1. This permit was issued based on the information provided in the permit application and accompanying plans and
specifications and is subject to the compliance with those documents, and all applicable statutes, ordinances,
regulations, policies and procedures.
2. This permit shall expire 365 days after the date of issuance regardless of activity. Requests for extension must
be made in writing and received prior to the date of expiration. An extension of no more than 180 days made be granted
at the discretion of the Chief Building Official and may be subject to a fee equal to one- half of the original permit fee.
3. If this permit expires, a new permit may be required to be obtained. Issuance of a new permit shall be subject to
the standard requirements, fees and procedures for approval of any new permit. Re- issuance or extension of expired
permits is at the sole discretion of the Chief Building Official and is not guaranteed.
4. No work of any manner shall be performed that shall results in a change of the natural flow of water without prior
and specific approval.

State of Colorado and to the Zoning Regulations and Building Codes of wheat Ridge, Colorado or any other applicable
ordinances of the City.

Dear Ms. Braithwaite,
The Department of Public Works has reviewed the building permit
application for the proposed addition to the existing US West
commercial property at the above referenced address, and has the
following comments:
1. In accordance with Section 5-45 of the Wheat Ridge City Code,
a Development Covenant will need to be processed. Briefly,
this document stipulates that the respective property owner
will be responsible for the cost to construct those public
improvements along the property's frontage for a period of
ten (10) years should the City choose to improve Tabor Street
within this period of time. This document is required
in lieu of constructing the public improvements along the
property's frontage.
The building permit can be issued at this time. However, please
be aware that the Public Works Department can not approve the
Temporary Certificate of Occupancy or Certificate of Occupancy
until the above referenced document has been processed.

We have been asked to review and comment upon the installation of
a drilled oun for a monopole tower that has been
erected a 980 Tabor Street,in Wheatridge, Colorado for the
purpose of supporting a group of new cell antennas. This work has
been undertaken by Lucent Technologies and Bechtel and was
completed approximately two weeks ago.
While we were not present on the site during the construction we
did observe the site prior to construction and have also been
furnished with stamped copies of the design calculations for the
foundation by Engineered Endeavors Znc., photos of the construction
and copies of the concrete test report.
Based on our review of the materials provided to us it appears the
antenna foundation has been constructed in a good, workmanlike
fashion in compliance with the stated criteria and there are no
irregularities in its installation. The pier reinforcing appears
to have been placed in substantial conformance with the
construction plan. The concrete was placed using a tremie,
vibrated and according to the testing agency has achieved over 70$
of required strength in seven days.
Copies of the photographs taken during the construction, the
foundation drawings and calculations and concrete tests reports are
enclosed with this letter for your information.

The Department of Public Works has reviewed the building permit
application for the proposed addition to the existing US West
commercial property at the above referenced address, and has the
following comments:
1. In accordance with Section 5-45 of the Wheat Ridge City Code,
a Development Covenant will need to be processed. Briefly,
this document stipulates that the respective property owner
will be responsible for the cost to construct those public
improvements along the property's frontage for a period of
ten (10) years should the City choose to improve Tabor Street
within this period of time. This document is required
in lieu of constructing the public improvements along the
property's frontage.
The building permit can be issued at this time. However, please
be aware that the Public Works Department can not approve the
Temporary Certificate of Occupancy or Certificate of Occupancy
until the above referenced document has been processed.
