Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of

Payments: The parties agree that RopaRoofing will be entitled to all insurance proceeds related to all claims

the owner on the addendum to this agreement. The owner understands that they are fully responsible for their
insurance deductibles, as stated by the Colorado Law 6-22-105. Waiver of insurance deductible prohibited.
Payments: Owner hereby releases their insurance company to provide RopaRoofing with information related
to this claim and furthermore grants permission for their insurance company to negotiate and settle all claims on
behalf of the owners with third party adjusters, if necessary.
Owner's Rights: Should the owner decide to cancel this agreement, it must be done within 72 hours of the
date signed. This may be done via mail, email, or hand delivered letter — stating that they wish to—terminate this
agreement.
Scope: To be defined by the Owner's insurance summary plus any additional upgrades, which they choose;
and shall be attached along with the addendum to this agreement.
THE ROOFING CONTRACTOR SHALL HOLD IN TRUST ANY PAYMENT FROM THE

SCOPE
A geotechnical and geological engineering investigation was conducted
at 4065 Saulsbury Street, Wheat Ridge, Colorado, to determine
foundation design criteria and interior floor system recommendations
for a proposed addition to a single family residence. Recommendations
for design and construction, based on soil and groundwater conditions,
are provided. This investigation does not include evaluation of soil
or groundwater for the presence of pollutants or other environmental
hazards.
Conclusions and recommendations presented herein are based on results of
a field and laboratory investigation, other investigations conducted by
this firm in the area and experience with similar projects and
conditions.
PROPOSED CONSTRUCTION
Plans call for constructing a living space addition on the west side of
the house. The addition will be one story over crawl space or slab -on -
grade. A wood frame superstructure will be supported by a new
reinforced concrete foundation and the existing foundation.
SITE CONDITIONS
The site is in a fully developed residential subdivision in the city of
Wheat Ridge, Colorado. The surface in the general vicinity slopes to
the north at gentle to moderate grades. The surface within this lot is
relatively flat. Vegetation consists of shrubs, trees, sod and
groundcover plants typical of landscaped residences in the area.

The only apparent geologic hazard that may affect the proposed
construction is shallow groundwater. Recommendations for mitigating
this hazard are presented herein.
SUBSURFACE CONDITIONS
Subsurface conditions were investigated by drilling two foundation
design test holes as close as practical to the anticipated building
envelope. Test hole locations are shown in Figure 1, Test Hole Location
Plan.
The test holes were drilled to a depth of seven and 25 feet and samples
were taken at various intervals based on anticipated foundation bearing
depths and materials encountered. The seven foot hole was drilled to
obtain additional shallow samples for laboratory testing. The litho -
logic log and Standard Penetration test results are shown in Figure 2,
Test Hole Log.
Soils have been deposited by wind and low energy stream flow. Test Hole
1 (TH-1) encountered approximately five feet of fill overlaying a medium
coarse to coarse-grained, loose sand to the total depth investigated.
Fill is the result of a below -ground swimming pool being removed. Test
Hole 2 (TH-2) encountered four feet of very fine to fine-grained silty
sand overlaying a medium stiff clay to the total depth investigated.
Detailed soil descriptions are presented in the test hole logs on Figure
2.
Bedrock was not encountered during drilling operations. Bedrock has
been mapped by the U.S.G.S. as the Denver and Arapahoe Formations. The
Denver and Arapahoe Formations consist of interbedded sandstones,
siltstones, claystones, shales and conglomerates. Bedrock contacts are
gradational. Detailed soil and bedrock descriptions are presented in the

Swell -consolidation testing was conducted on two representative soil
samples from depths of three and six feet. Test results indicate that
the soils at, and below, foundation bearing depth have no swell
potential. Each soil sample tested consolidated 0.4 percent when
saturated under a constant load of 1000 psf. Detailed test results are
presented in Figure 3, Swell -Consolidation Test Results.
Groundwater was encountered at a depth of five feet below existing grade
during drilling operations in TH-1 and TH-2 remained dry during drilling
operations. Free water remained at a depth of five feet below existing
grade in TH-1 and TH-2 remained dry when checked 17 days after drilling
operations. Groundwater appears to be part of localized perched water
tables on top of the bedrock.
Groundwater shallow enough to affect foundation drainage requirements,
foundation construction, and foundation depth is possible. Top of
interior floor elevation should be a minimum of three feet above the
seasonal high groundwater table. Available information indicates the
high water table is at approximately 5 feet in some locations on the
property.
Groundwater conditions can be better evaluated during construction by
observing the foundation excavation.
FOUNDATION DESIGN SPECIFICATIONS
The foundation can be supported on continuous spread footings designed
for a maximum contact pressure of 1500 psf. Maximum contact pressure
shall be calculated using full dead load plus full live load.

Footings shall be constructed on native soil that is smooth, even and
free of loose soil, rocks and debris. Footings shall not be constructed
on frozen or saturated soils. The existing fill placed after the
removal of the below -ground swimming pool appears to be placed in an
uncontrolled manner. Uncontrolled fill may have variable
characteristics and is not suitable for foundation support. All
existing fill must be removed from the excavation and replaced with
moisture -treated and compacted structural fill.
The following procedures and guidelines shall be followed if removal of
uncontrolled fill is needed:
1. Remove all fill within the building footprint until native soils
are encountered. The excavated area shall extend laterally a
minimum of four feet beyond the foundation perimeter.
2. Scarify the bottom of the excavation to a depth of 12 inches,
moisture -treating and compacting soil as specified in Item 4,
below.
3. Add suitable structural fill to bring the excavation to the
required elevation. Fill shall be approved by a representative of
this office prior to use. On-site materials may be suitable if
properly moisture treated.
4. Fill shall be placed in maximum 12 -inch -thick loose lifts,
moistened to within zero to plus 3 percent of optimum moisture
content, as defined by ASTM D-698 (standard Proctor), and compacted
to a minimum 95 percent of maximum density, as defined by ASTM D-
698. Thinner lifts may be necessary depending on equipment used.
5. Foundation support soil over -excavation and replacement should be
observed and tested by a representative of this office to verify
proper placement and compaction.
Foundation walls shall be designed to withstand an at -rest lateral earth
pressure of 50 lb/cu-ft/ft. An equivalent fluid pressure of 250 pcf/ft
may be used for passive lateral soil pressures.

