In order to pass a final inspection on commercial elastomeric or similar type roofing, a
letter of inspection and approval from the manufacturer technical representative stating
that "the application of the roof at (project address) has been applied in accordance with
the installation instruction for (roof material brand name) roof covering" is required to
be on site at the time of final inspection.
Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) is required on the entire roof

Dear Mr. Berkowitz,
Family Environmental Compliance Services, Inc. (Family Environmental) is pleased to submit this report
summarizing the inspection activities conducted at the building located at 8235 West 44"' Avenue, Wheat
Ridge, Colorado 80034 (the Site). Family Environmental was contracted by Mr. Berkowitz (the Client) on
April 25, 2017 to inspect and identify suspected Asbestos -Containing Material (ACM) in the limited areas
of the upstairs storage closet and downstairs women's restroom, collect and submit samples for laboratory
analysis, and provide an inspection report. Fieldwork was performed on April 25`h, 2017.
Data gathered during the visual inspection of the Site, collection of bulk samples of inspected suspect
building materials, and the interpretation of analytical results is intended to provide information that meets
federal, state, and local regulatory requirements. The sampling and inspection process was conducted in
accordance with the following standards:
• Asbestos Hazard Emergency Response Act (AHERA) — 40 CFR 763 Subpart E sections 763.80 to
763.99
• Colorado Department of Public Health & Environment (CDPHE) Regulation Number 8 (Reg. 8)
• United States Environmental Protection Agency (EPA) National Emissions Standards for
Hazardous Air Pollutants (NESHAP) — 40 CFR 61 Part M
• Occupational Safety and Health Administration (OSHA) asbestos regulations — 29 CFR 1926 parts
58 and 1101, and 29 CFR 1910 Paris 134 and 1001
1.0 REGULATORY COMPLIANCE
This limited inspection was performed in anticipation of renovation operations that are expected to impact
suspected ACM. Asbestos has been added to over 3,000 materials and products produced and used in the
United States. According to AHERA and CDPHE Reg. 8, suspect ACM must be properly sampled and
submitted for analysis before any disturbance occurs at the Site.
For a building material to be categorized as ACM, it must contain asbestos in a quantity greater than one
percent (>1%), as confirmed by an EPA accredited National Voluntary Laboratory Accreditation Program
(NVLAP) laboratory. The EPA and OSHA distinguish between Friable and Non -Friable forms of ACM.
Friable building materials can be pulverized, crumbled and/or reduced to powder by hand pressure when
dry. Non -friable building materials cannot be pulverized, crumbled and/or reduced to powder by hand
pressure when dry. Friable building materials are more likely to be released into the air, especially if
impacted during normal use (i.e. frequency of contact, mechanical vibration and air erosion), demolition
and renovation. Therefore, the distinction between friable and non -friable ACM is crucial to the proper
management of these building materials. The EPA further classifies non -friable ACM into Category 1 and
Category II. Category I non -friable building materials include packings, gaskets, floor tiles, mastics and
roofing materials and/or tar impregnated felts (i.e. interstitial concrete flooring/foundation membranes).
Removal of these ACM are not required prior to demolition and/or renovation as long as they remain in
good condition, are not friable or are not rendered friable (i.e. cutting, grinding, sanding, sawing, etc.)
utilizing proper engineering controls (i.e. wet methods and negative pressure). Category H non -friable
building materials are all other ACM and removal may be required prior to demolition and/or renovation if
those building materials will be rendered friable and therefore considered a regulated asbestos containing
material (RACM).
ACM identified in this report are subject to the EPA NESHSAP regulatory requirements for asbestos and
the CDPHE Reg. 8. The CDPHE is responsible for the administration of the EPA NESHAP program in
Colorado. EPA NESHAP prohibits the release of asbestos fibers into the atmosphere during demolition
and/or renovation operations. According to OSHA, the EPA and the CPDHE, building samples with
asbestos concentrations greater than one percent (>I%) are classified as ACM and are considered a RACM
as well. The asbestos NESHAP requires that potentially regulated ACM be identified, classified, quantified
and removed where and when necessary prior to disturbance. All friable ACM as well as non -friable ACM
that can be rendered friable (RACM) during demolition and/or renovation operations must be removed by
a certified licensed General Abatement Contractor (GAC).
