In response to your request, Mr. Todd Bernshausen of our office conducted construction observations
for the above referenced home building construction project to review and verify the construction of
the basement foundation items as noted below:

completed in a workmanlike manner in accordance with the intent of the approved project plans and
or/ the city of Denver building code requirements. Our observations are intended only to verify
general conformance with the intent of the approved project plans.

Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding one half inch exists. Asphalt shingles are
required to be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and
water shield is required. Eave and rake metal is required. A ladder extending 3 feet above
the roof eave and secured in place is required to be provided for all roof inspections.
Roof ventilation is required to comply with applicable codes and/or manufacturer
installation instructions, whichever is more stringent. In order to pass a final inspection
of elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer technical representative stating that "the application of the roof at (project
address) has been applied in accordance with the installation instruction for (roof
material brand name) roof covering" is required to be on site at the time of final
inspection.
Roof inspections for ROOFS OVER 6/12 PITCH: 3rd party inspection will be required, the 3rd
party inspection report will be collected at final roof inspection performed by City of
Wheat Ridge. If report is not available the final inspection will not be completed.

This application is exclusively for new permits for residential roofs and for licensed
contractors only. This type of permit is ONLY being processed online --do not come
to City Hall to submit an application in person. Permits are processed and issued in
the order they are received and due to the volume of requests, time to process
varies and is subject to change. YOU WILL BE CONTACTED WHEN YOUR
PERMIT IS READY FOR PICK-UP AND WILL BE GIVEN A SPECIFIC DATE

fraud (CR518.13.219.5) states that paying, waiving, or rebating a property

company does not have to consider the estimate from any contractor violating the
provision, and the property owner or the insurer may sue the contractor for any

materials that a contractor may have used in order to offset the cost of the
deductible, etc. Recoverable damages would be case specific. (2) A contract
lacking COS838's specific written provisions may be found to be unenforceable by

PAYMENT: In the case that roofing is accompanied by other interior or exterior
repairs, the roofing portion of this contract is due in full as soon as roof is complete.
Late Payment/ Interest: In the event of non-payment of any sums due

