building components during the normal
tear off procedure, CONTRACTOR'S employees' negligence excluded.
CONTRACTOR cannot accept responsibility for any damages done to the roof
by CUSTOMER, other tradesmen or any others not directly employed by
CONTRACTOR.CONTRACTOR is not responsible for any damages which
occur to owner's property because of detects, in architectural, engineering,
construction plans and drawings or with respect to any failure to Comply with
any applicable building codes through construction occurring under such
plans, CONTRACTOR is specifically not performing design services and is
specifically not responsible for damages caused by occurrences such as
mold, mildew, condensation, loose flue pipes or other similar occurrences. To