DRAIN SYSTEMS
A foundation drain system is not required for the soils present and the
type of construction proposed. Foundation drain system recommendations
and specifications can be provided if plans change to include habitable
below -grade space.
FOUNDATION BACKFILL
Backfill placed against foundation walls shall be free of trash and
fragments of rock over six inches in maximum diameter, moistened and
adequately compacted to prevent excessive settlement. Walls should be a
minimum of seven days old prior to backfilling. Walls over four feet
high should have the floor system in place or be well braced prior to,
backfilling. Controlled puddling should not be used.
The ground surface shall slope away from the foundation. A minimum
grade of 10 percent (one foot in ten) shall be provided within 10 feet
of the structure, or as far as property boundaries permit. The owner
should periodically inspect the surface around the foundation to locate
and correct grading problems that may occur due to normal foundation
backfill settlement.
INTERIOR FLOOR SLAB CONSTRUCTION
The builder and/or owner should be aware that concrete floor slabs
placed on native soil or bedrock may move if support materials become
wet. One means of preventing slab movement is construction of a
suspended floor over crawl space. Expansion potential of materials at,
and below, interior floor slab bearing depths indicate there is a very
low risk of significant floor slab movement. The following procedures
will help improve slab performance, minimize movement and prevent damage
if some risk of movement can be tolerated:

1. Subgrade should be prepared based on the soil type encountered.
Granular soils should be wetted and thoroughly compacted prior to
slab placement. The surface should be thoroughly wetted 24 hours
prior to slab placement.
2. A minimum four -inch -thick layer of 3/4 -inch washed gravel or
crushed rock should be placed beneath the slab. Rock should be
covered with a minimum six mil impervious vapor barrier. A four -
inch -diameter PVC conduit should be provided through the slab into
the gravel layer near the mechanical system to provide for future
mechanical ventilation, if necessary.
3. Concrete slabs should be reinforced with steel mesh and should be
separated from all bearing members and utilities to allow
independent movement (i.e., a "floating slab").
4. Joints should be scored in the slab at maximum 200 -square -foot
areas.
5. A minimum void space of 1-1/2 inches should be constructed above or
below any non-bearing partition walls. In finished areas, furring
strips, drywall, paneling, etc., should stop a minimum of two
inches above the floor.
6. Piping should not be placed under the floor slab if hot water
heating is used. If forced -air heating is used, a flexible
connection should be utilized to allow some movement between the
furnace and the heating ducts.
7. Flexible connections should be used on all mechanical systems
placed on floor slabs. All plumbing which extends below floor
slabs should have flexible connections and allowance for movement
with the slab.
8. Foundation drain systems and landscaping should be completed in
strict compliance with recommendations outlined herein to help
prevent wetting of slab support soils.
LAWN AND LANDSCAPE IRRIGATION
Lawn and landscaping irrigation should be designed and controlled to
prevent additional wetting of subsoils. Automatic sprinkling systems
should be placed so that, under full pressure, spray does not fall
within five feet of foundation walls. Drip or mist irrigation systems

should not be placed within five feet of foundation walls. Installing
any portion of the irrigation system within five feet of walls should be
avoided.
Decorative landscape, such as rock and/or bark should be used in areas
next to foundation walls. Vegetation planted within five feet of
foundation walls should be hand watered and this watering should be
minimized.
Downspouts and sillcocks should not be allowed to discharge directly on
the ground surface. Splash blocks and/or downspout extensions should be
used to discharge water beyond foundation backfill where backfill soils
are not covered by pavement and/or sidewalks.
The owner is encouraged to read A Guide to Swelling Soils for Colorado
Homebuyers and Homeowners, Special Publication No. 43, available from
the Colorado Geological Survey. Recommendations contained in the report
are applicable to all residential construction in the Denver area,
regardless of soil expansion potential.
INSPECTION AND CONSULTATION
In any subsurface investigation it is necessary to assume conditions
encountered in the field investigation are representative of the site.
Actual conditions may differ from anticipated conditions. Subsurface
conditions should be verified through visual inspection of
the foundation excavation by a representative of this office prior to
construction.
Footing forms and reinforcing steel placement in foundation walls should
also be inspected to insure compliance with these recommendations and
engineered foundation plans.

SCOPE
A geotechnical and geological engineering investigation was conducted
at 4065 Saulsbury Street, Wheat Ridge, Colorado, to determine
foundation design criteria and interior floor system recommendations
for a proposed addition to a single family residence. Recommendations
for design and construction, based on soil and groundwater conditions,
are provided. This investigation does not include evaluation of soil
or groundwater for the presence of pollutants or other environmental
hazards.
Conclusions and recommendations presented herein are based on results of
a field and laboratory investigation, other investigations conducted by
this firm in the area and experience with similar projects and
conditions.