project described, CUSTOMER agrees to pay interest on such sums from the date
due as described until paid to CONTRACTOR at the rate of twenty-one percent
(21%) above annum as well as any and all costs of collection incurred by
CONTRACTOR, including but not limited to: Reasonable attorneys fees whether or
not suit is brought to affect such collection.
Exclusions: This contract excludes the following (a) Any work not specifically listed
on the front of this contract; or (b) any additional work that maybe ordered by the
building department having jurisdiction over the site of this project.
Certificate of Substantial Completion: As part of consideration for the work of
CONTRACTOR on this project, CUSTOMER agrees, upon the request of
CONTRACTOR to execute a Certificate of Substantial Completion in the form
submitted by CONTRACTOR upon substantial completion of the project herein
described.
Force Majeure: No failure nor commission by either party in the performance of
any obligation pursuant to this acceptance shall be deemed a breach of this
Acceptance or create any liability if the same shall arise from any cause beyond the
reasonable control of a party, including, but not restricted to, acts of nature, fire,
storm, flood, earthquake, strikes or other work stoppage, war, difficulties from any
energy shortage, raw material shortage, governmental acts or guidelines, or other
causes beyond the control of CONTRACTOR. A party whose performance is affected
by said conditions shall promptly inform the other of the details of such cause and
shall endeavor to rapidly remedy the cause, if within their ability to do so, and
resume its obligation as soon as possible after such cause shall have been removed.
Permits: CONTRACTOR will be responsible for obtaining all necessary and required
state, county, city and local building permits.
Governing Law/Choice of Venue: This acceptance and the legal relations between
CONTRACTOR and CUSTOMER shall be construed in accordance with the laws of
Colorado. CUSTOMER'S signature warrants that for purpose of determining venue
for bringing any legal action on the account that venue shall be either DENVER or
ADAMS county at the election of CONTRACTOR.
Waiver/ Severability: Any delay, waiver or omission by either party to exercise any
right or power arising from breach or default by the other party of the terms,
provisions or covenants of the Acceptance shall not be construed, except as herein
oth=erwise set forth, as a waiver by either party of subsequent breach or default of
the same or other terms provisions or covenants of the other party. Should any
provision hereof be deemed contrary to the laws of Colorado such provision shall
be deemed severable from the acceptance and not affect the enforceability of the
balance of this Acceptance.
Warranty: Work includes a three (3) year warranty on labor and workmanship
unless stated otherwise in the Contract. Warranties on materials furnished by
CONTRACTOR are limited to the manufacturer's warranty and do not cover abuse,
neglect alterations or damages caused by others.
(15) days of the earlier of: (a) receipt by CONTRACTOR of a Certificate of Substantial
Completion; of (b) CONTRACTOR advises CUSTOMER it has completed the
Project/Work described in the aforesaid contract, then the Project and the work
performed in conjunction therewith shall be deemed to have been unqualifiedly
accepted by CUSTOMER, and the CUSTOMER has waived any and all claims with
respect to said Project/Work and material and all performance by CONTRACTOR of
this contract. No such claims against CONTRACTOR of any kind shall be valid except
those raised in writing by CUSTOMER during performance of this contract as set forth
herein. On no account shall CONTRACTOR be liable to CUSTOMER for consequential
damages and liability of CUSTOMER as to any claim for loss or damages arising out of,
connected with or resulting from the work performed by CONTRACTOR and in no
case shall any liability of CONTRACTOR exceed the price of the project to which the
claim relates.
Premise Availability: CUSTOMER will make the property upon which the Work is to
be performed available to CONTRACTOR from 7:30 a.m. until 5:30 p.m. Monday
through Friday and all other agreed upon times as modified and agreed to in writing
on page (1) of this contract. CUSTOMER understands that roofing activities will result
in elevated noise levels and debris. CONTRACTOR will insure that the work area will
be cleaned up upon final completion of the work as outlined in this contract.
Materials: If materials for this Project/Work are ordered and cancelled before work
on the Project commences, CUSTOMER is responsible for any restocking costs and
inbound and returned shipping charges to CONTRACTOR. If materials are specially
ordered to CUSTOMER'S specifications, CUSTOMER is responsible for the entire value
of the order as soon as production of said materials begins. CONTRACTOR will retain
title to any materials that may be furnished until final payment is made and if
settlement is not agreed to, CONTRACTOR will be held harmless for any damages
resulting from the removal thereof. CONTRACTOR will retain title to all unused
materials upon completion of the work.
Assignment and Delegation: The CUSTOMER may riot assign any rights or interest to
this Contract without the prior written consent and approval of CONTRACTOR. No
delegation of any obligation owed to CONTRACTOR nor the performance of any
obligation of CUSTOMER to CONTRACTOR shall be made without CONTRACTOR's
prior written consent
Indemnification: CUSTOMER agrees to indemnify and hold CONTRACTOR harmless
from any and all costs (including attorney's fees), expenses and damages resulting
from any claim brought against CONTRACTOR by virtue of any negligence or fault of
CUSTOMER including, but not limited to, claims of personal injury.
Entire Agreement/ Amendments: This acceptance constitutes the entire final
binding agreement between CONTRACTOR and CUSTOMER with respect to the
project (i.e. work and materials) itemized on page 1 of this form and supersedes any
and all prior orders and agreements between the parties, either verbal or written.
Any alteration or deviation from the provisions on the front of this contract will be
valid only if made upon written Change Order signed on behalf of CUSTOMER and
CONTRACTOR. and will be subject to extra charges over and above the amount set
forth in the Contract. This Contract shall become a contract between CONTRACTOR
and CUSTOMER upon signature by CUSTOMER and is subject to the Force Majeure
provisions hereinabove set forth. Copies of this document disclosing signatures of
the parties shall have the legal effect of any original. This Contract may be executed
in counterparts with the same force and effect as though all signatures appeared on
one original document. Facsimile signatures are binding and enforceable.
Tearoff Clause: CONTRACTOR will exercise reasonable care and diligence
in the removal of the existing materials. We strongly advise customer to
remove sensitive wall hangings, ceiling suspended articles, to cover furniture
and otherwise protect their property since CONTRACTOR cannot be
responsible for replacement or repair of these items. CONTRACTOR cannot,
unless otherwise specifically stated, be responsible for replacement or repair
of associated areas such as fascia, gutter, siding, trim, old flashings or any
previously damaged or deteriorated building components during the normal
tear off procedure, CONTRACTOR'S employees' negligence excluded.
CONTRACTOR cannot accept responsibility for any damages done to the roof
by CUSTOMER, other tradesmen or any others not directly employed by
CONTRACTOR.CONTRACTOR is not responsible for any damages which
occur to owner's property because of defects in architectural, engineering,
construction plans and drawings or with respect to any failure to comply with
any applicable building codes through construction occurring under such
plans. CONTRACTOR is specifically not performing design services and is
specifically not responsible for damages caused by occurrences such as mold,
mildew, condensation, loose flue pipes or other similar occurrences. To the
maximum extent allowed by law, all work performed by CONTRACTOR is
without specific express or implied warranty

VENTILATION CALCULATION (RE -ROOFS ONLY):
IMPORTANT: Failure to provide proper attic/under roof deck ventilation may result in the premature aging
of the roofing system, warping, cracking or breakdown of wood framing, damage to siding, exterior or
interior paint and wallpaper. In addition, roof deck (plywood) warping and rotting, mold and mildew growth,
buckling of felt and winter ice dams — are often the result of inadequate ventilation. Inadequate ventilation
could void the roofing manufacturer's warranty. Proper attic/under deck ventilation systems allow a
continual flow of outside air through the attic/truss spaces, protecting the efficiency of the insulation and
helping to lower temperatures in the living space. It consists of a balance between air intake (at eaves or
soffits) and air exhaust (at or near the roof peak).
