THE REPAIR IS INTENDED TO STABILIZE THE
MAIN FLOOR OF THE HOME AND PROVIDE
SUPPORT FOR THE FLOOR JOIST SYSTEM.
THE REPLACEMENT OF THE POSTS WITH
SMARTJACKS IN THE CRAWLSPACE IN THE
HOME IS INTENDED TO ALLEVIATE
UNEVENNESS IN THE MAIN FLOOR OF THE
HOME.
IT IS RECOMMENDED THAT THE
HOMEOWNER WAIT 3-6 MONTHS AFTER
BEAM REPLACEMENT AND PUSH PIER
INSTALLATION TO MAKE REPAIRS TO
DRYWALL IN THE HOME. THIS IS INTENDED
TO ALLOW TIME FOR THE FOUNDATION TO
STABILIZE BEFORE REPAIRS ARE MADE.
THIS PROJECT IS NOT A GUARANTEE THAT

RE: Structural Inspection
To Whom It May Concem;
In accordance with your request we reviewed the structural requirements
for the subject address. The property was visited on Monday, 02/15/2021
by Joe O'Neil, P.E. The scope of the project was determined by Peak
Structural and accepted by the homeowner.
DESCRIPTION
All descriptions of the structure are based on observation from Pierson St.
The front of the structure is assumed to face East for the purpose of this
report.
The subject structure is a one-story single-family home, wood -framed
construction with lapped siding. Structure has a hip -style, asphalt -shingle
roof with gutters and downspouts for drainage. Structure was reportedly
built in 1960.
There is a crawlspace with a concrete stem wall and assumed footer. No
foundation or as -built construction drawings were available for review for
this project. No soils testing was available for this report.

ANALYSIS
There is some identifiable settling of the foundation as indicated by an
elevation survey completed by Peak Structural utilizing the Techrddea
Pro 2000 instrument. Unevenness in the main floor of the home can be
felt by walking across the area in direct contact with the beam.
The floor of the structure is supported by 2x8" dimensional lumber floor
joists on 16" centerlines. Floor joists span from the front to rear of the
home. Joists bear on the front and rear stem walls and a triple 2x8"
wooden beam. The beam is currently supported by (3) 7X7' concrete
posts in the crawlspace. The posts bear on concrete piers that are
approximately 30"x30".
There is a supplemental beam located near the crawlspace entrance at the
left rear comer of the crawlspace. The beam is a wooden 4x4" member
bearing on a wooden 4x4" post. Post bears on a rotated CMU block in the
crawlspace.
Structural modifications included in this project is the planned
replacement of the 3 concrete posts with three (3) Supportworks
SmartJacks model SJ288 to support the main beam of the home.
Concrete footers for the posts will be re -used if determined adequate by
the Contractor.
A new steel W8xl5 wide -flange beam will replace the existing beam
near the crawlspace entrance. The beam will bear on 2 new
Supportworks Smartjack model SJ288 posts on 10'-0" centerlines. Posts
will bear on new concrete or crushed stone footers per the
manufacturer's instructions.

This report is valid only for the address and the project scope listed,
by the client listed above. The observations described in this
document are valid for the date of the observation and have been
made under the conditions noted in the report. Use by any other entity
or for any other purpose is probibited, without the express, written
consent of the undersigned engineer. This report is based on published
and anecdotal information; variations from the anticipated soil profiles
may be encountered; contact the engineer immediately if they are
discovered. This report does not cover geological hazards, such as
slope stability or landslide. Such investigations require extensive
investigative techniques and are well outside the scope of this report.
Should such information be desired, contact the engineer for guidance
in obtaining it.
This report does not express or imply any warranty of the future
performance of the structure or drainage condition. This report should
not be considered a warranty of condition, and no warranty is implied.
The opinions and recommendations presented in this report are based
solely on visual observations available for viewing at the time of the
site visit and information, if any, provided by the Client and/or their
representative(s). We are not responsible for conditions that were not
within the scope of our services at the time of inspection. We did not
undertake or perform any destructive or invasive testing. No
additional subsurface testing, material testing, calculations, nor
monitoring over time of the conditions presented in this report are
included in this project, but may be obtained, if desired, under
separate agreement.
Not included in this site visit was a review of the architectural,
mechanical, electrical, plumbing, cosmetic, landscaping, drainage or
structural conditions, other than those items specifically discussed, nor
a comprehensive review of compliance with applicable building
codes. The work covered by this project is not held out as being a
complete repair for all known or suspected structural or foundational
issues within the building; other work may be justified or needed, but

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding A/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€Tms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