If a building, structure, facility or facility component is scheduled for demolition or renovation friable
samples that contain Trace amounts of asbestos or one percent or less (<I%) are also subject to OSHA
regulatory compliance (i.e. personal air monitoring and proper PPE) if the air concentrations sampled
during operations are at or above the permissible exposure limit (PEL) of 0.10 fibers per cubic centimeter
(f/cc) as averaged over an eight hour time weighted average (TWA), or the excursion limit of 1.0 f/cc over
a 30 minute TWA.
OSHA regulations for General Industry (29 CFR Part 1910.1001) and the Construction Industry (29 CFR
Part 1926-1101) include provisions for communication of hazards by the building owner, to building
employees, or in the case of subcontracted building services, the subcontractor to subcontractor employees:
• The employer must inform maintenance staff and subcontractors if building components have the
potential to expose staff and subcontractors to asbestos.
The building owner shall maintain records concerning the presence, location and quantity of ACM
in the building for the duration of ownership, and records shall be transferred to any successive
owners.
• The building owner shall affix labels or signs to positively identified ACM so that employees and
building visitors will be notified of materials that contain asbestos. Such labels or signs shall be
placed in areas where employees are likely to impact ACM as a part of their work.
2.0 SITE / BUILDING INFORMATION
The subject building is located at 8235 W. 44"' Avenue, Wheat Ridge, Colorado 80034. This was a pre -
renovation asbestos building survey of the limited areas of the upstairs storage closet and the downstairs
women's restroom. The only suspect ACM to be impacted during renovation was the drywall in these
limited areas. Drawings of the existing conditions were not provided to Family Environmental prior to the
inspection and sampling activities. The inspected areas are detailed in the drawings provided in Appendix
B.
During the inspection, two functional spaces were identified and sampled:
o Upstairs storage closet
o Downstairs women's restroom
3.0 FIELD ACTIVITIES
On April 25t", 2017, an AHERA/CDPHE accredited asbestos inspector from Family Environmental
conducted a limited asbestos inspection of the upstairs storage closet and downstairs women's restroom at
the Site. The purpose of this asbestos inspection was to evaluate for the presence of ACM in the areas to
be impacted by renovation activity in the limited areas sampled. The inspection was conducted in
accordance with the methods and procedures outlined in the aforementioned regulatory requirements.
3.1 Visual Assessment
Inspection activities began with visual observation of interior and exterior of the shop to identify
homogeneous areas for suspect ACM. Homogenous areas (materials) are classified into one of the
following three categories:
Surfacing Material (S) refers to a wide range of trowel or spray -applied materials
typically used for acoustical, decorative or fire proofing purposes;
Thermal System Insulation (TSI) refers to insulation that is applied to heating or
mechanical system components. Examples include pipe, tank and boiler insulation;
Miscellaneous Materials (M) refers to refers to all other materials that do not fall into one
of the previously mentioned categories. Examples include floor tiles, adhesives, composite
drywall systems and ceiling tiles.
Appendix A provides a summary of the areas determined to be homogeneous and the laboratory
results for those areas.
3.2 Physical Assessment and Sample Collection
A tactile assessment of each homogeneous area of suspect ACM was conducted to assess the
general condition and the friability of the material. Friability was assessed by physically touching
suspect materials. Following categorization for material type and friability, a sampling strategy

Based on the results of the visual observation, bulk samples of suspect ACM were collected in
accordance with AHERA protocols. Random bulk samples of suspect materials were collected in
each homogeneous area using wet methods as applicable to reduce the potential for fiber release.
During the inspection, Family Environmental staff identified one suspect homogenous material,
and collected five bulk samples.
• Composite Drywall (CDW): One unique area of homogenous CDW was identified during
this inspection and five samples of the material were obtained. Laboratory analysis
determined that all five samples of drywall obtained during this inspection was non -detect
for asbestos.
Each sample was placed in a sealed container and labeled with a unique sample number using an
indelible marker. Appendix C contains the Laboratory Analytical Summary of all the bulk samples
collected.
3.3 Chain of Custody
Samples were delivered along with a completed chain -of -custody (COC) to River North
Environmental Testing (RNET) Laboratory. Laboratory personnel signed and dated the COC at
the time of delivery to acknowledge receipt of the samples. Upon receipt, the laboratory arranged
the samples in numerical order to establish any discrepancies between the sample set which was
delivered and the COC form. Where (if) discrepancies exist, laboratory personnel noted and
initialed the COC and reported the error to the inspector. The laboratory included the signed
original copy of the COC with the final analytical report for evidentiary purposes.
