In order to pass a final inspection on commercial elastomeric or similar type roofing, a
letter of inspection and approval from the manufacturer technical representative stating
that "the application of the roof at (project address) has been applied in accordance with
the installation instruction for (roof material brand name) roof covering" is required to
be on site at the time of final inspection.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof
when spaced or board sheathing with ANY gap exceeding one half inch exists. Sheathing and
Mid -roof inspections may be called in at the same time, 100 percent of the sheathing must
be complete and 25-75 percent of the midroof may be complete. Asphalt shingles are required
to be fastened to the roof deck with a minimum of 6 nails per shingle. A ladder extending 3
feet above the roof eave and secured in place is required to be provided for all roof
inspections. Roof ventilation is required to comply with applicable codes and/or
manufacturer installation instructions, whichever is more stringent.

MATERIALS OR HAS PERFORMED A MAJORITY OF THE ROOFING WORK ON THE RESIDENTIAL PROPERTY

additional work as detailed in this agreement This contract is contingent upon LCI and my insurance carnets agreement as to the full scope of work and
payment amount to have said work completed Upon agreement between LCI and the insurance carver. LCI may .mmediately begin the work detailed above
Owner grants full access to property for staging and execution of work unless otherwise agreed. The specified work and quoted price above is subject to
change upon discovery of any hidden defect Any alteration or deviation from specifications outlined in this agreement involving additional cost will be
executed upon written change orders from the Owner

correspondence from the insurance carrier and/or mortgage holder related to the work agreed upon
Delays - Owner hereby acknowledges that LCI may be subject to delays occasioned by inclement weather, labor disputes, mortgage endorsement or funding
disputes, inspections and material supply shortages all of which are beyond the control of LCI The Owner hereby accepts any delays occasioned by one or all
of these circumstances
Workmanship - LCI provides a 5 -year labor warranty Such warranty is expressly limited to labor, and does not extend to materials, Warranty does not include
damaged caused by lightning. gale (50 mph or greater) force winds. hurricane tornado, hailstorm impact or foreign objects or other cracking due to settlement
distortion, failure or cracking of walls or foundation of building LCI shall supervise and direct work. using its best skill and attention. All work to be completed in
a workman like manner according to standard industry practices. LCI makes no other warranty. expressed or implied, including merchantability and fitness for
particular purpose. WARRANTY IS NOT VALID UNTIL PAID IN FULL
Collateral/Unforeseen Damages - Should property owner become aware of damage to said property by LCI, its subcontractors, agents or employees during
the course of installation of any trade dem, said damage should be immediately brought to the attention of LCL if owner fails to notify LCI of said damage
within 5 days of occurrence, owner shall waive all rights against contractor concerning said damage LCI is not responsible for damage to driveways or
sidewalks during delivery of materials or construction. Further. LCI is not responsible for ANY interior damage caused during construction This includes but is
not limited to nail pops or cracks in drywall broken light fixtures, damaged personal property or any other interior damage that is a result of improper initial
installation, deterioration or poor overall interior material quality.
Payments - Upon material delivery, Owner will provide LCI with the Actual Cash Value (ACV) portion of the scope of loss in addition to their policys
deductible Upon completion of work and final settlement of damages with the insurance carrier. LCI will provide the Owner(s) with an itemized invoice These
remaining proceeds will be dile to LCI within 5 days of the funds being available from the insurance carrier or mortgage holder. Property owners out-of-pocket
expense will not exceed Owner(s) insurance deductible for work agreed upon between the insurance camer and LCI unless upgrades or change orders are
approved during installation Payments more then 10 days past due shall bear interest at the rate of 10% per month or any portion thereof, from the due dale
until paid. ALL CHECKS SHALL BE MADE PAYABLE TO: LALLIER CONSTRUCTION, INC.
VENTING AND CARBON MONOXIDE WARNING Any construction project that operates near or around HVAC facilities should be dealt with extra care.
Lallier Construction, Inc. suggests a Licensed HVAC Technician impact and certify that no Carbon Monoxide leak has occurred during the construction of your
new roofing system Contractor cannot and will not conduct these services for the customer and will not bill nor be responsible for the charges that are
incurred by the customer. Contractor suggests that as a matter of consumer safety, customer purchase and install a Carbon Monoxide Detector in their home
per Manufacturer Specifications. Lather Construction. Inc shall not be held liable for any injury or death related to a Carbon Monoxide leak in the home due to
work performed
NOTICE OF CANCELLATION -Notice With Respect to Residential Property Located in Colorado as applicable under C.R S. 6-22-101 et seq.: Property
Owner may rescind this Contract Pursuant to Section 6-22-104 Colorado Revised Statutes
- If you are notified by your insurer that all or any part of the claim or contract is not a cowered loss under the insurance policy, you may cancel the contract by
mailing or delivering a signed and dated copy of this cancellation notice or any other written notice to LALLIER CONSTRUCTION, INC. at 5665 Gray Street
Arvada CO 00002 any time prior to midnight on the 5th business day after you have received such notice from your insurer. K you cancel, any payments made
by you under the contract, except for certain emergency work already performed by the LCI, work already in progress or materials delivered to the job site. will
be returned to you within ten business days following receipt by the contractor of your cancellation notice.
o In line event that Insurance Company only agrees to pay for repairs, neither party is bound by this contract. Customer agrees to provide a copy of Insurance
Claim Report to LCI upon receipt A cancellation fee of 20% will be charged if customer cancels contract after insurance company has agreed to replace the

Miscellaneous - In the event of default by Owner, or legal fees are incurred for the collection of any sums due under this contract, Owner shall be responsible
for at attorneys' fees, costs and expenses of collection LCI may file a supplemental claim with owners insurer as a result of material or labor price increases
due to market conditions or errors/omissions in the initial insurance scope of work. LCI shall have the right to assert a mechanics lien for any sums due for the
improvements to the property but unpaid
Failure of this contractor to pay those person supplying material or services to complete this contract can result in the filing of a mechanic's lien on the property
which is the subject of this contract. To avoid this result you may ask this contractor for'Lien Waivers' for the work described in this contract once paid
Unconditional Lien Waivers can be provided once final payment has been received. cleared and applied to all outstanding protect balances.
