1. This permit was issued based on the information provided in the permit application and accompanying plans and specifications and is
subject to the compliance with those documents, and all applicable statutes, ordinances, regulations, po Flans
and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and
received prior to the date of expiration. An extension of no more than 180 days made be granted at the discretion of the Chief Building
Official and may be subject to a fee equal to one-half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to the standard
requirements, fees androcedures for approval of any new permit. Re -issuance or extension of expired permits is at the sole discretion of
the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a changeof the natural flow of water without prior and specific approval.
5. The permit holder shall notify the Building and Inspection Services Division in accordance with established policy of all required
inspections and shall not proceed or conceal work without written approval of such work from the Building and Inspection Services
Division.
6. The issuance or granting of a permit shall not be construed to be a permit for, or an approval of, an violation of any provision of any
�plicable code pi; any ordinance or regulation of this jurisdiction. Approval of work is subject to field inspection.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd

At Tiley Roofing we like to provide our clients with several alternatives to ensure you are getting the roof that best tits your needs. Below is a
choice of a "Better" and "Best' package, several shingle product choices, and add on options to accommodate those needs.
ISCOPE OF WORK AND MATERIALS .
Existing Roof Condition: Hail damage was noted on the roof, penetrations, gutters, and Rat roof.
Existing Leaks with Location: Leak reported on flat roof connected to house.
Demolition: Removal and disposal of existing layers (roof covering) specified above and 1 layer roof underlayment. Additional hidden layers
may be difficult to determine until demolition process. Charges for these hidden layers may apply.
Better - Standard Roof Package: Sloped Shingles ONLY (Excludes Garage 8 Flat)
Underlayment: 1 layer of synthetic underlayment with 1 row Ice and water on eaves installed per local building, 1 row at crickets, chimneys
and skylights (if they exist).
Flashing: 26 gauge factory finish bronze (dark brown) color drip edge, rake edge flashing, headwall, and step flashing. Replacement of all
roofing jacks which will be painted to match shingles. Surface mounted visible flashing may be required at roof to wall intersections.
Venting: Ridge vent to replace existing.
Nailing: 6 nails per shingle.
Shingles: See choices below.
Hip/Ridge: Raised Hip/Ridge to match existing.
Warranty: Five - Year Workmanship through Tiley Roofing.

Gutters/Downspouts: 26 gauge pre -painted 5" K -style gutters and 2"x3" corrugated downspouts. The same gutter configuration will be used
unless otherwise instructed.
Gutter Screen: Metal gutter screen to reduce debris build-up in the gutters. Gutter screen still requires some maintenance and may cause
additional icicle formation.
Chimney Cap: Furnish and install sheet metal cap on the chimney in your color of choice (stock standard 26g).
Solar Powered Fan: Furnish and install roof mounted 500 CFM (800 sq. ft.) solar powered fan center back side of roof deck.
Chimney Maintenance: Install mortar sealant or re -mortar sections of the chimneys exhibiting holes and cracks.
Modified on Garage: Furnish and install perimeter flashing, base sheet and modified bitumen on flat roof.
TPO on Garage: Remove existing membrane and perimeter flashing and install 1/4" densdeck to achieve Class A fire rating and 60 MIL white