4.0 LABORATORY ANALYSIS
All bulk samples were relinquished to RNET. All samples were analyzed via Polarized Light Microscopy
(PLM) in accordance with EPA/600/R-93/ 116 Method for the Determination ofAsbestos in Bulk Building
Materials. RNET is accredited by the National Voluntary Laboratory Accreditation Program (NVLAP)
under Laboratory Code 200448-0. A copy of the RNET Report of Test Results can be found in Appendix
C of this report.
5.0 ASBESTOS CONTAINING MATERIALS
The United States EPA and OSHA define an asbestos containing material as a material containing greater
than 1% asbestos, or a material that is assumed to contain asbestos. Materials that contain asbestos at or
below one percent (<I%) are considered to be trace asbestos containing.
Current EPA and State of Colorado regulations require the removal of both confirmed, and assumed,
FRIABLE ASBESTOS prior to renovation or demolition activities that will impact these materials. The
following is a list of friable ACM observed by the inspector:
• No friable asbestos was found during this inspection.
Depending on the renovation or demolition techniques used, the current EPA and State of Colorado
regulations may require the removal of both confirmed, and assumed, NON -FRIABLE ASBESTOS prior
to any activities that may impact these materials. The following is a list of non -friable ACM observed at
the site:
• No non -friable asbestos was found during this inspection.
Prior to any disturbance, the following ASSUMED ASBESTOS (ACM) should be sampled by a certified
asbestos building inspector to determine the actual asbestos content, or be treated as asbestos containing:
• No materials were assumed during this inspection.
Current EPA and State of Colorado regulations do not require the removal of materials found to contain
TRACE ASBESTOS (<I%) prior to initiating renovation or demolition activities that will impact this
material. The following materials contain trace amounts of asbestos:
• No trace materials were found during this inspection
6.0 CONCLUSIONS & RECOMMENDATIONS
Based on our observations and the analytical results, asbestos was not detected in the limited areas of
composite drywall sampled in the upstairs storage closet and the downstairs women's restroom.
If during demolition/renovation activities, materials are encountered which have not been discussed in this
report; stop all activities in the immediate area, and contact a certified asbestos building inspector to sample
the suspect material. Family Environmental is available to perform further sampling at your request.
7.0 LIMITATIONS
This asbestos inspection was conducted in a manner consistent with the level of care and skill ordinarily
exercised by members of the profession currently practicing under similar conditions in the same locale.
The results, findings, conclusions and/or recommendations expressed in this report are based on conditions
observed during our limited inspection, and the analytical results of the samples collected. The information
contained in this report is relevant to the date on which this survey was performed, and should not be relied
upon to represent conditions at a later date. This limited asbestos inspection report is not considered a
comprehensive inspection report and it must be understood that the existence of unique or concealed
asbestos may be possible.
This report has been prepared on behalf of, and exclusively for use by the client for specific application to
the project indicated. Contractors or consultants reviewing this report must draw their own conclusions
regarding further investigation or remediation deemed necessary. Family Environmental does not warrant
the work of regulatory agencies or other third parties supplying information which may have been used in

Bulk samples are analyzed in accordance with USEPA test method EPA/600/M4'82-020 and EPA/600/R-
93/116, which requires that distinctly different materials or layers present within a sample be analyzed and
reported separately. (For measurement uncertainty, refer to table 2.1 in EPA/600/R'93/116 method.)
Sometimes it is not possible to completely separate thin or strongly adhering layers, such as paint or
adhesive, and a combined result is given. For some onrnp|ex materials, such as drywall/tape/mud joints, a
composite oroverall asbestos content may be reported in add/Mon to individual layer results. RNET, Inc. is
currently accredited for bulk asbestos analysis by the National Voluntary Laboratory Accreditation Program
(NVLAP) of the Nationa| Institute of Standards and Technology (NIST). Our NVLAP laboratory code number
is 200448-0.
Due to the limitations of the EPA 600 Method, nonhiab|e organically hound materials (NO8s) such as Moor
tiles can bedifficult to analyze via PLM. EPA recommends that all NOBs reported as non -detect by PLM
analysis befurther analyzed by Transmission Electron Microscopy for more definitive results. Please note
that the PLM analysis of dust and soil samples for asbestos is not covered under NVLAP accreditation.
The analytical results provided in this report apply only to the samples submitted to the laboratory. This
report is confidential. Details of this report will not be discussed with any person or agency not associated
with you or your organization. This report must be reproduced in its entirety and shall not be copied in part
or used by the client to claim product endorsement by NVLAP or any agency of the U.S. Government.
