The Electrician we originally had on the job is not working at the job anymore. So please let me know what needs to be done to get that changed and whatever we will need to do so he can start working at this location. We are the general contractor for the active job at 5883 W 38th Ave Wheat Ridge CO. 80212. You will see below the email from TIB Inc. General Contractor, in regards to the changes in the electrician at the job. We were told to send an email to y'all and let you know of the changes, but I was given an invalid email email address to send this information to yesterday when we had called and got the email address to send this information over. Then when we called back throughout the day we got no answer from anyone. So if you can please see the attached emails for the changes that are needing to be made at that job location on W 38th Ave. Also we have provided the new Electrician Sub Contractor who will be on the job with the Authorization Form he will fill out and send it to contractors@ci.wheatridge.co.us as directed below once this change request gets sent. Please let us know if you have any questions, issues, or if you need anything else to get this change made so we can have the new electrician send over the authorization form and get an inspection scheduled asap! (D&D) is pleased to provide the results of the asbestos survey for renovations at the vacant commercial duplex structure located at 5883/5885 W. 38th Avenue, Street, Wheat Ridge, Colorado. Both Units are under the same roof but there is no interior access between the Units. Access is from the exterior only and both Units face east and Unit 5883 is the east side and 5885 is the west side Unit. The walk-through, collection of representative bulk samples, and delivery of the bulk samples to an independent analytical laboratory were completed on December 18, 2019 by D&D Building Inspector, certified and accredited by the Environmental Protection Agency (EPA) and the State of Colorado. UNDERSTANDING AND SCOPE OF WORK D&D understands a representative of T.I.B., Inc., was seeking a professional environmental consulting company to conduct a walk-through, collect representative bulk samples of suspect materials and submit the samples to an independent laboratory for analysis based on standard turnaround time. D&D further understands the asbestos survey, of the commercial duplex structure, was completed to facilitate interior renovations consisting of demolition of the existing center wall between the Units and the half -walls located in Unit 5885. The commercial building was constructed of brick and cinder block and the interior consists of drywall components, joint compound, cinder block mortar and cove -base mastic and each Unit is estimated at 1200 -square feet. ANALYTICAL PROCEDURES The bulk samples collected of suspect asbestos containing materials were delivered to EMC Labs, a National Voluntary Laboratory Accreditation Program (NVLAP) asbestos laboratory. All bulk samples are archived for six - months unless otherwise stipulated by the client. According to the analytical laboratory, the bulk samples were analyzed in accordance with EPA Method 600/R-93/116. Small portions of the samples were placed in Series: E High Dispersion Refractive Index Liquid on a microscope slide. The prepared samples were observed at 100X (power) under polarized light using a McCrone Dispersion Staining Objective. The characteristics of the fibers were compared to the known properties of asbestos fibers for dispersion color, polarity, extinction and general morphology. Sample content (given by percentage) was made by visual estimates by comparison of asbestos fibers to total materials. Homogeneous sampling areas were identified and are listed below. Homogeneous sampling areas are divided into categories of asbestos containing materials, as follows: thermal system insulation (TSI) including duct & pipe insulation; surfacing materials, (sprayed/troweled-on materials and textures); and miscellaneous materials including floor tiles, floor mastics and transite panels. Also, the regulations, for example, prohibit the use of tracked vehicles on materials left in and on the structures after demolition. However, the State of Colorado no longer allows ACM floor tiles to remain under any demolition processes. The legal description of the property, as noted in records of the Jefferson County Assessor's Office, is as follows: East 69 ft of Lot 15, Hahn Subdivision The Property is currently occupied by three single level structures. A commercial building is situated in the southern portion of the Property and an adjoining residence and detached garage are located in the northern portion. The current tenants of the property are listed below: 5883 W. 38th Ave, JJ's Upholsters 5885 W. 38th Ave, Economy Office Products The records of the Assessor's Off ice indicate that the Property covers a total area of approximately 12,420 square feet, The Assessor's records further indicate that the southern, commercial structure contains 2.500 square feet of floor space and was built in 1968. The residence, initially constructed in 1948 with an addition built onto the south side in 1950, contains a total of 994 square feet of floor space. The garage, the construction date of which is not specified in the Assessor's records, covers an area of 400 square feet. The Subject Property is located in a strip of commercial activity which has developed along West 38th Avenue in an area otherwise primarily developed for residential use. Activities in the commercial corridor along West 38th Avenue have included a variety of retail and service operations, auto repair facilities and gas stations. Lake Rhoda and Lakeside Amusement Park is located one-half mile north and northeast of the Property. Interstate 70 is located north of the amusement park, approximately one mile north of the Subject Property. Topographically, the Property is situated in an upland area (elevation approximately 5,415 feet) which slopes generally to the north. The 1990 Polk Directory indicates that the Property was, used by Don's Medical Supply i5883 West 38th Ave.) with no listing for the 5885 West 38th Avenue address, The review of the city directories also confirmed that a variety of commercial operations and res:idences have been present in the area along West 38th Avenue in the same period, These operations have included laundry and dry cleaners, auto repair/service operations and auto service stations, The nearby businesses which could conceivably be of environmental concern are listed in Table 3 below. These maps were originally published in approximately 1890 and 1929. They were periodically updated by pasting overlays over the original maps. With the Sanborn system it can be difficult to establish when a structure was constructed or removed, but these maps can be useful in determining land use history in an area. A review of the Sanborn Maps indicated that coverage did not extend into the vicinity of the Subject Property. IN 3.0 ENVIRONMENTAL ISSUES Federal, state and local government offices were contacted to evaluate whether the site has been the subject of an environmental investigation, report or complaint. mam�� The Colorado Department of Public Health and Environment (CD Ply E) was contacted regarding the subject property, and the following agency databases were reviewed: The Subject Property was not identified on the CDPHE current CERCLIS list and there have been no recorded incidents involving hazardous substances on the Subject Property. No CERCUS-listed sites exist within a half-mile search radius of the Subject Property. The Property was not listed on the Department's current SARA (Superfund Amendments and Reauthorization Act) Title Ill, Section 313 tabulation of sites reporting releases of potential environmentally damaging substances. No underground storage tanks (USTs) were identified on the Subject Property in CDPHE's listing of USTs and no sites were listed in department files within a three block radius of the Property. Additional information on nearby UST sites was gathered from the State Oil Inspector's Office (Section 3.31 and the Wheat Ridge Fire Department (Section 3, 5). No leaking underground storage tanks were identified on the Subject Property. One (1) leaking LIST M There was no record of spills having been reported at the Subject Property on the CDPHE database concerning the ERNS (Emergency Response Notification System), There were no additional listings for the Subject Property or nearby locations, within the ASTM record search distances, on the remainder of the CDPHE databases. 3,3 Oil inspection Section A review of the files of the State Oil Inspection Section, the agency responsible for maintaining UST registration records, revealed that no USTs are registered at the Subject Property. The files did list U as being present at two locations within one-quarter mile of the Subject Property, Those locations are the same as encountered in CDPHE records (Section 3, 2), 5630 and 6000 West 38th Avenue, According to a note from the Oil Inspector's Office, the USTs at 6000 West 38th were to be removed on April 14, 1995. No additional information on these sites and the other former gas stations at 5798, 5990, 5995 or 6005 West 38th Avenue was available from the Oil Inspector's office. No reports on problems associated with the use, disposal or release of hazardous materials on the Subject Property were identified in the files of the Department of Health & Hospitals. Further, no such problems were identified by the Department within a one-half mile radius of the Property. The commercial units have been occupied by retail operations since 1968.