Price is good for. 30 days
Access: Price is based on direct roof access with a dumpster next to the house ifor safety, ease of access) and conveyor truck loading materials.
Payment Terms: We must have a signed copy to schedule work. 50 % payment of the contract amount is due upon material arrival, 90% is due upon substantial completion. 10%
can be withheld until inspection/punch list is complete. Contract becomes binding upon written acceptance or commencement of work. Interest on all overdue amounts shall be
added at the rate of 18% per annum. Owner agrees to pay for costs and fees that may be incurred by contractor to enforce the terms of this contract. Price is based on cash or
check. Credit card purchases are accepted. If Tiley Roofing Inc. and owner of the above mentioned property need to go into mitigation, homeowner agrees to cover costs
associated with mitigation.
Mader Agreement Amendment
Our proposal should be an amendment to any master agreement. The following conditions also apply: Five days will be allotted to dispute any pay application otherwise the pay
application will be considered to be accepted. If 30 days terms are not met late fees may apply as well as stoppage of work with a five day notification.
Additional Information: Please call and ask about possible upgrades. extended warranties, and different types of shingles that are available. Please note bronze counter Flashing
maybe used at roof to wall intersection if notable to install under siding. It is common for granules to be dispersed near downspouts after anew installation and rainfall.
Inclusions: Fasteners, sealants, permit Iremof only), sales tax, workers compensation , liablity insurance and landscape protection Irefoof only). Note for insurance claim projects
we will supplement. Ily bill your insurance provider for the permit. Price is based on direct roof access with a dumpster next to the house and conveyor truck loading.
Exclusions: Roof deck replacement. Roof deck replacement is $60.00 per sheet and done only upon notifying the customer. Siding repair from installation of flashing may be
additional. Driveway damage, correction of structural imperfections, and additional hidden layers of roofing will cost extra upon notifying the customer. Leaks and icicles created
by snow or ice damming. Unwanted animal entry, tree or vegetation damage to the roof covering or acts of God. Satellite Dish adjustment or reinstallation. Electrical mast
disconnection or connection. Drywall, texture or painting. Additional work other than specifically mentioned (HVAC. electrical, woodwork, painting) or any other home repairs or
replacement. Leaks caused be re -using existing skylights are excluded due to the process necessary and disturbance that could cause the failing of existing skylights that may not
have previously leaked. If an estimate for a new skylight has not been provided please request one from your estimator. Please call for any additional clarification. Excludes
engineering or correction of structural imperfections. Owens responsible for all color choices. Snow or Ice damage as a result of weather to shingles or gutters is excluded as well as
Engineered snow retention, unless otherwise indicated. Ventilation design or environmental engineering is excluded, please call regarding consulting with structural engineer or net
free ventilation recommendations.
New Construction Information: Snow removal or work in severe conditions is excluded. Trip charges may apply if project is not ready for our scope of work. Please contact TRI prior
to commencement of work as to necessary preparatory measures needing to be complete). Project is based on accessibility with a conveyor loading truck. Temporary
waterproofing for new construction projects. Permit to be provided by Contractor (except Denver) unless specifically mentioned otherwise.
Safety: Tiley Roofing may choose to install permanent anchors on your roof. These anchors should not be used by anyone other than Tiley Roofing Employees.
Colorado Law:All provision of CRS 18.13.13.119.5 "Abuse Of Property Insurance" SB -38 (senate bill 38) Concerning measures to protect consumers who engage a roofing contractor
to perform roof services on residential property are included, attached in Initial brochure/packet (insurance provider information including workers compensation, automobile and
general liability insurance), incorporated in job information, scope of work or additional information wording and are to be considered part of this contract in compliance with
Colorado Revised Statutes. Additional stipulations are as follows; upon acceptance of contract initial job cost, terms and conditions, property owner has the right to rescind
contractwithin 72 hours, cancel all conditions and receive refund of any deposit (if applicable). Typical time frame forjob completion upon acceptance, material availability, work
load, pending weather (acts of God) delays and work commencement are as follows. Tile roof installations 2-4 weeks, shingle roof installations 1-2 weeks, small repairs 1-3 days,
large repairs 4-10 days. Contractor cannot pay or waive all or part of a property owners insurance deductible, provide any rebating directly or indirectly to compensate, eliminate
or offset owners obligations to pay deductible. Please call or email for clarifications, explanations or any additional information regarding Consumer Protection SB38 Compliance.
Lead: For houses built prior to 1978 there is a high probability lead may have been used to paint the exterior. We are required to use certain techniques to ensure the lead is
correctly removed when the amount disturbed exceeds twenty square feet. Typically the amount of point disturbed is negligible if just the roof is being replaced but almost always
the case if gutters or siding are being removed. Tiley Roofing is a lead certified renovation firm allowing us to perform this procedure. Prior to contracting please let us know if your
house was built prior to 1978. If the house is pre -1978 we will perform a lead test. If the testis positive for lead we will assess a fee in addition to the above costs. Generally this fee
ranges from $500 to $1000.
Insurance Claims: Owner is responsible to verify insurance settlement payment is sufficient to cover contract amount.
Owner is responsible for any difference. Owner is responsible for any Non Recoverable Depreciation. Owner is responsible for deductible payment. Pursuant Colorado Statutes all
insurance claim payments must be in the form of check only,
Owner is responsible for any code compliance or ordinance of law payment difference with Actual Cash Value Policies, please call for mora details. If any additional damage or
costs are identified, for which you believe your insurer is responsible for, they must be approved by a representative of your insurance provider prior to having the additional work
