subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the originalpermit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.

5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, an violation of any provision of any
applicable code or any ordman a or regulation of this jurisdiction. Approval of work is subject to field inspection.

Due tothe potential ofthe possible expansion ofthe bathroom requiring approximately SOsquare feet
of wall removal and the age, appearance and texture of wall materials, I did bulk sampling of the
respective areas in question. The suspect materials were sampled on August 25, 2017.
A total of four (4) bulk samples of suspect ACM were collected of the materials in question to provide
adequate representation ofthe homogenous materials inquestion,
All samples were analyzed by Polarized Light Microscopy (PLK8)at Reservoirs Environmental Inc., an
American Industrial Hygiene Association (AIHA) accredited laboratory, Samples demonstrate that the

Mechanical: Move exhaust fan location, install new 80 cfm exhaust fan, improve exhaust venting to
outside.
Shower construction: Build shower curb and install dry -pack mortar shower pan, 3'8" by 2'6", using
Schluter Kerdi waterproofing membrane and drain assembly. Construct shower walls with fiber cement
board with liquid waterproofing membrane (e.g. Redguard). Install tile on shower floor and walls.
Install glass shower door and partitions.
Drywall: Install new % inch drywall on bathroom walls and new living room partition walls. % inch mold -
resistant drywall on bathroom ceiling.
Overall improvements to meet current building codes (existing bath does not meet current codes, for
example non —GFCI outlet, electrical circuit shared with kitchen outlets, existing shower leaks into crawl
space and is unusable, sink has S -trap, fixture clearances too small, plumbing drain lines are various
wrong sizes and mixed materials).
Current bathroom is 26 SF, proposed bathroom is 42 SF.
BULK SAMPLING SCOPE OFWORK:
David Still is an EPA, AHERA, & CDPHE accredited Asbestos Building Inspector by experience, training,
and testing,
David Still's inspection/assessment incorporated non-clestructive sampling techniques and visual
inspections in areas which were visual/accessible. Conditions and/or not inspected and/or commented
on may very well differ from those which were inspected and/or commented on.
David Still selected sample locations and frequency of sampling was based Vnobservations and
requirements, functional spaces and/or the assumption that like materials by appearance, texture, age
in the same areas are homogenous (HA). The limited bulk samples were collected inaccordance with
EPA/AHERA&[DPHERegulation 0recommended procedures.
BULK SAMPLE ANALYSIS:
Bulk material samples may have been analyzed utilizing Polarized Light Microscopy (PLM) analysis and
dispersion staining techniques according toUSEPA guidelines (EPA/6UOR-83/1lG).Thisguideline was
developed for use onfriable building materials and is not recommended by EPA for non -friable building
materials such as floor tiles. Additional analysis is recommended by EPA, such as Transmission Electron
Microscopy (TEM) to confirm negative Polarized Light Microscopy (PLM) results on floor tiles. TEM
analysis requires specific requests from the client due tocost differential.
Bulk material samples may have been analyzed utilizing PLM Point Counting analysis according to the
National Emissions Standard for Hazardous Air Pollutants (NESHAP).
Any materials that contain over one percent (1%) of any type of asbestos is considered Asbestos
Containing Materials (ACM)orAsbestos Containing Building Materials (ACBK4) and must be handled
according to State, OSHA, and EPA regulations if disturbed.
According to EPA (NESHAP) as published in the Federal Register, 40 CFR, Part 61, Subpart M, and CDPHE
Regulation 8, if the asbestos content is less than ten percent (10%) as determined by a method other
than point counting by Polarized Light Microscopy (PLM), verify the asbestos content by point counting
using PLM.
For purposes of this Environmental Asbestos Building Inspection/Assessment, friable Asbestos
Containing Materials which are <1.0% (TR) by PLM and friable Asbestos Containing Building Materials
which are 1.0% or less by PLM are considered positive, however, point counting byPLM iarequired
should renovation ordemolition ofthese materials occur,

REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

2. This. permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
