Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

at City Hall. Revisions to existing permits (for example, to add redecking) must be

person at City Hall. The Building Division will be open from 7:30-10:30 a.m.,
Monday through Friday to process these types of requests.

It has come to our attention you constructed/replaced a caz port / patio cover at the above referenced address
without proper permits or inspections. Pursuant to Section 106 of the 1997 Uniform Building Code, no
building or structure shall be erected, constructed, repaired or improved without first obtaining a building
permit.
There appear to be some serious structural concerns with the structure that has been constructed on this site.
There are numerous code violations evident upon inspection this structure, below is a partial list:

On JWy 17~, 2003, 1, Mr. Patrick Lawrence personally inspected the new carport awning constructed and in place a
the residence of Mr. Janery Pisichio and Mrs. Irene Pisichio located al 4425 Salsbury Place, Wheatridge. Colorado 80033.
The items listed below are compiled from visual inspec[ion only.
L Three decorated load bearing steel column supports located at outer most portion of awning are not plumb.
2. Westerly load bearing steel column support severely out of piumb.
3 Carpentry nails used in some places to locate load Uearing wlumn suppor[s in existing concrete.
4. Poor welding throughout entire structure - including load bearing cantilevered longitudinal beams.
5. The canulevered longitudinal beams are inverted to focus loads towards the center of the beam instead of away from the
center of the beam.
6. Roof span has no laural center load bearing support
7. Scrap wood was used [o join [he awning to the existing roof of the house.
8. Weather proofing grommets on fasteners are split due ro over torque.
9. Structural load bearing components are already rusting.
10. No rust preventative wa[ing(s), primer(s) or undercoating(s) were applied.
11. Rain gutters were conswcted of scrap gutter material.
12. Lateral load bearing support beams are severely warped.
13. Lateral load bearing support beams are de-laminating from the wntoured wooden roofing supports.
14. Contoured wooden roofing supports are split and or broken due to fasteners being impropcrly installed.
15. pne lateral load bearing support beam has been gusseted due to severe warping.
16. RooSng material is of fleacible rubber and dces not appear to be snow worthy.
17. Roofing material is not the color specified by owner.
18. Roofing material not sufliciently overlapped to prevent sunlight from being visible tluough seazcu.
19. Roofing material has two (2) holes above owner's love sea[.
20. Roofing material has numerous holes that are not sealeA where fasteners penetrate.
21. Numerous locations of caulldng tha[ is either too thick or too thin.

23. Roofing panels used as siding aze poorly fit and inadequately fastened
24. Decorative iron work placed belund front rain gutter and hidden from view.
25. Lighting modifications cast inadequate cwerage of doonvay.
26. Lighting modifications incomplete.
27. Non-essential or required modificadons [o existing roof rain gutters where mee[ing north east comer of awning.
28. No design drawings were ever submitted to owner.
29. No snow or wind loading calculauons were submitted [o owner.
30. Previous sUUCmre was completely demolished and removed - no new pemut applied for through ciry.
31. Overall aesthetics and craftsmanship are of extremely poor standards.

Dear Mrs. Pisicchio,
On Friday July 11, 2003 Weldrite Mobile Service completed an awning at your address.
The awning has been tested for leaks and to our satisfaction is finished. In the proposal it
plainly states replace damaged carport and awning, replace damaged gutters and under
board, and replace light fixture. All of this work has been accomplished. On the south
side of your house we added eleven linear foot of gutter and painted it all for free. We
tested the roof for water leaks and fixed what was leaking. On Friday the 1 lth we told
you we were finished. We expected a paycheck for our work. You said that you were
unhappy with the gutter and the downspout, even though before we put up the down
spout we asked you specifically where you wanted it placed, so that is where we put it.
Now you want us to move it on our time and money. We explained to you that if we
raised the gutter that it wouldn't work properly and leak. You told us you didn't care and
wanted us to raise it, again you wanted this done at our expense. You also told Monty
that you wanted'another light put in, the garage door painted and another 12 feet added to
the addition before you would pay us. On the invoice there is no mention of painting the
garage door, putting in two lights or redoing the structure. We have sent pictures to the
insurance company and spoke to Chris Suettee about getting additional payment for the
extra work. He says that the job is done to their satisfaction and is not going to give the
ok on the extra work. Irene we will not be coming over to do any extra work. We aze
done and would like to be paid in full. Please do not involve Dave with this, we have
already spoken to him and explained the whole story. This is not his problem. Irene,
Monty and I were willing to come over and do ihe gutter for free until you started adding
all of these other requirements. You keep making excuses about an inspector, but aze
unwilling to give us his name or number. If you do not pay us in a timely fashion then we
will put a lien on your property and pursue our claim in court. We have finished the job
and would like to get paid the remainder balance. In retum we will fumish you a
notarized mechanics lien release.
