2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must be made in writing and

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.
Midroof & Final Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be
required, the 3rd party inspection report AND THE ORIGINAL PERMIT CARD needs to be dropped
off to the Permit Desk at the City of Wheat Ridge. The report MUST BE SIGNED by the

"Payment agreement remains intact even in, the case city is delayed in performing inspection. Integrous Roofing & Restoration Inc commits to
passing of city inspection.
All proposals are subject to approval of management. Client agrees to pay for "unforeseen expenses (Examples: rotted decking at $50
per sheet, rotted or broken space decking at a $100 minimum fee covering up to 24 linear feet plus $4 per additional linear foot, extra layers of
shingles at approximately $32 per square, or rusted roof jacks and gas vent pipes - price TBD) that cannot be detected upon initial inspection of
roof. Adequate photos and/or documentation will be provided for client approval. Any variation from agreement beyond the control of Integrous
Roofing & Restoration Inc. (Integrous) and/or requests by client for services not included in original contract resulting in increased project cost shall
be paid in full by client upon completion of project. Please note that this is a demolition process. There will be much movement and activity
on your roof. integrous cannot be held responsible for any interior damage caused by faulty craftsmanship or improperly secured decor and/or
other fixtures. Owner agrees that equity in this property is security in this contract. This contract shall become binding only upon written
acceptance hereof by the contractor or by an authorized agent of the contractor or upon commencement of work. This contract constitutes entire
understanding of both parties, and no other understandings, collateral or otherwise, shall be binding unless in writing and signed by both parties.
Integrous 15 -Year Craftsmanship Warranty (applicable to 30 -year and greater products): 1) is transferable for life of warranty, 2) is limited to
roof leaks, 3) Does not cover normal maintenance caused leaks (eg: caulking) 4) covers roof only, and 5) shall be revoked if payment is not
received in full. Should water damage occur to home interior, Integrous reserves the right to asses the situation and make provision as seen fit by
Integrous. Integrous is not responsible for mold or mildew. Labor warranty does not cover damage to roofs caused by lightening, gale (50 MPH+),
hurricane, tornado, hailstorm, impact of foreign objects, or other violent storm or casualty, or damage to roof due to settlement, distortion, failure
or cracking of the roof deck, walls or foundations. Material warranty is provisioned solely by manufacturer. All material guaranteed to be
provided as specified and all work shall completed in a professional manner. Integrous is not liable for damage to concrete or driveways. The
property owner is to carry fire, tornado, and other necessary insurance. For insurance projects, Integrous reserves the right to file for supplemental
insurance claims at no additional cost to the client. All payments from insurance company due within 15 business days of receipt of payment. Final
payment is due upon completion of non -insurance related projects. If full amount due is not paid within 30 days of completion, a finance charge of
1.75% per month will be charged on account balances not paid in full. Client agrees to pay all expenses of collection, including reasonable attorney
fees, and $50.00 per returned check.
I/We the Client (1) agree to pay upon substantial completion the sum of the total amount of insurance allowance and (2) am/are liable for my/our
deductible and all upgrades. Integrous Roofing & Restoration Inc. (1) will collect the first insurance check (deductible & any upgrades) upon
completion of the roof and (2) will collect the remaining check(s) upon your receipt of them. Insurance claim contracts are contingent upon
Insurance company totaling the roof. Customer may rescind the contract within seventy-two hours (72) after signing contract.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
mid -roofs may be called in at the same time, one hundred percent of the sheathing must be
complete and 25-75 percent of the mid -roof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield
is required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturer installation instructions,
whichever is more stringent. In order to pass a final inspection of elastomeric or similar
type roof coverings, a letter of inspection and approval from the manufacturer technical
representative stating that "the application of the roof at (project address) has been
applied in accordance with the installation instruction for (roof material brand name)
roof covering" is required to be on site at the time of final inspection.

passing of city inspection.
All proposals are subject to approval of management. Client agrees to pay for "unforeseen expenses (Examples: rotted decking at $50
per sheet, rotted or broken space decking at a $100 minimum fee covering up to 24 linear feet plus $4 per additional linear foot, extra layers of
shingles at approximately $32 per square, or rusted roof jacks and gas vent pipes - price TBD) that cannot be detected upon initial inspection of
roof. Adequate photos and/or documentation will be provided for client approval. Any variation from agreement beyond the control of Integrous
Roofing & Restoration Inc. (Integrous) and/or requests by client for services not included in original contract resulting in increased project cost shall
be paid in full by client upon completion of project. Please note that this is a demolition process. There will be much movement and activity
on your roof. integrous cannot be held responsible for any interior damage caused by faulty craftsmanship or improperly secured decor and/or
other fixtures. Owner agrees that equity in this property is security in this contract. This contract shall become binding only upon written
acceptance hereof by the contractor or by an authorized agent of the contractor or upon commencement of work. This contract constitutes entire
understanding of both parties, and no other understandings, collateral or otherwise, shall be binding unless in writing and signed by both parties.
Integrous 15 -Year Craftsmanship Warranty (applicable to 30 -year and greater products): 1) is transferable for life of warranty, 2) is limited to
roof leaks, 3) Does not cover normal maintenance caused leaks (eg: caulking) 4) covers roof only, and 5) shall be revoked if payment is not
received in full. Should water damage occur to home interior, Integrous reserves the right to asses the situation and make provision as seen fit by
Integrous. Integrous is not responsible for mold or mildew. Labor warranty does not cover damage to roofs caused by lightening, gale (50 MPH+),
hurricane, tornado, hailstorm, impact of foreign objects, or other violent storm or casualty, or damage to roof due to settlement, distortion, failure
or cracking of the roof deck, walls or foundations. Material warranty is provisioned solely by manufacturer. All material guaranteed to be
provided as specified and all work shall completed in a professional manner. Integrous is not liable for damage to concrete or driveways. The
property owner is to carry fire, tornado, and other necessary insurance. For insurance projects, Integrous reserves the right to file for supplemental
insurance claims at no additional cost to the client. All payments from insurance company due within 15 business days of receipt of payment. Final
payment is due upon completion of non -insurance related projects. If full amount due is not paid within 30 days of completion, a finance charge of
1.75% per month will be charged on account balances not paid in full. Client agrees to pay all expenses of collection, including reasonable attorney
fees, and $50.00 per returned check.
