I visited the above reference project on April 6, 2015 to observe the progress of construction
including the four exceptions that were noted in the Terracon Report dated 03/27115,
I previously observed the foundation forming and rebar placement, the wood floor reinforci
framing and the new masonry wall openings On this visit I observed the Terracon exceptio
and the new entry canopy and the roof screen framing. Based on my observations it appea
that the structural portions of the construction have been completed in substantial conforman
to the approved plans and specifications,
I The gap between the bottom flange and the plate that ties the two beams together at the
cantilever is acceptable.
Z The exposed bolt shank at the column cap to the concrete beam is acceptable.
1 The wood screws that are not fully engaged are engaged enough to develop the needed
capacity of the connection and are acceptable.
4Bolting of the mechanical screen wall connections in lieu of screw attachment is
acceptable.

performed sampling of suspect asbestos-containing building materials (ACBM) at 6920 W 38th
Avenue, Wheat Ridge, CO (the Subject Property).

The Subject Property is currently improved with a single story vacant structure approximately 5,000
square feet in size,

An ACBM survey of the Subject Property was performed on March 28, 2014 by Mr. Ryan Passarelli,
Colorado accredited asbestos inspector (Cert # 13393). Accessible areas of the Subject Property
were inspected. The survey was conducted in accordance with Federal, State, and local, asbestos-
industry standards for building renovation or demolition.
The survey involved locating and assessing the condition • accessible suspect ACBM using sampling
and visual inspection techniques, to develop a report which identifies the extent of the materials
present within the Subject Property.
ACBM's are generally classified as friable or non-friable, Friable materials are those which can be
crumbled, pulverized • reduced to powder by hand pressure. Representative samples of the suspect
ACBM's were collected at the time • the inspection and were submitted • Environmental Hazards
Services, L.L.C, a laboratory state-licensed and accredited for analysis of asbestos and lead content
located in Richmond, Virginia.

Asbestos was not detected in the samples collected by EBl, A full sample table can be found in
Attachment B, Laboratory results and Chain of Custody can be found in Attachment C.
Laboratory analytical results indicate the samples collected at the Subject Property do not contain
asbestos. Additional suspect ACBMs may be present at the Subject Property in areas that were not
accessed during the survey, If additional suspect ACBM's are identified at the Subject Property, which
are not identified in this report, they must be treated as ACBM unless future sampling determines
otherwise.

TERMS AND CONDITIONS
As applicant for a building permit in the City of Wheat Ridge, i do hereby agree to the terms and
conditions set forth below for the issuance of a Certificate of Occupancy in connection with this
building permit. The issuance of a Certificate of Occupancy requires:
1) The completion of ail required landscaping and related site improvements. Inspections
are pertormed by the Pianning and Zoning Division.
2) The complefion of site drainage/grading requirements perthe approved site grading pian

- and/orfinal drainage report and plan, if such report and plan is appiioable. For all residential
and nonresidential projects with an approved final drainage report and pian, a letter of
certification is required from the civil engineer of record. (nspection are performed by the
Pubiic Works Department.
3) The completion and acceptance by the City of Wheat. Ridge of all required public
improvements, or a Development Covenant or.cash escrow may be required in lieu of
construction, as determined bythe Public Works Department. The Public VVorks Department
performs public improvements inspections.
4) The completion and approval of a Certificate of Occupancy Application form and final .
inspections. inspecfiorrs are performed by the Building Division.
All the above items must be completed and approved by Planning and Zoning, Building and
Public Works before a Certificate of Occupancy is issued. Final Inspection by the Building
Division does not constitute authorization of a Certificate of Occupancy nor permission for
occupancy. Occupancy of the sfructure is nofpermitted until the Certificate of Occupancy
is issued. (Uniform Building Code, Secfion 109) ' .
