In order to pass a final inspection on commercial elastomeric or similar type roofing, a
letter of inspection and approval from the manufacturer technical representative stating
that "the application of the roof at (project address) has been applied in accordance with
the installation instruction for (roof material brand name) roof covering" is required to
be on site at the time of final inspection.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
Mid -roof inspections may be called in at the same time, 100 percent of the sheathing must
be complete and 25-75 percent of the midroof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. A ladder extending 3
feet above the roof eave and secured in place is required to be provided for all roof
inspections. Roof ventilation is required to comply with applicable codes and/or
manufacturer installation instructions, whichever is more stringent.

Dear Mr. Mosher,
At the request of the your tenant, Christine Forbes, I performed an inspection at 3745 Quail Street,
Unit #5 on Apol 12, 2000. The tenants roommate was present at the time. The request was based
on problems from an above unit having plumbing leaks that caused the ceiling in your lower unit
to be in a state of disrepair. The bathroom floor in the lower unit was also warped from possible
water damage.
As of our last conversation, the plumbing was going to be fixed and the roof was to be repaired. At
the time of my inspection, I did not visually detect any leaks, but did notice that the ceiling was still
open and exposed the floor joists and plumbing of the upstairs unit. Therefore, I am requesting that
verification that the leak repair has been accomplished and then that the roof repair has also been
completed. The floor damage needs to be repaired but does not present a health issue at this time.
The repairs must be completed within seven(7) days from receipt of this notice.

Dear Mr. DiCino:
On November 29, 1990, i was requested to inspect the electrical
system at 3745 Quaii Street, #2-
Upon this inspection it was discovered that the unit's electrical
panel had been iliegally modified so that 15 Amp wire was fed
with 20 Amp breakers. This being in direct violation to the
National Electrical Code. I instructed the owner to correct this
violation as soon as possible.
it should also be noted that this apartment is wired with
aluminum wire and all devices should be changed out to an
approved style.

Dear Mr. Gaspar,
I am today in receipt of your letter dated April 18, conceming the inspection you made to my
rental condo at 3745 Quail St. # 5, Wheat Ridge.
I assure you Mr. Gaspar, that the ceiling would have been fuced long ago, except that the tenant,
Ms. Christine Forbes, kept insisting that there was still a leak from above. On several occasions,
I instructed her to inform me as soon as she noticed the leak, but she never did. She just kept
making claims every month that there was a leak, and I kept telling her that we can not find a
leak, and that when she sees it leaking, to come and get me RIGHT THEN. Well, as you can
imagine, this never happened.
It appears that the leak has in fact been fixed for some months, on the 3rd of 0 attempt, I can't
recall exactly which one. But we were afraid to seal the hole up because of Ms. Forbes
insistence that the leak was not fixed.
It appears Ms. Forbes allegations have been nothing more than a goose chase for myself, the
owner of the unit above, and the conVactors and repairmen we have had out trying to find the
phantom leak. The last two contractors that have been out to check the leak (one before your
original phone call, and one since your call) have NOT been able to re-create the problem, nor
have myself or the owner of the unit above been able to cause the leak.
We have run the shower, the dishwasher, the kitchen sink in one side, then the other, then the
bathroom sink, then the toilet, then the bath, etc, etc in the unit above. We have tried running the
hot water, the cold water, a combination of hot and cold, etc. We have pooled water here and
there on the floor in the unit above, trying to re-create the leak that Ms. Forbes insists is there,
and we can not find it.
I have scheduled the repairs to be started on April 1g, 2000. Ms. Forbes will be vacated from the
premises by that time as she is out at the end of this month. The warping of the floor is a VERY
new occurrence, that has happened as a result of Ms. Forbes backing up hertoilet in March of
this month. There was no problem before that, but I witnessed the floor being pooled up in
sewage water for a period of several hours that moming. That combined with the fikhy state of
the house when I arcived that moming is what drove me to end their residency in the first place.
I appreciate your phone calls in this matter and can assure you that in my 11 years 1 have had
rental properties, IYe never had quite the tenants as this family has been.
