Midroof & Final Roof inspections for ROOFS 6/12 PITCH & OVER: 3rd party inspection will be
required for both the midroof and final inspections. The 3rd party inspection report AND
THE ORIGINAL PERMIT CARD needs to be dropped off to the Permit Desk at the City of Wheat
Ridge. The report MUST BE SIGNED by the Homeowner.
REGARDING ROOF VENTILATION: Roof ventilation shall comply with IBC Sec. 1203.2 or IRC Sec.
R806. The installation of ridge venting requires the installation or existence of soffit
venting. For calculation purposes, one hat or turtle vent equal to one-half of one square
foot of opening.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name) roof
covering" is required to be on site at the time of final inspection.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR

2. Terms: Owner agrees to provide ASAP Roofing full access to the property as necessary for staging and completing the work. Copies of the
Loss Sheet and any other documents applicable to the Replacement Work shall be furnished as requested. If the terms of this agreement
are violated it is agreed that 33% of the approximate cast listed above will be charged for punitive and actual damages suffered. If
for any
reason the person authorizing this agreement wishes to terminate it is their option to do so and any amounts deposited will be fully
refunded, provided written notice is sent to ASAP Roofing within 72 hours after entering the contract. Additionally, in the event Owner
receives written notice from Owner's insurance company that the claim for payment for the work under this contract is denied in whole
or in part, Owner may rescind this contract, in accordance with and subject to the limitations set forth in Colorado Revised Statutes 6-22-
104, by giving ASAP written notice within 72 hours of receiving notice that the claim for payment is denied. If Owner will be using
:nsurance proceeds to pay for the roofing work, ASAP cannot pay, waive, rebate, or promise to pay, waive or rebate all or part of any
insurance deductible applicable to the insurance claim for payment of the roofing work on Owner's residential Property. ASAP SHALL
HOLD IN TRUST ANY PAYMENT FROM OWNER UNTIL ASAP HAS DELIVERED THE MATERIALS AT THE OWNER'S PROPERTY OR HAS
PERFORMED A MAJORITY OF THE WORK ON THE PROPERTY. Surety and Liability Coverage Insurer: ASAP Roofing holds Liability insurance

3. Acceptance: Owner and ASAP Roofing agree that upon approval by the insurance company ASAP Roofing will perform the roof
replacement and ASAP Restoration will complete other agreed upon services. Customer agrees to commence the restoration work as
soon as the funds are made available by the insurance company. All trade work is to be paid as funds are made available for each
completed trade- Payment is due 10 days after received by customer and work is completed. If the file is placed in default interest will be
charged at the maximum amount allowed by law. Any modifications to this agreement must be made in writing and agreed to by the

This installation specification and wide has been produced to
aid the specifier, ownerfinstaller, and roofer in the installation
of BARTILE products. Every effort has been made herein to
adhere to general code requirements. However, it must be
explicitly understood by the user that ii is his responsibility
to understand and comply with the "then current" building
codes, local jurisdiction requirements and climatic conditions
applicable to this project.
Within these pages are various alternative methods for the
installation of BARTILE. The installer must choose which
method is practical for each installation as he must be responsible
for the performance of the system as a whole. Local practices vary
and it should be understood that this booklet should not be
interpreted to be the only "proper' way to install a BARTILE roof,
but in fact contains various proven installation techniques. BARTILE
does not certify nor warrant any particular installation method or
process. The BARTILE product warranty is limited to the furnishing
of replacement BARTILE shingles for those which in the course of
normal use, decompose or wear out during a period of 75 years.
Breakage, color variance, and acts of God and normal weathering
are excluded. See the text of the printed BARTILE limited 75
year warranty for details.
This Installation Aid is intended to Inform the Installer of Various options, and Factory Recommendations.
It is NOT INTENDED to Replace Local Codes and Practices. but Rather to Provide General Information.
Consult Your Local Building Inspector for Specific Information Relative to a Local Code Question.

The BARTILE COLD ROOF SYSTEM Incorporates Both Vertical and Horizontal
Strips, an Eave Cold Air Intake, and a Ridge (Warm Air) Exhaust.
The Underlayment Area Stays Cooler in the Summer, and Reduces
Ice Buildup at the Eave Area in the Winter by Disipating Attic Heat
Below the Tile, and Exhausting it Through The Ridge Vent. The Tile
Surface Therefore Remains Cooler, Which Reduces Snow Melt and
Eave Ice Buildup Below the Heated Zone. The Vertical Strip System
also Prevents Potential Underlayment Moisture From Collecting Behind

