Effective December 1, 2014, asphalt shingle installations require an approved midroof
inspection, conducted when 25-75 percent of the roof covering is installed, prior to final
approval. Installation of roof sheathing (new or overlay) requires an approved inspection
prior to installation of ANY roof coverings and is require on the entire roof when spaced
or board sheathing with ANY gap exceeding k/2 -inch exists. Asphalt shingles are required to
be fastened to the roof deck with a minimum of 6 nails per shingle. Ice and water shield is
required. Eave and rake metal is required. A ladder extending 3 feet above the roof eave
and secured in place is required to be provided for all roof inspections. Roof ventilation
is required to comply with applicable codes and/or manufacturera€ms installation
instructions, whichever is more stringent. In order to pass a final inspection of
elastomeric or similar type roof coverings, a letter of inspection and approval from the
manufacturer's technical representative stating that "the application of the roof at
(project address) has been applied in accordance with the installation instruction for
(roof material brand name) roof covering" is required to be on site at the time of final
inspection.

BEC Engincering, Inc. (BEC) hes reviewed the sketch and tLe Cable Lock Tu foundation
repair system proposed for the subject locadon. In BEC's opinion, the proposed
foundation repair should stabilize the foundadon movement that has been experienced at
the subject location provided proper instailadon teclmiques are used based on the
manufacturer's technical manual / specifications and industry standacds.
The areas that are represented by cross-hatching on the attached sketch are the areas, in
BEC's opinion that will be affected by the proposed foundation repair system, provided
the foundauion system is of acceptable strength. If the actual strength of the foundation
system is found to be inadequate to support the spacing of the piles as shown on the
attached sketch then additional piles may be required to support the foundation.

As requested by Olshan Foundation Repair, we have reviewed the repa'v proposal and installation data
from Olshan Foundation Repair regazding the repairs made to the above referenced residence. Olshan
Foundation Repa'v presented the repaired portion of the foundation using 5 eaRerior segmental pre-cast
Cable Lock'TM concrete piles beneath t6e above referenced localion for our review. The pile locations and
spacing as represented were found to be in generel compliance with industry standards, and generally in
accordance with Olshan Foundation Repair's proposal.
In our opinion, the piling depths in conjunction with the pile driving force as repoRed are generally
appropriate for this type of structure and for the area where the work is being performed. The repa'v work
performed to the subject location is believed to have been acceptably completed, based on the information
provided, in accordance with good industry practice for foundation repair work using pre-cast segmental
piles. The repairs performed on the subject loca6on should be expected to stabilize the foundarion
movement observed prior to the foundation repa'v work. In instances where partial repairs arc perfortned,
meaning the entire founda6on has not been underpinned, potential differential movement may occur. It
should be noted that partial repairs modify the design of the foundation and while partial repairs aze
generally accepted industry pracHce the possibility of future movement should be recognized. Non-
supported areas aze generally not covered for downward foundation movemenu by the contractor's
warranty-
The future performance of the foundation system on tha subject location should function as generally
intended, provided proper soil moisture is maintained and there is not a loss in the load bearing capacity of
the soil beneath the foundation. Soils should be graded such that there is positive drainage away from the
foundation or a drainage system can be installed to prevent water from ponding around the foundation. A
foundation maintenance program is recommended which can be found at www.foundationperformance.ore.