($500.00 ) is being held until all landscaping improvements are completed at,

single family residential structures The street tree will be required to be placed in the
front ard setback at the home located at 3331 Routt St.
In the event that landscaping required by Wheat Ridge Code of Laws, Section 26-502
and/or as shown on an approved landscape plan is not installed by April 1,
2008 ,(date) the City of Wheat Ridge or its designee shall have the right to enter
upon said property to construct and/or install such improvements or do other such work
to accomplish that purpose as may be necessary, provided that the City of Wheat Ridge
shall firsY give notice in writing to the contractor, developer or owner of its intent to do
so.
If the City of Wheat Ridge exercises its right to perform work of any nature, including
labor, materials and use of equipment either by force account or contract, the City shall
have the absolute right to withdraw the escrow funds to complete said landscape
improvements. The only precondition to the City's right to witbdraw the escrow funds
far this purpose shall be a letter from the Planning and Development Director, stating that
the landscape improvements have not been completed or provided and that the
compliance date of AnCil 1 2008 has expired.

the same, subject to general real estate taxes for the curzent year, easements of record or in existence, end recorded declarations,
roshictions, rcservations end covenants, if any, as of this date and oxcept for a First Dad of Truat for the benetit ot First American
State Bank dated Au¢ust 3. 2006 and recorded stmultaneouslv herewith.
4. Payment of Principal and Interest. Borrower shall promptly pay when due the principal of and interest on the
indebtedness evidenced by the Note, and late charges as provided in the Note and shall perforra all of Borrower's otHer covnnants
contained in the Nota

S. Application ot Paymeats. All payinents received by Lender under the terms hereof shall be applied by Lender iust in
payrnent of amounts due pursuant to paragraph 23 (Escrow Funds for Taxes and Insurance), then to amounu disbursed by Lender
pursuant to paragraph 9(Protectioo of Lender's Securiry), end the balance in accordance with the terms and conditions of the Note.
6. Prior Mortgapes and Deeds of Trust; CharQes; Liens. Borrower shalt perform all of Borrower's obligattons under any
prior deed of trust and any other prior liens. Bortower shall pay all taxes, assessmenu and other charges, fines and impositions
attributable to the Property which may hare or attain a prioriry over this Deed of Trust, and leasehold payments or ground rents, if any,
in the manner set out in paragraph 23 (Escrow Funds for Taxes and Insurance) or, if not requirod to be paid in such manner, by
Borrower making payment when due, directly ro the payee thereof. Despite the foregoing, Borrower shall noi be required to make
payments otherwise roquirad by this paragraph if Borrower, after notia to Lendor, ahail in good faith contest such obligation by, nr
defend enforcement of such obligation in, legal proceedings which operate to prevent the enforcement of the ohligation or forfeiture of
the Property or any part thereof, only upon Borrower making all such contested payments and ather payments as ordered by the court
to the registry of the court in which such proceedings are filed.
7. Property Insurxnce. Borrower shell keep the improvemenu now existing or hereaRer erected on the Propcrty insured
against loss by fire or hazards included with the term "extended coveragd" in an emount at least equal to the lesser of (1) the insurable

All permits shall be applied for through the Public Works Department. An approved permit is effecrive for a
period of one (1) year from date of issue and may be renewed. All fees shall be in accordance with those fees
established by the Uniform Building Code or other applicable City adopted Resolutions or Ordinances.
Note: All public improvements, when constructed, sha11 be maintained (by the respective individual(s) and/or
company responsible for the consduction of that public anrUor private improvement in the respective
development) on a daily basis, or as needed, such that they are free oj mud and other construction debris
tracking from the site. Failure to comply with this requirement will result in the enforcement of Article II,
Specified Nuisances, Section I5-16 (4) of the City of Wheat Ridge Code ofLaws.
MINOR GRADING, EXCAVATION, AND FILL PERNIITS
I. 1-50 Cubic Yards: No permit is required for dumping or excavation of earth materials, which do not exceed
50 cubic yazds, provided, however, that any fill deposited is on nahual terrain of less than three (3) horizontal
to one (1) vertical slope, or such fill is less than three (3) feet in depth and is not intended to support
permanent structures, and in addition, such fill or excavation does not obshuct or otherwise adversely affect
any drainageway. Should any of the above standards be exceeded, or a drainage way be affected, a permit
shall be required under the guidelines for No. 2.
2. 51-500 Cubic Yards: Dumping or excavation of earth materials not exceeding 500 cubic yazds inay be
allowed with a permit approved by the City Engineer. All applications are to be submitted on the appropriate
completed application form and shall be accompanied by the appropriate fee and sketch plan indicating the
following information before the permit will be issued.
A. Location and dimensions of all property boundaries and structures on the site.
B. Location and eactent of azeas to be Filled and/or excavated.
C. Loca6on of existing and proposed drainageways, 'urigation ditches, etc., and indication of how and where
historic run-off will be maintained on and through the site.
D. Cross-section area to be filled and/or excavated indicating original slope, new slope and depth of fill.
E. Statement that indicates the proposed use or purpose for said fill or excavation.
F. Relative elevation of adjacent proper[ies.
G. Erosion control plan showing placement of control devices such as hay bales, etc.

prescribed by Wheat Ridge Code of Laws, Section 26-1004. Any fill requested under this permit within the 100
yeaz flood zone MUST COMPLY WITH WI-IEAT RIDGE CODE OF LAWS, SECTION 26-801, FLOOD
PLAIN ZONING ORDINANCE.
